M. Kennon, Family Child Care Home Operator, assisted me with the visit. Your program currently operates with a five-star license, issued January 11, 2026 (capacity change), earning 5 points in the education component, 7 points in the program standards component and 1 quality point for serves no more than two infants younger than one year of age. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. You currently do not have any preschool-four-year-olds enrolled but stated that you use Creative Curriculum for Family Child Care Homes. The last annual compliance visit was conducted December 12, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of April 2, 2026. The family child care home is owned as a Sole Proprietorship. License and contact information were current. Contact me to request any changes to your license or contact information. The license was posted, and the restrictions were in compliance. The operator was present with seven children, two through four years of age. She was preparing lunch while the children played with blocks on the carpet where they were easily supervised. When lunch was ready, they washed their hands and returned to the dining area where they ate at a child-sized table. Lunch was relaxing and pleasant. After lunch, the children washed hands and played with toys and materials in the play area. They practiced a fire drill from the nap room before they laid down for nap. The operator played soothing music during naptime. Lunch was observed and consisted of dino chicken nuggets, mashed sweet potatoes, banana and apple slices, whole grain bread, and milk. I observed and documented the following violations during today’s visit. You corrected one of the violations during the visit. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation of a March 2026 outdoor inspection was not available. 10A NCAC .1721(e)(5)(A-F) 1853 The operator did not conduct a monthly fire drill. Documentation of a monthly fire drill was not available for March 2026. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The last quarterly emergency drill was dated December 10, 2025. An emergency drill was not completed during the first quarter of 2026. .1719(a )(16) & .1721(e )(7) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The two violations that have not been corrected must be corrected immediately. On or before April 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance With Documented Violations: Outdoor Inspections – Regular outdoor inspections are critical to prevent deterioration of equipment and accumulation of hazardous materials within the play site, and to ensure that appropriate repairs are made as soon as possible. Documentation of a March 2026 outdoor inspection was not available. The operator stated that she just missed it. Consider creating a Google Calendar on your phone for the beginning of each month to remind you to complete your monthly outdoor inspection. (Combine it with a fire drill reminder!) Complete your April 2026 outdoor inspection this week. For your compliance letter, state the date you complete your April 2026 outdoor inspection and include a copy of the outdoor inspection checklist. Fire Drills and Emergency Drills – Regular fire and emergency drills constitute an important safety practice. The routine practice of such drills fosters a calm, competent response to an emergency when it occurs. Documentation of a monthly fire drill was not available March 2026. The last quarterly emergency drill was dated December 10, 2025. An emergency drill was not documented for the first quarter of 2026. The operator stated that March just slipped by her. You practiced a “naptime” fire drill today and corrected the violation related to fire drills. (Don’t forget to document it on your Fire Drill Log. Consider creating a Google Calendar on your phone for the beginning of each month to remind you to complete your monthly outdoor inspection. (Combine it with an outdoor inspection reminder!) Later this week, conduct an emergency drill (lock-down or shelter-in-place. For your compliance letter, state the date you completed the emergency drill and include a copy of the Emergency Drill Log with your compliance email. Resources: As a licensed child care facility, it is your responsibility to be knowledgeable of the child care laws and rules and to teach your staff the rules to ensure your center remains in compliance. Reminders: FCCH Verification Form – We reviewed your FCCH Verification Form today that is updated and provided to you with the visit summary. Use it to track items that expire or need to be updated annually, e.g., on-going training, H&S Training, CPR/FA, ITS/SIDS, CBC, EPR Plan, etc. I highlighted items that will expire in the next year and reviewed them with you. ABCMS Portal – The process of notifying the Division when you have new staff (or household members for family child care homes and centers licensed in a residence) has changed and is now captured in ABCMS, the new Criminal Background Check portal. You have completed the ABCMS Provider Portal Training and are now authorized to log in the ABCMS Provider Portal https://ncabcms.nc.gov/DCDEE/ using your Business NCID. You said that you are working through the