Name of Operation: NORTH ELEMENTARY SCHOOL Facility ID: 23000430 Consultant: TIFFANI SIMS Operation Type: Center Case Number: Visit Date: 12/16/2025 Number Present: 16 Completed Date: 12/16/2025 Age: From 4 To 5 Total Minutes: 190 Time In: 09:30 AM Time Out: 12:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit, including compliance with requirements located in Child Care Rule Section .3000 in spaces #1, where children participating in the NC Pre-K program are cared for. You, Ashley Fowler, Lead Teacher, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 15, 2024, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: daytime care, enhanced ratios reduced by 1, and enhanced space. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent (98%) as of December 11, 2025. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Your program was also monitored for compliance with implementing an approved curriculum Creative Curriculum: Teaching Strategies Gold as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 8, 2025. The sanitation inspection was completed October 8, 2025, with a “Superior” classification and zero (0) demerits. The last fire inspection was conducted on June 12, 20255, and your facility was approved for daytime care only. The last shelter in place emergency drill was completed on November 6, 2025, at 9:00am and recorded. The last playground inspection was completed on November 3, 2025. The last fire drill was completed on November 6, 2025, at 2:00pm and recorded. Each indoor and outdoor space was monitored. Children enrolled were observed during teacher directed activities, group time, music and movement, story time, hand washing, bathroom routines, indoor free play, and lunch. Proper hand-washing techniques procedures were observed in both classrooms. Lunch was observed during today’s visit and included chicken tenders or chicken quesadillas, sweet potato fries or corn, orange slices or diced pears, garlic whole wheat bread, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Files Reviewed: The current NC Pre-K staff are lead teachers and teachers. A selection of files was monitored for completed health assessments and developmental screenings. Two (2) children’s files were reviewed during today’s visit. Three (3) staff file was reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. You stated that you had completed the Brigance screenings for the children in your classrooms by September 30, 2025. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning in October 2025, middle in February 2026, and end of the school year in late April or May 2026. The first day of school was August 25, 2025. Staff also stated that additional resources are used in the classroom, such as Second Steps, Heggerty, Ready Rosie, and Finch. Classroom operating hours are from 7:35am to 2:50pm. Parent conferences are held twice per year. Two home visits are conducted two times per year. You stated that you use the Class Dojo (app) to communicate with parents, as well as in person, and through telephone calls. In addition, a folder is sent home daily with information such as daily reports, newsletters, second steps links are sent home to the parents which includes any information about the child’s day and any reminders. Documentation was reviewed of parent newsletters, monthly calendar with notes, parent contacts in Class Dojo, and observational notes of children. The environment rating scale score received on February 13, 2024, was 6.14. NC Pre-K requirements were monitored today. The analysis date for the most recent lead water test was April 18, 2024. A checklist was used to note the requirements I monitored today. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space #1, it was observed in the Lead Teacher’s unlocked desk, was one BIC Wite Out Correction Pen with written warnings keep out of reach of children. The correction pen was accessible to children. .2820(b) Technical Assistance was provided on the following Hazardous Materials with Written Warnings: -All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled must be stored in a locked storage room or cabinet. I suggest that you monitor all areas of the child care facility/classroom each day to ensure all hazardous products are stored appropriately. I also suggested that you check all storage cabinets and closets to ensure the lock is operating properly. I suggested that each staff go through their desks to ensure hazardous materials with written warnings are not accessible to children but kept in locked storage. Violation was corrected during the visit. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. Education will be evaluated for administrators and staff who provide direct care to children. Safe Environments/Supervision During Naptime: I discussed with you the child care requirements about providing a safe environment and supervision during naptime. Upon arrival, the purpose of today’s visit was reviewed with you, Sonya Pressley, owner/operator. You, Ashley Fowler, Lead Teacher, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 15, 2024, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: daytime care, enhanced ratios reduced by 1, and enhanced space. During today’s visit, the children were observed during breakfast, music and movement, group time, teacher directed activities, and outdoor gross motor play. I observed children in the indoor learning environment and found supervision and staff/child ratios to be incompliance. Children were observed during indoor free play. A walk-through of the licensed space used by children was completed today, all indoor and outdoor areas were monitored. You stated no new staff have been hired since your most recent annual compliance visit conducted on January 8, 2025. