You, Melinda Renee Odems, Operator, assisted me with the visit. Farran Rhyne, Child Care Consultant, accompanied me during the visit. Your program currently operates with a Four star rated license effective January 13, 2020. The program’s compliance history was 100 percent prior to today's visit. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4)(c). Please note any violations cited today and during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your FCCH is not incorporated. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form were completed and signed during today’s visit. Your signature on these forms served as verification the information provided was accurate. Five (5) children were present today. We observed children engaged in free play activities indoors. The caregiver was actively involved with the children. Sixteen (16) total children were enrolled, and you currently operate on first and second shifts. You stated no children are currently enrolled on third shift. No infants were enrolled. The most recent outdoor inspection checklist was completed April 6, 2024. The most recent fire drill was conducted on April 3, 2024. The most recent quarterly drill was conducted on April 2, 2024. Your Criminal Background Check Qualifying letter was reviewed and was in compliance during today’s visit. The following violations of child care requirements were observed during today’s visit. Violations and technical assistance were thoroughly reviewed with you and documented in the handwritten Visit Summary given to you at the conclusion of the visit. The computer-generated visit summary was sent to you after the visit was conducted on April 17, 2024. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Gasoline, paint cans, and chemicals were observed in an unlocked storage shed in the backyard accessible to children. .1719 (a) (1) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. One aerosol can of Easy Cheese was observed on a table in the dining room accessible to children. Two aerosol cans of Raid insect spray were observed in an unlocked cabinet below the counter in the kitchen accessible to children. .1719 (a)(7) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. One child enrolled on 7/10/2023 did not have an acknowledgement of the Summary of the Law signed by the child's parent until 1/16/2024. GS 110-102 1828 The Family Child Care Home operator did not discuss the operational policies with parents on or before the child's first day of attendance. One child enrolled on 7/10/2023 did not have an acknowledgement of operational policies/parent handbook signed by the child's parent until 1/16/2024. 10A NCAC 09. 1715(b) The violations observed were corrected during the visit. Technical Assistance regarding the violations observed: Storage of Hazardous Substances: You locked the storage shed containing hazardous items during the visit. We suggested that you check for hazardous items in and outside of your home to ensure hazardous items are stored properly and the storage sheds containing hazardous items are kept locked during child care hours of operation. Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. You discarded the aerosol can of Easy Cheese and moved the two aerosol cans of Raid to locked storage during the visit. We suggested that you check all cabinets at the beginning of each day to ensure hazardous products were not left in unlocked storage from the night before. Child Care Rules states to keep all corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product that is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled shall be kept in its original container or in another labeled container, used according to the manufacturer's instructions, and stored in a locked area when not in use. Locked areas shall include those that are unlocked with a combination, electronic, or magnetic device, key, or equivalent locking device. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. Toxic substances shall be stored below or separate from medications and food. Any product not listed in this Paragraph that is labeled "keep out of reach of children" without any other warnings shall be kept inaccessible to children when not in use, but is not required to be kept in locked storage. The product shall be considered inaccessible to children when stored on a shelf or in an unlocked cabinet that is mounted a minimum vertical distance of five feet above the finished floor. Children’s Records: Operational policies and the NC Summary of the Law shall be discussed and reviewed with parents on or before the child's first day of attendance in the home. A copy of the policies shall be given to the parents and the parents shall be notified in writing of any changes in policy. I discussed the Children’s File Checklist from the DCDEE website found under Provider Documents and Forms and recommended that you utilize this checklist for your children’s files. You must have documentation of acknowledgement on file for review for Consultants to verify compliance with these requirements. Rated License Reassessment: To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in cohort two. The preparation year for cohort two will begin July 1, 2024. The reassessment year will be July 1, 2025, through June 30, 2026. More information will be forthcoming once your cohort year approaches. Consultation: (The following items discussed were in compliance today.) -All licensed child care programs must register by May 1, 2024, at www.cleanwaterforUSkids.org/carolina and watch one of the pre-enrollment webinars. Family Child Care Homes have until January 1, 2025, to complete the required lead testing. You completed the pre-enrollment webinar on March 21, 2024. -You must complete on-going health and safety trainings every five years. You previously completed health and safety trainings on April 15, 2020. -When reviewing children’s records completed by the child’s parent prior to enrollment, ensure that all required information is obtained. A copy of the Prevention of Shaken Baby and Abusive Head Trauma policy shall be given and explained to the parents of children up to five years of age on or before the first day the child receives care at the home. The operator shall obtain the parent's signature. The acknowledgement shall contain the following and be maintained in the child's file for review by the Division: (1) the child's name; (2) the date the child first attended the home; (3) the date the operator's policy was given and explained to the parent; (4) the parent's name; (5) the parent's signature; and (6) the date the parent signed the acknowledgment.