process of getting yourself and household members linked to your facility in the ABCMS portal. I am emailing the ABCMS Provider Portal Access Guide which should provide you the step-by-step instructions to complete this. • You may need to complete the Moodle Training again or have gone back to complete the Powerform where you list your business NCID email. • If you need assistance using the ABCMS Provider Portal, contact the Criminal Background Check Unit at (919) 814-6401 or DCDEE_ABCMS_Provider@dhhs.nc.gov and someone will assist you. Nap Room – Congratulations on increasing your capacity to ten children. You currently have nine children, two years of age and older, enrolled with a tenth child starting on Monday. You have a separate room for naptime with ten toddler beds set up so that each child has their assigned bed with individual linens, pillows and blankets that are laundered at least weekly. Adding ten beds to this room is tight. Today, you demonstrated how children can safely evacuate the room in the event of a fire or other emergency. They did well and evacuated in under one minute, but I encourage you to practice this scenario more often. (Remember, the children were fully awake. Emergencies also happen when children are asleep and groggy. You want them to be confident with evacuating, so practice is key.) We also played with the configuration of the toddler beds to determine if a better layout might add more floor space. You are considering the following: putting a Plexi-glass or similar divider between both sets of four beds and changing the window on the south side of the room to a door. (Check with Town of Kill Devil Hills to determine if a building permit is required.) Both of these will be very helpful to ensure a safe sleeping environment where children can easily and safely evacuate in the event of an emergency. Additional Comments: FCC Connect & Reflect Engagement Hour - Every second Wednesday of the month from 1-2 p.m. join this relaxed space to share ideas and chat with other family child care home educators. Healthy Behaviors Helpline - The Healthy Social Behaviors Project offers coaching for child care providers who would like support managing challenging classroom behaviors. The goal is to empower teachers to create learning environments that promote pro-social skills, like self-control and problem solving. QRIS (Quality Rating Improvement System) Technical Assistance: On December 2, 2025, M. Kennon, FCCH Operator, and I discussed the facility’s plan for completing a rated license assessment using the new QRIS rules found in Section .3200 of the NC Child Care Rules. You selected Pathway 1. Pathway 1 – Program Assessment: We reviewed current staff education to determine the facility’s expected star level based on education is 4 Stars. You have completed additional ECE coursework at COA. Request your transcripts and submit them to the Workforce Unit at DCDEE. This will increase your education to NCECC + 19 ECE which will earn 5 stars for QRIS education! Complete the Family and Community Engagement Plan annually. We reviewed the foundational practices and the optional practices. She checked items that are already met and those that she feels she meets or can meet. Consider keeping a notebook or file to maintain documents that verify that foundational and optional practices are met. We walked through the FCCH Continuous Quality Improvement (CQI) Plan. The FCCH CQI form must be completed and reviewed annually by the operator. This form will replace the Professional Development Plan. The operator completed her CQI Plan on December 2, 2025. The operator currently uses Creative Curriculum for Family Child Care Homes. Your are wanting to request an Outreach Assessment for possibly June 2026 and then use the results for your self-study. The Outreach Assessment is between you and the NC Rated License Assessment Project. You set it up with them. They send you the results and review them with you. Once you receive the results, contact me so that we can move forward with your application and official request for your FCCERS-3 assessment. I completed your last Annual Compliance visit on 4/13/2023. The FCCH Early Learning Professional, M. Kennon, was present and assisted me with the visit. The family child care home currently operates with a Four-Star License, issued 6/7/2022, earning points in the following components: Program (7 PTS) + Education (3 PTS) + Quality Point (1 PT) = 11 Points = 4 Stars *Quality Point met and remains in effect: Serves no more than two (2) children younger than one-year of age. The program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum) as required for all Four and Five-Star facilities with four-year old pre-kindergarten children enrolled. The three-year rated license assessment is due 6/7/2025. See below for information pertaining to transition back to rated license reassessments after Covid and your next reassessment. The family child care home’s 18-month compliance history from 1/8/2022 – 7/7/2023 = 100%. The family child care home is owned as a Sole Proprietorship. Please contact me prior to any information changing regarding the ownership of this facility. