Today’s visit was completed with Administrator, S. Bryant. Kimberly Barnes, Child Care Consultant assisted with today’s visit. Currently this center operates with a Five (5) Star License issued December 16, 2025, utilizing Pathway 3, Head Start Accreditation. The program is licensed to serve 117 children ages 1-5 years of age. This program consists of seven (7) classrooms. The program is continuing to meet reduced enhanced ratios and enhanced space. The last sanitation inspection was completed on February 18, 2026, earning a “Superior” classification. The last fire inspection was completed on March 24, 2026, and the center was approved for daytime care only. The last Annual Compliance visit was completed on December 16, 2025. Prior to today’s visit, the program’s compliance history was 95% over the current 18-month period. A copy of your compliance history report was left with you. Two substitutes were present on today’s visit. Those staff files were monitored today for compliance and some additional staff files. Greene Lamp is listed as the current owner of this center. According to information verified on the North Carolina Secretary of State’s website, Greene Lamp remains Current and Active. Prior to making any changes in the corporation status, please contact me to discuss change of ownership procedures. The corporation status for your program must always remain Current and Active. Failure to comply may affect your child care license. You stated the mailing address, phone number and email address listed for your child care center are correct. Children were observed participating in free play in activity areas, playing outdoors, interacting with caregivers during group time, completing routines, and completing transitions. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. Violations documented were reviewed with you at the conclusion of the visit. A copy of this computer-generated visit summary was discussed and a copy was left with you. The following violations were documented: Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. A thermometer was not available to determine a temperature of 45 degrees or below. 15A NCAC 18A .2806(j)(2) 807 A safe indoor and outdoor environment was not provided for the children. In Space G6 a closet which contained staff members purses was not locked and a fan was located on top of a shelf not 5 ft off the floor. In Space G4 a glass window is cracked. The kitchen door located in the modular was unlocked. On the 1 and 2 year old playground the underpinning was broken and damaged exposing shard edges. On the 2 and 3 year old playground the black surfacing borders were split causing pinching hazards. 10A NCAC 09 .0601(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Power strips in Space G6 and G5 were missing safety plugs. 10A NCAC 09 .0604(c) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Staff members T. Brooks and H. I observed children participating in routines, group activities, free indoor and outdoor play, and completing transitions. All interactions were observed to be positive and nurturing and adequate supervision was provided to all children in care. A checklist was used to note the requirements I monitored today. The staff/training worksheets were received and monitored during the visit. Today’s visit was completed with V. Kornegay, Administrator. The last annual compliance visit was conducted May 8, 2024. The last sanitation inspection was completed on February 25, 2025, with a “Superior” classification. The last fire inspection was conducted on March 25, 2025, and the program was approved for day time care only. Prior to today’s visit, the program’s compliance history was 99% over the current 18- month period. A copy of your compliance history report was left with you. The center is owned and operated by Greene Lamp, (non-profit corporation) with a current-active status based on today’s research in the North Carolina Secretary of State database. This corporation owns the center, which means a Current/Active status must always be maintained. I observed children participating in routines, group activities, free indoor and outdoor play, and completing transitions. All interactions were observed to be positive and nurturing and adequate supervision was provided to all children in care. The visit summary was reviewed with you at the conclusion of the visit and a copy was provided to you. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements.
