This visit was conducted with Ms. Colleen Rabenold, Director. This program currently operates with a Four Star license. The license was posted and the restrictions were being followed. The compliance history is 91% today. Programs are required to achieve and maintain an eighteen-month compliance history score of at least 75%. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were taking a nap, eating snack and playing with toys. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • SIDS training • Criminal Background Checks • General health • Administration of medication • Storage of hazardous products • Storage of medication • General safety • General license requirements • Program records • License posted • Fire safety • Sanitation Limited monitoring of staff files was conducted in accordance with DCDEE procedures for the purpose of reviewing CPR and First Aid, EPR, SIDS training, Health and Safety trainings and Criminal Background Checks. The storage and administration of medications was monitored. Medication authorization was monitored. The storage of hazardous items was monitored. A fire drill was documented on 12-31-2024. A shelter in place drill was documented on 11-01-2024. An outdoor inspection was documented on 12-03-2024. The following violations were documented and corrected during this visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space 106, the lesson plan was dated for September. In space 132, the lesson plan was dated fro last week. Today I provided Technical Assistance in the following areas: Consultation: I have observed new staff files today and pointed out to Ms. Rabenold that the doctor made a note on one person’s medical form. Ms. Rabenold stated that she would get to the bottom of it. I have reviewed this visit summary with Ms. Rabenold at the conclusion of the visit and a copy was given to her. This visit was conducted with Ms. Colleen Rabenold, Administrator. This program currently operates with a Four Star license. Restrictions include daytime care only, 2nd grade and above on 2nd floor only, only second grade and above on second floor, meets enhanced ratios, meets enhanced space, children under 2 ½ in rooms with direct exits. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 09-23-2024. The program’s compliance history was 94% prior to today’s visit. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit I observed children playing with toys, eating lunch, playing outdoors and taking a nap. The last annual compliance visit was conducted on 10-25-2023. The last sanitation inspection was conducted on 07-10-2024. The last fire inspection was conducted on 07-11-2024. A fire drill was documented on 08-22-2024. A shelter in place drill was documented on 07-29-2024. An outdoor inspection was documented on 09-23-2024. I observed program records and posted items today. I observed staff and children’s records today according to DCDEE procedures. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program provides transportation. During today’s visit, I monitored two vehicle(s) for the following: • Emergency information • Permission to transport • Transportation logs • First aid kits and fire extinguishers • Condition of the vehicle(s) • Paperwork concerning vehicle compliance with NC Law I documented the following violations during today’s visit: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. The daily schedule was not posted in space 104. GS 110-91(12);.0508(a) 705 Equipment and furnishings were not sturdy, stable and free of hazards. There is one table in space 132 with very loose legs. .0601(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Playground inspections had not been completed since June 2024. .0605(q) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff person had not completed Child Maltreatment training. .1102(g) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member had not completed 5 of 9 health and safety trainings in the last five years. .1103(b) I have asked Ms. Rabenold to send me written documentation by October 7, 2024 indicating that the violations that were documented today have been corrected. I have asked her to include the center name and license number in her documentation, each violation number and how each was corrected, and to both sign and date the document. The document may be mailed or emailed to me at the address below. I have reminded Ms. Rabenold that if the documentation indicates that corrections have been made when they have not, this may be considered falsification of information and an Administrative Action may be recommended. In addition, repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date indicated in this visit summary could lead to the recommendation of an Administrative Action. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: The table legs on the table in space 132 are very loose and the table is in danger of falling over. The table should be repaired or disposed of. Ms. Rabenold completed a playground check today. Consultation: Ms. Rabenold can consider if the sandbox needs more sand. Ms. Rabenold needs to evaluate whether the children in space 134/135 needs more books and purchase age- appropriate books that will take wear and tear. Ms. Rabenold will remind teachers to pick up trash when they see it on the playground. Hand sanitizers need to be higher than five feet off the floor since they say “keep out of reach by children” on the back. I have reviewed this visit summary with Ms. Rabenold at the conclusion of the visit and a copy was given to her. The following violations were documented and corrected during this visit: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. One non-emergency medicine was more than five feet off the floor, but was not in the locked medicine box. 15A NCAC 18A .2820(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One infant gas drops was leftover and was not sent home with the parent. .0803(12) 1879 Prescribed medicines, that are pharmaceutical samples, was not stored in the manufacturers original packaging, was not labeled with the child's name, and/or written instructions did not include the required information. One bottle of mucus medicine did not have the child’s name or a medicine permission form to accompany it. 10A NCAC 09 .0803(2)(b)(i-v) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Today I provided Technical Assistance in the following areas: Medicine that is not for emergency situations must be locked. Emergency medicine that is for emergency situations must not be locked, but must be higher than five feet off the ground so that children cannot access them. Medicine must be labeled with the child’s name and must match the medicine slip that accompanies it. Leftover medicine must be sent home with the child’s parent. Consultation: I have suggested that the center keep the menu in the hallway where it is posted and take the other menus posted around the center down. The cook will only have to make changes to one menu when he does not serve what is listed, as opposed to several. I have reviewed this visit summary with Ms. Rabenold at the conclusion of the visit and a copy was given to her. One child who was allergic to peaches was not listed on the allergy list in the kitchen or dining room. .0901(g) 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The cook served mixed fruit today instead of peaches and corn instead of green beans and no substitution was made on the menu which was posted for parents to see. 10A NCAC 09 .0901(b) Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. I provided the following Technical Assistance during today’s visit: The children’s records have many extra papers in the files. I have told Ms. Rabenold that the extra papers are not requirements for the child’s file and there is no need to keep multiple copies of the same paper. I recommended that the children’s paperwork with permission to transport be both on the bus and in the building in the child’s file. Today the paperwork was on the bus. I recommended that the license be moved from inside the office door to the entrance hallway for parents to view and this was completed during the visit. Ms. Rabenold has four staff training worksheets but is only able to find three of them. I have asked her to send the last worksheet to me and told her that I may return to observe the staff files. I have reviewed this visit summary with Ms. Rabenold at the conclusion of the visit and a copy was given to her.