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 4/1/2022 and Item Number Listing, effective 6/2022. I also reviewed program records. The family child care home chooses not to transport children. The operator stated that she never uses a substitute and closes the family child care home when she is not available to provide care. The Written Plan of Care currently reflects that the operator does not perform routine tasks during operating hours. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Observations The operator was present with six preschool-age children, three through five years of age. The children were waking up from nap. They were doing water play today so they changed into their swim suits and went outdoors where they played with a hose. Lunch was reported to be ham and cheese sandwiches, cucumbers, applesauce, and milk. They returned indoors for snack which was yogurt and juice. They went outdoors to swing on the front porch swing, sing songs, and wait for their parents. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 102 Number of preschool children exceeded five children, including the provider's own preschool children. Six preschool-age children, three through five years of age, were present. GS 110-91(7)(b) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The EPR Plan was not updated in the Emergency Management Portal since November 2, 2020. .1714(e ) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, M. Kennon. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 7/25/2023. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Send compliance verification letter in an email from your fcch’s official email address, marykennon41@gmail.com to my email address, Marjorie.White@dhhs.nc.gov. You may include the compliance verification as an attachment or in the body of the email. I will email a sample compliance verification letter to you. TECHNICAL ASSISTANCE WITH DOCUMENTED VIOLATIONS: Preschool Capacity – Children are not school-age they are five years of age and have attended their first day of Kindergarten (or the first day of Kindergarten has started if there is staggered enrollment.) Today, six preschool-age children were present – three years, four years, four years, four years, preschool five years and preschool five years. You recently enrolled a child, five years of age. The mother told you that he was school age because he was enrolled to start Kindergarten with Dare County Schools. The first day of Kindergarten is 8/28/2023. Until that date, he is a preschool-age child. You stated that you did not think through that the child was preschool-age because the mother said he was school-age. You immediately contacted her to inform her that he could no longer attend your program. For your compliance letter, describe your strategy to ensure that you maintain your preschool-age capacity of no more than five children younger than school-age. Emergency Preparedness and Response (EPR) Requirements – An effective EPR Plan is critical in the event of an emergency. It must be completed in the Emergency Management Portal and then reviewed, and updated if needed, annually. You review your EPR Plan annually, and there have been no changes, but you have not reviewed it in the Emergency Management Portal since 11/2/2020. You stated that Log into the Emergency Management Portal and review your EPR Plan annually and update, as needed. *DCDEE website (https://ncchildcare.ncdhhs.gov/) -> Provider tab -> Training and Professional Development -> Emergency Preparedness and Response -> Emergency Management Portal -> Log in using NCID used to create or update EPR Plan If no updates are needed, document your date of review on Page 28 and only print a new cover page and page 28. Include it with your EPR Plan. If updates are needed, revise in the Emergency Management Portal and print a new copy with the revision date. Document revision date on Page 28 and print the entire plan through page 28. REMINDERS: FCCH Verification Form – Your FCCH Verification Form is updated and provided to you as a part of your visit summary and is an excellent tool to help you stay current with items that expire, i.e., Criminal Record Checks, well water tests, pet vaccinations, CPR, First Aid and ITS-SIDS trainings, on-going and H&S training requirements, annual health questionnaire, household members, pet vaccinations, and vehicle information. We reviewed your FCCH Verification Form today, and I highlighted items that will expire in the next year. Activity Plans - Complete weekly activity plans (dated) that include developmentally appropriate activities for enrolled children. Activity plans for all children must meet the following minimum licensing requirements: *Include activities daily to stimulate the following 5 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social &emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). *Include a daily gross motor activity which may occur indoors or outdoors. *Reflect at least 4 activities daily (one of which is gross motor), one of which is offered outdoors, weather permitting. The remaining 3 activities should occur in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). *Include both free choice and adult-directed activities, which allow children to