Staff and training worksheets were received prior to the visit. Today’s visit was completed with you, D. Edwards, Administrator. You were notified of the purpose of the visit and were available for consultation at the conclusion of the visit. Kim Barnes, Child Care Consultant assisted with today’s visit. Currently this center operates with a Four-Star - License, issued July 11, 2024, earning seven (7) points in Staff Education, two (2) points in Program Standards and 1 Quality point based on meeting the education option of the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. And the facility was approved for daytime care only. The last Annual Compliance Visit was completed on April 22, 2025. The last Sanitation Inspection was completed on June 5, 2025, with a “Superior” classification and “0” demerits. The last Fire Inspection completed at the center was on October 22, 2025. Children were observed participating in free indoor play, group time, routine care needs being met, eating lunch which consisted of beans and franks, corn, bread and milk, and completing, transitions. The infants were observed interacting with caregivers, sitting in bouncy seats, and having routine care needs met. Teachers interacted with children and participating in free play. The teachers supervised and interacted with the children in a positive manner. Prior to today’s visit, the program’s compliance history was 91% over the current 18- month period. A copy of your compliance history report was left with you. You stated the mailing address, phone number and email address listed for your child care center are correct. The Department of The Secretary of State website was checked prior the visit. The status of the corporation listed as The Church of Alpha and Omega was found to be current-active. It appears that the corporation is in good standing with the NC Department of the Secretary of State. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing (DCD 0357) as basic monitoring tools to assess compliance with all applicable child care requirements. A computer-generated visit Summary, along with all violations, was reviewed with you and a copy was provided to you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. Incident log was not available for review. .0802(g)(1-6) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were accessible to children under three years of age in Spaces 1 and 3. .0604(q) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Today’s visit was completed with administrator D. Edwards and owner, E. Edwards. Child Care Consultant, Kim Barnes assisted with today’s visit. Currently this center operates with a Four-Star - License, issued July 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the education option of the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. A compliance history score of 95% was shared with you today. E. Edwards, and two (2) additional staff members were interviewed. FINDINGS/OBSERVATIONS: Allegation 1: There is a concern that on February 10, 2026, the facility exceeded the licensed capacity of 45 children. It was reported that there were 52 children in attendance between the hours of 1:07pm and 4:27pm. All children’s attendance records for the month of February were reviewed, and all enrolled children’s files were counted for enrollment verification. There were 52 children’s files available for review. When asked, it was stated that there are 52 children enrolled in the program. It was further stated that eleven Head Start children come after school, along with ten school-age children, bringing the total of twenty-one children that attend the afterschool program. Ms. Edwards stated that the children normally leave as soon as they get to the facility; however, on February 10, 2026, the children did not leave as they normally would. After reviewing attendance records for the month of February, it appears that the facility has been out of compliance with the capacity requirement more than one time. The following attendance totals are: February 9,2026 – 51 total attendance February 10, 2026 - 52 total attendance February 11, 2026 - 48 total attendance February 12, 2026 - 47 total attendance February 13, 2026 - 49 total attendance February 16, 2026 - 46 total attendance February 17, 2026 - 41 total attendance We reviewed your capacity and ways to ensure you are in compliance with your capacity requirements. Based on observations, staff reports, and my investigation, the allegation of facility exceeding the licensed capacity is substantiated. The computer-generated visit summary, including all violations, was reviewed with you at the conclusion of the visit, and a copy was provided to you. The following violations of child care requirements were documented: Violation Number Comment Rule 103 The number of children present was not within permit capacity. On Tuesday, February 10, 2026, the facility had 52 children present at one time. The capacity for the facility is 45; therefore, the facility was not in compliance with the permit capacity. GS 110-91(7) & .1401(f) 1756 Enhanced staff/child ratios and group sizes were not met. In the outdoor space there were ten children, one year of age present with one teacher. The teacher for the one year old children left the playground and went inside, leaving ten children one year of age, with the two preschool teachers. The two preschool teachers were caring for twelve children ranging in age from two to four years of age. Today’s visit was completed with, E. Edwards, Administrator and . You were present when I arrived, and you were notified of the purpose of the visit. Currently this center operates with a Four-Star - License, issued July 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the education option of the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. You stated the mailing address, phone number and email address listed for your child care center are correct. The Department of The Secretary of State website was checked prior the visit. The status of the corporation listed as The Church of Alpha and Omega was found to be current-active. It appears that the corporation is in good standing with the NC Department of the Secretary of State. Prior to today’s visit, the program’s compliance history was 96% over the current 18- month period. A copy of your compliance history report was left with you. Children were observed participating in routines, free play in the indoor environment, group activities, and transitions. All interactions were observed to be positive and nurturing and adequate supervision was provided to all children in care. Sleep checks were observed as current and safe sleep practices were adhered to. Infants