A checklist was used to note the requirements I monitored today. The staff and training worksheets were submitted prior to the visit. Kim Barnes, Child Care Consultant, assisted with today’s visit. Currently this center operates with a Five Star License, issued December 12, 2018, earning six (6) points in the Education Component, six (6) points in the Program Component and one (1) Quality point based on the program having a staff benefits package and an infrastructure of parental involvement. You, M. Aldridge, Administrator, were present and available for consultation at the conclusion of the visit. S. Wolmarans, the second Administrator, was also present and assisted with today’s visit. Children were observed participating in group activities, free play in centers, completing transitions, completing routines such as handwashing and toileting and participating in outdoor free play. The teachers supervised and interacted with the children in a positive manner. Infants were observed participating in tummy time, interacting with caregivers, and napping. Sleep checks were in compliance with child care requirements. The last Annual Compliance visit was completed on April 1, 2025. The last sanitation inspection was completed today, March 19, 2025, with a “Superior” classification. The last fire inspection was completed May 6, 2025. This facility is owned by Farmer-N-Dell Learning Center, LLC (Limited Liability Company). As of today, the status is listed as “Current-Active” on the NC Secretary of State website. Prior to any changes with the corporation, please contact me to discuss change of ownership procedures. Prior to today’s visit, the facility has earned a Compliance History score of 98% over an 18 - month time- period. A copy of the compliance history was provided to you. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. The computer-generated visit summary was reviewed with you, including all documented violations, and a copy was provided to you. Today’s visit was completed with both administrators, M. Aldridge and S. Wolmarans. You were notified of the purpose of the visit, and you were available for consultation at the conclusion of the visit. Currently this center operates with a Five Star License, issued December 12, 2018, earning six (6) points in the Education Component, six (6) points in the Program Component and one (1) Quality point based on the program having a staff benefits package and an infrastructure of parental involvement. Six (6) new staff members have been hired since your last Annual Compliance visit completed on April 2, 2024. Although a staff/training worksheet was not available for the new staff members, the personnel files were monitored and were found to be in compliance. Children were observed participating in group activities, free play in centers, completing transitions, and completing routines such as handwashing and toileting. There were adequate staff supervising the enrolled children. The teachers supervised and interacted with the children in a positive manner. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. Due to technical issues, a one-page visit summary was reviewed, and a copy was provided to you. A computer-generated visit summary will be emailed to you. RESOURCES: QRIS MODERNIZATION: PATHWAYS TO THE STARS: North Carolina led the nation in 1999 when it launched its Quality Rating and Improvement System (QRIS), a star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality. It encourages early care and learning programs to engage in continuous quality improvement, and families may use the star rating to make informed decisions when choosing a child care facility. Listening to and communicating with early childhood professionals, families, communities and businesses is a critical component to ensure a successful QRIS modernization. The Commission began the modernization process by seeking public comment, and the Commission invites and welcomes public comment as we move towards finalizing rules. Some QRIS Modernization changes are still in draft form. Name of Operation: FARMER-N-DELL LEARNING CENTER, LLC Facility ID: 54000328 Consultant: TINA JONES Operation Type: Center Case Number: 0824-179L Visit Date: 8/16/2024 Number Present: 147 Completed Date: 8/16/2024 Age: From 0 To 11 Total Minutes: 150 Time In: 09:00 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of this unannounced visit was to investigate a report alleging violations of child care requirements regarding adequate supervision. The allegation report was reviewed with you, Michelle Aldridge, Administrator. You were given the opportunity to discuss the allegations and ask questions. An additional staff member was interviewed, and they were given the opportunity to discuss the allegations and ask questions. One hundred and forty-seven children ages infant to five (5) years of age were present. Children were observed participating in free choice activities in the outdoor and indoor environment, completing transitions, and completing routine care needs. I observed children in the classroom for infants interacting with caregivers, participating in tummy time, and completing routine care needs. Enrollment was not completed. Prior to today’s visit, the compliance history score was 97%. A copy of the compliance history report was provided to you. FINDINGS/OBSERVATIONS: Limited but required sections of child care requirements were monitored. Supervision, staff/child ratio, adequate approved space, and permit restrictions were monitored. Allegation 1: There is a concern that a child was adequately supervised. When speaking with M. Aldridge, it was stated that on Monday, August 12, 2024, a five (5) month old infant was taken to the doctor due to an infant rubbing his face on a boppy pillow and the carpet while participating in tummy time. It was stated that the child has a severe case of eczema. It was stated that the parent of the infant is very peculiar of the child and has stated that “no one can take care of her children the way she can”. Ms. Aldridge stated that the infant was able to hold his head up. When speaking with both teachers, it was stated the child was placed on his stomach, propped up on a