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on March 18, 2024. The next fire inspection is due by March 18, 2025. Sanitation Inspection: The last sanitation inspection was completed on December 4, 2024. A “superior” classification was issued with zero (0) demerits noted on the grade card. The most recent shelter in place emergency drill was conducted on February 3, 2025, at 10:00am and recorded. The most recent fire drill was conducted on April 28, 2025, at 2:00pm and recorded. The most recent playground safety inspection was conducted on April 1, 2025. The following violation of child care requirements was observed and documented during today’s visit. Violations and technical assistance were thoroughly reviewed with you and documented in the visit Summary that was given to you at the conclusion of the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection documentation on file and available for review was dated March 18, 2024. 10A NCAC 09 .0304(a) Technical Assistance: - Fire Inspection I discussed with you that fire inspections must be conducted annually prior to the date of the last fire inspection. I reminded you that once you have received an approved fire inspection, you have seven (7) days to submit a copy to your DCDEE Child Care Consultant. I suggested that you contact the Fire Marshall’s office at least four to six weeks prior to when the fire inspection is due to ensure that the fire inspection would not be delinquent. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance letter by May 20, 2025. On the home page scroll down on the link for What’s New and click on the link QRIS Moderation. -Child Care Rule Effective January 1, 2024 Lead Testing: Child Care Rule .0601(f)- Safe Environments We discussed the test result that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on April 18, 2024. On May 5, 2025, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had no results available for review for lead-based paint or asbestos testing. -On-going Trainings: Please visit Moodle on the DCDEE website to obtain necessary on-going training hours as required annually. Another website for staff to earn on-going training hours is from the NC Rated License Assessment Project. You, Diane Wilson, Lead Teacher, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 30, 2023, earning seven (7) points in staff education, seven (7) points in the program standards, and one quality point. The program's license restrictions include the following: daytime care, enhanced ratios reduced by 1, and enhanced space. During today’s visit, the children were observed during indoor and outdoor free play, group time, story time, and lunch. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on October 5, 2023. Sanitation Inspection: The last sanitation inspection was completed on November 27, 2023. A “superior” classification was issued with zero (0) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter in place drill was completed on October 12, 2023, at 12:20pm and recorded. The last playground inspection was completed on December 1, 2023. The last fire drill was completed on December 15, 2023, at 8:16am and recorded. A complete walkthrough of licensed indoor and outdoor spaces was conducted. A checklist was used to note the requirements I monitored today. You said that you are going to apply for the rated license and request to have the Environment Rating Scale Assessment completed. We discussed the Application for Assessment for a Two Component Star Rated License and the Rated License Assessment Request form. We reviewed and discussed Program Standard points, Staff Education points and Quality point options. I asked you, Diane Wilson, if you had any questions concerning the Environment Rating Scale Assessment and you stated no, not at this time. Files Reviewed: Children’s files, program files, and staff files were monitored today. Three (3) children's files were reviewed during today’s visit. Two (2) staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violation was observed during the visit today. I discussed this violation with you in detail at the end of the visit. The DCDEE child care consultant received the current fire inspection report dated October 5, 2023, from the child care provider during today’s annual compliance rated license visit. 10A NCAC 09 .0304(a) The following Technical Assistance regarding the violation observed during today’s visit was provided: Fire Inspection Reports: As a reminder, fire inspection reports are required annually and must be completed before the last date of when the last fire inspection was conducted. I suggested contacting the Fire Marshall’s office at least four to six weeks prior from when the last fire inspection was completed to get on their calendar to ensure the fire inspection report does not exceed the one-year date. Also, fire inspections must be submitted to DCDEE within one week of the completed fire inspection. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-six percent (96%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by February 1, 2023. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Consultation: ECERS-R Suggestions Blocks: ECERS-R: A variety of different types of blocks such as wooden unit blocks, soft unit blocks, cardboard blocks, and homemade blocks with enough blocks in each set for at least three children to build a structure should be provided. The block center should be large enough for three children to build a substantial structure without interfering with one each other’s play. Block play and accessories will need to be made accessible to children outdoors. Art: ECERS-R: Art projects should allow individual expression by children. Copied, cookie cutter “Alike” type art projects do not offer children an opportunity for individual expression. Art projects should allow individual expression by children. The following are suggestions for art materials: markers, chalk, colored pencils, paper, crayons, dry erase markers, finger paint, tape, glue, stickers, small pieces of paper, play dough, clay, hole punchers, stencils, stamps, and scissors. Child Related Display: Display in the classroom should be primarily children’s work and/or realistic pictures, and not teacher or store-bought display at varying eye levels. Transitions: Avoid long periods of waiting during transitions such as bathroom time or from one activity to another. Develop ideas and strategies to reduce waiting time for children. Teachers can sing songs and do finger plays with the children who are waiting. If children wait more than three minutes without activity, then you will be marked down on the Environment Rating Scale Assessment score. Fine Motor: ECERSR-R: Review the Activities section of the ECERS-R for the required amount and type of materials required for fine motor play. The categories required for fine motor play include small building materials, art, manipulative, and puzzles. Each category must be a complete set-in order for it to count. For example, you need lacing strings for lacing cards. Books: ECERS-R: The classroom should have at least two books per child that are age appropriate for the children in the group. I suggested that books that portray violence be removed. A variety of books that portray real animal pictures, families, disabilities, and different cultures should be added to each classroom. Remove any books that are worn, torn or missing pages. You are only allowed three books throughout the classroom that are in poor repair. You will also need to add at least two additional language activities and/or games to this area. Dramatic Play: ECERS-R: I suggested that you have or purchase hats; boys dress shirts, ties, jerseys and dress coats from a consignment shop, yard sale or dollar store to enhance the dramatic play center. Other items that enhance dramatic play are telephone books, cooking aprons, restaurant menus, calculators, cash registers, cookbooks, and baby doll accessories. Inclusion of Diversity: Add pictures, books, puzzles, block play people, baby dolls, music, play food and dress-up clothes that represent different cultures. Add pictures, puzzles and block play people that represent people of all ages and disabilities. Daily Schedule: ECERS-R: Substantial Portion of the Day means that one-third of the time the program is in operation that free play must occur where children have free access to centers that have this requirement. For example, if a program operates for twelve hours, an ECERS-R classroom must have four hours scheduled of free play, excluding outdoor gross motor play and clean-up from free play. Math/Number: ECERS-R: Math/number - Review the Activities section of the ECERS-R for the required amount and type of materials required for math/number. Math/number materials can be found in multiple areas of the classroom. This center is not required to have its own space. One instance must be observed during routine care as well as during free play of using math/number. Health Reminders: Drinking Water: Water must be available to children throughout the day. Cleaning/Sanitizing/ Disinfecting Surfaces: Tables must be cleaned with soapy water then wiped dry. Then spray table with the sanitizing solution and let it sit for at least two minutes before wiping dry. Follow this procedure before and after eating. Make sure no children are near the table when spraying the sanitizing solution. Sinks must be disinfected between changes of use. The staff should spray the sink with the disinfectant spray and allow the disinfectant to stay for at least two minutes before turning on the water and using the sink. Hand Washing: Children must wash their hands upon arrival, before and after eating, before and after sand and water play, after toileting, after outdoor play, after handling messy art materials such as paint, glue, and playdough and after touching nose, mouth, floor, trashcan, and anything else that re-contaminates the hands. Staff should follow proper handwashing procedures and wash their hands as required. Nap/Rest ECERS-R Item #11: •As reminder for the ECERS-R, cots/mats must be 36 inches or 3 feet or separated by a solid barrier per ERS standards. •Non-Nappers and Early Risers – I suggested that you give children items, such as books or small quiet toys to do quietly on their cots if they are non-nappers and/or early risers. -COVID-19 Information and Resources: I shared information and resources about COVID-19 vaccines, visit MySpot.nc.gov, call the CDC-INFO Contact Center at 800-CDC-INFO, or visit ncdhhs.gov/LHD to contact a local health department. Read the CDC’s full statement about recommending the updated COVID-19 vaccine for the fall/winter virus season.