were observed interacting with caregivers and having routine care needs met. Upon arrival, the temperature in the facility was 66 degrees. We discussed the temperature requirement for the infant classroom. The temperature in this room must always be between 68 and 75 degrees. You adjusted the thermostat during the visit. The computer-generated visit summary was reviewed with you at the conclusion of the visit and a copy was provided to you. All violations were reviewed with you. I used the Child Care Center Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements. A checklist was used to note the requirements I monitored today. Today’s visit was completed with you, E. Edwards, Administrator. You were notified of the purpose of the visit and were available for consultation at the conclusion of the visit. Staff and training worksheets were received prior to the visit. Currently this center operates with a Four-Star - License, issued July 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the education option of the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. The last Annual Compliance visit was completed on April 24, 2024. The last Sanitation Inspection completed at the center was on June 6, 2024. The last Fire Inspection completed at the center was on October 23, 2024. Children were observed participating in free indoor play, group time, routine care need being met, and transitions. The infants were observed interacting with caregivers, sitting in bouncy seats, and having routine care needs met. Teachers interacted with children and participating in free play. The teachers supervised and interacted with the children in a positive manner. Prior to today’s visit, the program’s compliance history was 88% over the current 18- month period. A copy of your compliance history report was left with you. You stated the mailing address, phone number and email address listed for your child care center are correct. The Department of The Secretary of State website was checked prior the visit. The status of the corporation listed as The Church of Alpha and Omega was found to be current-active. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing (DCD 0357) as basic monitoring tools to assess compliance with all applicable child care requirements. Aerosol cans of Lysol disinfectant spray was stored unlocked, on the van used for transportation. .2820(b) 1898 Staff did not complete the health and safety training within one year of employment. Staff member, J. Sutton has not completed the required trainings within one year of employment. The hire date was 5/8/2023. All of the health and safety trainings are required. You were notified of the purpose of the visit and were available for consultation at the conclusion of the visit. Staff and training worksheets were received prior to the visit. Currently this center operates with a Four-Star - License, issued July 11, 2024, earning seven (7) points in the Education Component, two (2) points in the Program Component and 1 Quality point based on meeting the education option of the child care administrator having at least 10 years of documented child care administration work experience in a licensed program. The last Annual Compliance visit was completed on April 24, 2024. The last Sanitation Inspection completed at the center was on June 6, 2024. The infants were observed interacting with caregivers, sitting in bouncy seats, and having routine care needs met. Teachers interacted with children and participating in free play. The teachers supervised and interacted with the children in a positive manner. Prior to today’s visit, the program’s compliance history was 88% over the current 18- month period. A copy of your compliance history report was left with you. You stated the mailing address, phone number and email address listed for your child care center are correct. The Department of The Secretary of State website was checked prior the visit. The status of the corporation listed as The Church of Alpha and Omega was found to be current-active. It appears that the corporation is in good standing with the NC Department of the Secretary of State. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing (DCD 0357) as basic monitoring tools to assess compliance with all applicable child care requirements. ACTIVITY PLANS: In space 2, there was not an updated activity plan posted for the enrolled children. Activity plans are critical to the structure of the classroom. It assists in the teacher’s day-to-day routines and ensures the children receive developmentally appropriate instruction. LEAD IN WATER, PAINT/ASBESTOS: Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Most facilities have completed their initial testing and now it is time for their three-year retest. We reviewed that your water testing was completed on March 27, 2024. RECOGNIZING AND RESPONDING TO SUSPICIOUNS OF CHILD MALTREATMENT: Jalen Sutton was required to complete this training by August 8, 2023; however, the training was completed on August 23, 2023. This training is required to be completed within 90 days of employment. FALL ZONES/SURFACING: While completing a walk-through of the facility and the outdoor play area, I observed two new play structures on the playground for preschool and school-age children. I measured the two play structures that were stationary. Both structures measured greater than 18 inches; therefore, the two play structures must have surfacing and fall zones. We reviewed the required depth of surfacing based on the critical height of the play structures. We reviewed that there must be 6 inches of surfacing material. We reviewed the approved surfacing material that is documented in the child care rules. We discussed that the surfacing must extend 6 feet beyond the external limits of the equipment. CHANGES IN LICENSING FEES: License fee collection for 2023 is moved to December. There are a few other changes to the Annual License Fee process: All invoices will be emailed, and no paper copies will be sent through the U.S. Postal Service. All invoices will be emailed by December 1, 2023 and will include the invoice number and total amount due. All payments are due no later than December 31, 2023. Payments must be submitted online only through the Division’s website using a Mastercard or Visa debit or credit card, or by e-check. Providers will need their Facility ID number and their 2023 Invoice number to complete the payment process. Failure to pay the license fee is a violation of NCGS § 110-90(1a) and can result in a revocation of your child care license. In addition, unpaid license fees will be assessed a 10% late fee and accrued interest as established by NCGS § 147-86.23.