boppy pillow. The child slipped off the boppy pillow causing his face to be in contact with the carpet, along with the boppy pillow. One teacher stated that the child has a previous spot on their nose prior to Monday’s incident. There were two teachers in the classroom with seven (7) children present. During the time of the incident, one teacher began changing a diaper at the time the child was placed on the boppy pillow One teacher was feeding an infant. It was stated that the infant did not like tummy time. During tummy time, the infant began rubbing his face back and forth across the pillow. The infant became agitated and began crying. The teacher feeding an infant noticed some blood on the pillow but did not pick up the infant on the pillow. During this time of agitation, the infant slid off the pillow, causing the infant to be in contact with the floor. Both teachers stated that although they saw the infant agitated, crying, and no longer on the pillow, they did not pick up the infant until after one teacher completed the diaper change. After completing the diaper change, the teacher immediately picked up the child on the floor. When the child was picked up from the floor, the parent entered the classroom. An incident report was not completed at the time of the incident due to the parent picking up the child and leaving. A meeting was requested by the parent for Tuesday, August 13, 2024. During this time, an incident report was not provided to the parent. The parent received an email with the incident report on Wednesday, August 14, 2024. The parent did not have the opportunity to sign the incident report. The child received medical treatment at the pediatrics. I received a copy of the unsigned incident report today. The facility does have video footage of the incident; however, the footage was not available during the visit. Based on my observations and the information provided, the allegation regarding adequate supervision is unsubstantiated. Due to connection issues, a one-page visit summary was reviewed, and a copy was provided to you. All violations were reviewed with you. A computer-generated visit summary will be emailed to you. Although the allegation of supervision is unsubstantiated, the facility received the following child care requirement violations: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not presented to the parent to sign. The incident report was emailed to the parent two days after the incident occurred. .0802 (e) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. An infant became agitated during tummy time. The child began to cry. The child was not attended to when he began rubbing his face on a boppy pillow and the floor which caused abrasions on their face. G.S. 110-91(10) 1417 The equipment and materials were not age-appropriate for the children in care. An infant was placed on a nursing pillow during tummy time. The infant slid off the nursing pillow due to the size of the pillow. 10A NCAC 09 .2508(d) CONSULTATION AND TECHNICAL ASSISTANCE: The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by August 30, 2024, describing how and when the violations were corrected. In the corrective action plan you must: -Explain how staff will provide responsive caregiving. -Explain what steps will be taken to ensure a safe and nurturing environment is provided for all enrolled infants. -Explain how the classroom environment will support the development of infants. -Provide trainings to staff members of children under 12 months of age that includes responsive caregiving, communication, and nurturing care. -Develop a plan for staff members that need additional assistance when completing routine care of infants. AGE-APPROPRIATE ACTIVITIES: Child care rule 10A NCAC 09 .0510 (e) (6) states each child under the age of 12 months shall be given supervised tummy time positioned on his or her stomach while awake and alert each day. We reviewed the use of boppy pillows. The boppy pillow that is being used in the classroom for infants is a nursing pillow. Infants should not be propped, or they should not lie face down on the pillow. Please visit the website https://www.boppy.com/pages/original-nursing-support-safe-use-and-product-care to review the warnings of the nursing boppy pillow. During the visit, you removed all nursing boppy pillows. For tummy time: Set-up a soft, safe space and lay baby down. A blanket or a tummy time mat on a firm, flat surface works well. The floor is an ideal spot, though you can also lay your infant tummy down on your stomach or chest or across your lap. INCIDENT REPORTS: Ensure that incident reports are completed each time a child is injured as a result of an incident occurring while you are providing care. The person completing the report shall sign the report and the parent must sign the report. The parent must initial if they decline to receive a copy of the report. Ensure a copy of the report is maintained in the child’s file. If medial treatments is required, a copy of the incident report must be mailed to your child care consultant within seven calendar days. AGE-APPROPRIATE ACTIVITIES: Child care rule 10A NCAC 09 .0510 (e) (6) states each child under the age of 12 months shall be given supervised tummy time positioned on his or her stomach while awake and alert each day. We reviewed the use of boppy pillows. The boppy pillow that is being used in the classroom for infants is a nursing pillow. Infants should not be propped, or they should not lie face down on the pillow. INCIDENT REPORTS: Ensure that incident reports are completed each time a child is injured as a result of an incident occurring while you are providing care. The person completing the report shall sign the report and the parent must sign the report. The parent must initial if they decline to receive a copy of the report. Ensure a copy of the report is maintained in the child’s file. If medial treatments is required, a copy of the incident report must be mailed to your child care consultant within seven calendar days. The allegation report was reviewed with you, Michelle Aldridge, Administrator. You were given the opportunity to discuss the allegations and ask questions. An additional staff member