Kim Barnes, Child Care Consultant, assisted with today’s visit. V. Stephenson, Administrator/owner, was present and assisted me with the visit. Currently this center operates with a Four Star-Rated license, issued April 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the educational option when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, teacher, and group lead positions over the last 12 months. The last sanitation inspection was completed June 11, 2025, with a superior rating. The last fire inspection was completed on 11/17/2025. The next rated license assessment is due April 11, 2027; today, you requested to be assessed for a Star-Rated License using Pathway 3 – Head Start Accreditation. Prior to today’s visit, the program’s compliance history was 98% over the current 18- month period. A copy of your compliance history report was left with you. I monitored the classrooms and spaces used for child care and outdoor play area for applicable child care center requirements. I observed children participating in routines and developmentally appropriate activities in the indoor and outdoor environments. Staff/child ratios and supervision were in compliance. Children were observed participating in a multicultural group activity and transitioning to the outdoor environment. Staff members interacted positively with the children. A computer-generated visit summary was reviewed with you, including all documented violations, and a copy was provided at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. There were three dates in which one child in space 1A did not have documented departure times. The dates were 3/6/26, 3/9/26, and 3/10/26. 10A NCAC 09 .0302(d)(4) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. Upon arrival, the menu observed posted was dated for the month of January. The March menu was posted during the visit. 10A NCAC 09 .0901(b) 807 A safe indoor and outdoor environment was not provided for the children. In the outdoor environment, two plastic child sized picnic tables were cracked along the edges, causing a pinching hazard. The two plastic tables were removed during the visit. 10A NCAC 09 .0601(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were being stored in plastic containers and stored in a place that were accessible to children in spaces 1A and 1B. .0604(q) CONSULTATION: Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. A checklist was used to note the requirements I monitored today. The staff and training worksheets were emailed prior to today's visit. Today's visit was completed with V. Stephenson, owner/operator. Currently this center operates with a Four Star-Rated license, issued April 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the educational option when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, teacher, and group lead positions over the last 12 months. The last Annual Compliance visit was completed on April 9, 2024. The last sanitation inspection was completed on June 4, 2024, with a superior rating. The last fire inspection was completed on November 21, 2024, and received on January 19, 2025. As a reminder, fire inspections must be mailed and received within seven (7) days of the inspection. Prior to today’s visit, the facility has earned a Compliance History score of 95% over an 18 - month time- period. A copy of the compliance history was provided to you. Children were observed participating in group activities, free play in centers, free play in the outdoor environment, completing transitions, and completing routines such as handwashing and toileting. There were adequate staff supervising the enrolled children. The teachers supervised and interacted with the children in a positive manner. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. A computer-generated visit summary was reviewed with you and a copy was provided to you at the conclusion of the visit. All violations were reviewed with you at the conclusion of the visit. The following violations of a child care requirements were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed 11/21/2024; however, the inspection was not submitted to the Division within one week of the inspection visit. The inspection was received on 1/19/2025. 10A NCAC 09 .0304(a) 1898 Staff did not complete the health and safety training within one year of employment. K. Jackson did not complete the health and safety trainings within one year of employment. The employment date was 3/7/2021. A checklist was used to note the requirements I monitored today. The staff and training worksheets were emailed prior to today's visit. Today's visit was completed with V. Stephenson, owner/operator. Currently this center operates with a Four Star-Rated license, issued April 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the educational option when the program has earned at least 4 points in education and the program has a combined turnover rate of 20 percent or less for the administrator, program coordinator, lead teacher, teacher, and group lead positions over the last 12 months. The last Annual Compliance visit was completed on April 9, 2024. The last sanitation inspection was completed on June 4, 2024, with a superior rating. The last fire inspection was completed on November 21, 2024, and received on January 19, 2025. As a reminder, fire inspections must be mailed and received within seven (7) days of the inspection. Prior to today’s visit, the facility has earned a Compliance History score of 95% over an 18 - month time- period. A copy of the compliance history was provided to you. Children were observed participating in group activities, free play in centers, free play in the outdoor environment, completing transitions, and completing routines such as handwashing and toileting. There were adequate staff supervising the enrolled children. The teachers supervised and interacted with the children in a positive manner. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. A computer-generated visit summary was reviewed with you and a copy was provided to you at the conclusion of the visit. All violations were reviewed with you at the conclusion of the visit. The following violations of a child care requirements were documented. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed 11/21/2024; however, the inspection was not submitted to the Division within one week of the inspection visit. The inspection was received on 1/19/2025. 10A NCAC 09 .0304(a) 1898 Staff did not complete the health and safety training within one year of employment. K. Jackson did not complete the health and safety trainings within one year of employment. The employment date was 3/7/2021.