was interviewed, and they were given the opportunity to discuss the allegations and ask questions. One hundred and forty-seven children ages infant to five (5) years of age were present. Children were observed participating in free choice activities in the outdoor and indoor environment, completing transitions, and completing routine care needs. I observed children in the classroom for infants interacting with caregivers, participating in tummy time, and completing routine care needs. Enrollment was not completed. Prior to today’s visit, the compliance history score was 97%. A copy of the compliance history report was provided to you. FINDINGS/OBSERVATIONS: Limited but required sections of child care requirements were monitored. Supervision, staff/child ratio, adequate approved space, and permit restrictions were monitored. Allegation 1: There is a concern that a child was adequately supervised. When speaking with M. Aldridge, it was stated that on Monday, August 12, 2024, a five (5) month old infant was taken to the doctor due to an infant rubbing his face on a boppy pillow and the carpet while participating in tummy time. It was stated that the child has a severe case of eczema. It was stated that the parent of the infant is very peculiar of the child and has stated that “no one can take care of her children the way she can”. Ms. Aldridge stated that the infant was able to hold his head up. When speaking with both teachers, it was stated the child was placed on his stomach, propped up on a boppy pillow. The child slipped off the boppy pillow causing his face to be in contact with the carpet, along with the boppy pillow. One teacher stated that the child has a previous spot on their nose prior to Monday’s incident. There were two teachers in the classroom with seven (7) children present. During the time of the incident, one teacher began changing a diaper at the time the child was placed on the boppy pillow One teacher was feeding an infant. It was stated that the infant did not like tummy time. During tummy time, the infant began rubbing his face back and forth across the pillow. The infant became agitated and began crying. The teacher feeding an infant noticed some blood on the pillow but did not pick up the infant on the pillow. During this time of agitation, the infant slid off the pillow, causing the infant to be in contact with the floor. Both teachers stated that although they saw the infant agitated, crying, and no longer on the pillow, they did not pick up the infant until after one teacher completed the diaper change. After completing the diaper change, the teacher immediately picked up the child on the floor. When the child was picked up from the floor, the parent entered the classroom. An incident report was not completed at the time of the incident due to the parent picking up the child and leaving. A meeting was requested by the parent for Tuesday, August 13, 2024. During this time, an incident report was not provided to the parent. The parent received an email with the incident report on Wednesday, August 14, 2024. The parent did not have the opportunity to sign the incident report. The child received medical treatment at the pediatrics. I received a copy of the unsigned incident report today. The facility does have video footage of the incident; however, the footage was not available during the visit. Based on my observations and the information provided, the allegation regarding adequate supervision is unsubstantiated. Due to connection issues, a one-page visit summary was reviewed, and a copy was provided to you. All violations were reviewed with you. A computer-generated visit summary will be emailed to you. Although the allegation of supervision is unsubstantiated, the facility received the following child care requirement violations: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. An incident report was not presented to the parent to sign. The incident report was emailed to the parent two days after the incident occurred. .0802 (e) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. An infant became agitated during tummy time. The child began to cry. The child was not attended to when he began rubbing his face on a boppy pillow and the floor which caused abrasions on their face. G.S. 110-91(10) 1417 The equipment and materials were not age-appropriate for the children in care. An infant was placed on a nursing pillow during tummy time. A checklist was used to note the requirements I monitored today. The staff and training worksheets were submitted prior to the visit. Currently this center operates with a Five Star License, issued December 12, 2018, earning six (6) points in the Education Component, six (6) points in the Program Component and one (1) Quality point based on the program having a staff benefits package and an infrastructure of parental involvement. You, M. Aldridge, Administrator, were present and available for consultation at the conclusion of the visit. S. Wolmarans, the second Administrator was also present and assisted with today’s visit. Children were observed participating in group activities, free play in centers, completing transitions, completing routines such as handwashing and toileting and participating in outdoor free play. Proper hand washing techniques were observed throughout the facility. The teachers supervised and interacted with the children in a positive manner. The last Annual Compliance visit was completed on April 4, 2023. The last sanitation inspection was completed March 14, 2023, with a “Superior” classification. The last fire inspection was completed May 11, 2023. This facility is owned by Farmer-N-Dell Learning Center, LLC (Limited Liability Company). As of today, the status is listed as “Current-Active” on the NC Secretary of State website. Prior to any changes with the corporation, please contact me to discuss change of ownership procedures. Prior to today’s visit, the facility has earned a Compliance History score of 100% over an 18 - month time- period. A copy of the compliance history was provided to you. I used the Annual Compliance Monitoring Checklist for Child Care and the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions.