choose to participate with the whole group, in a small group or independently. *In addition, activity plans must include each of these 3 enrichment activities on at least a weekly basis: sand or water play; science and nature; and music and rhythm. A sample lesson plan form containing all required components was provided. *Materials and equipment must be available both inside and outside to support the activities on the activity plan and the daily schedule. *When school-agers are enrolled, your activity plan must also list a balance of activities that are developmentally appropriate for the age, needs and interests of the school-age children. Medication Administration – You are currently administering a prescription medication. The medication permission form was completed perfectly, and you have documented daily administration of the medication. If you are not using sunscreens provided by parents, return them or update sunscreen forms. Consider using one type of sunscreen for everyone to make application easier. Label MK (or any other household member initials on any medications in your locked cabinet that are for your own use.) ADDITIONAL COMMENTS: I reviewed information about the following with you: • New Challenging Behaviors Helpline • Military Child Care in Your Neighborhood-Plus (MCCYN-PLUS) • NC Rated License Assessment Project (NC-RLAP) Resources • NCID – Keeping it Current • Optional Water Testing for Lead – will not be optional in the upcoming year Annual Licensing Fee – Licensed facilities that have a current license (whether active or inactive) on October 1st of each year will receive an invoice for a licensing fee based on their licensed capacity. Invoices will be mailed out in November 2023 and must be paid on-line by December 2023. TRANSITION BACK TO RATED LICENSE ASSESSMENTS AND COHORTS Your facility completed a three-year rated license assessment on 6/7/2022 and has been assigned to Cohort 3 for completing your next rated license reassessment. The Cohort 3 Rated License Preparation Year is from 7/1/2025 – 6/30/2026. During your preparation year, review the FCCERS-R by obtaining the manual and checking into assessment related resources at ncrlap.org. Complete your FCCERS-R Self-Assessment. Request technical assistance and from Children & Youth Partnership for Dare County. Consider requesting an FCCERS-R from NC Rated License Assessment Project (NCRLAP). This will be a full FCCERS-R assessment and not the modified assessment that you completed in Spring 2022. If your scores help you meet or exceed your Four-Star License and you are ready to move forward with your rated license assessment, you may choose to do so. If your score is not as high as you would like, you may wait to complete it again during your Rated License Reassessment Year. The Cohort 3 Rated License Assessment Year is from 7/1/2026 – 6/30/2027. During your assessment year may choose to complete the FCCERS-R again at no charge even if you completed it during your preparation year. Any current education needs to be posted in Works. The family child care home currently operates with a Four-Star License, issued 6/7/2022, earning points in the following components: Program (7 PTS) + Education (3 PTS) + Quality Point (1 PT) = 11 Points = 4 Stars *Quality Point met and remains in effect: Serves no more than two (2) children younger than one-year of age. The program was also monitored for compliance with implementing an approved curriculum (Creative Curriculum) as required for all Four and Five-Star facilities with four-year old pre-kindergarten children enrolled. The three-year rated license assessment is due 6/7/2025. See below for information pertaining to transition back to rated license reassessments after Covid and your next reassessment. The family child care home’s 18-month compliance history from 1/8/2022 – 7/7/2023 = 100%. The family child care home is owned as a Sole Proprietorship. Please contact me prior to any information changing regarding the ownership of this facility. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 4/1/2022 and Item Number Listing, effective 6/2022. I also reviewed program records. The family child care home chooses not to transport children. The operator stated that she never uses a substitute and closes the family child care home when she is not available to provide care. The Written Plan of Care currently reflects that the operator does not perform routine tasks during operating hours. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Observations The operator was present with six preschool-age children, three through five years of age. The children were waking up from nap. They were doing water play today so they changed into their swim suits and went outdoors where they played with a hose. Lunch was reported to be ham and cheese sandwiches, cucumbers, applesauce, and milk. They returned indoors for snack which was yogurt and juice. They went outdoors to swing on the front porch swing, sing songs, and wait for their parents. I observed and documented the following violations during today’s visit. GS 110-91(7)(b) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The EPR Plan was not updated in the Emergency Management Portal since November 2, 2020. .1714(e ) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, M. Kennon. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 7/25/2023. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. TECHNICAL ASSISTANCE WITH DOCUMENTED VIOLATIONS: Preschool Capacity – Children are not school-age they are five years of age and have attended their first day of Kindergarten (or the first day of Kindergarten has started if there is staggered enrollment.) Today, six preschool-age children were present – three years, four years, four years, four years, preschool five years and preschool five years. You recently enrolled a child, five years of age. The mother told you that he was school age because he was enrolled to start Kindergarten with Dare County Schools. The first day of Kindergarten is 8/28/2023. Until that date, he is a preschool-age child. You stated that you did not think through that the child was preschool-age because the mother said he was school-age. You immediately contacted her to inform her that he could no longer attend your program. For your compliance letter, describe your strategy to ensure that you maintain your preschool-age capacity of no more than five children younger than school-age. Emergency Preparedness and Response (EPR) Requirements – An effective EPR Plan is critical in the event of an emergency. It must be completed in the Emergency Management Portal and then reviewed, and updated if needed, annually. You review your EPR Plan annually, and there have been no changes, but you have not reviewed it in the Emergency Management Portal since 11/2/2020. REMINDERS: FCCH Verification Form – Your FCCH Verification Form is updated and provided to you as a part of your visit summary and is an excellent tool to help you stay current with items that expire, i.e., Criminal Record Checks, well water tests, pet vaccinations, CPR, First Aid and ITS-SIDS trainings, on-going and H&S training requirements, annual health questionnaire, household members, pet vaccinations, and vehicle information. We reviewed your FCCH Verification Form today, and I highlighted items that will expire in the next year. Activity Plans - Complete weekly activity plans (dated) that include developmentally appropriate activities for enrolled children. Activity plans for all children must meet the following minimum licensing requirements: *Include activities daily to stimulate the following 5 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social &emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). *Include a daily gross motor activity which may occur indoors or outdoors. *Reflect at least 4 activities daily (one of which is gross motor), one of which is offered outdoors, weather permitting. The remaining 3 activities should occur in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). *Include both free choice and adult-directed activities, which allow children to choose to participate with the whole group, in a small group or independently. *In addition, activity plans must include each of these 3 enrichment activities on at least a weekly basis: sand or water play; science and nature; and music and rhythm. A sample lesson plan form containing all required components was provided. *Materials and equipment must be available both inside and outside to support the activities on the activity plan and the daily schedule. *When school-agers are enrolled, your activity plan must also list a balance of activities that are developmentally appropriate for the age, needs and interests of the school-age children. Medication Administration – You are currently administering a prescription medication. The medication permission form was completed perfectly, and you have documented daily administration of the medication. If you are not using sunscreens provided by parents, return them or update sunscreen forms. Consider using one type of sunscreen for everyone to make application easier. Label MK (or any other household member initials on any medications in your locked cabinet that are for your own use.) TRANSITION BACK TO RATED LICENSE ASSESSMENTS AND COHORTS Your facility completed a three-year rated license assessment on 6/7/2022 and has been assigned to Cohort 3 for completing your next rated license reassessment. The Cohort 3 Rated License Preparation Year is from 7/1/2025 – 6/30/2026. During your preparation year, review the FCCERS-R by obtaining the manual and checking into assessment related resources at ncrlap.org. Complete your FCCERS-R Self-Assessment. Request technical assistance and from Children & Youth Partnership for Dare County. Consider requesting an FCCERS-R from NC Rated License Assessment Project (NCRLAP). This will be a full FCCERS-R assessment and not the modified assessment that you completed in Spring 2022. If your scores help you meet or exceed your Four-Star License and you are ready to move forward with your rated license assessment, you may choose to do so. If your score is not as high as you would like, you may wait to complete it again during your Rated License Reassessment Year. The Cohort 3 Rated License Assessment Year is from 7/1/2026 – 6/30/2027. During your assessment year may choose to complete the FCCERS-R again at no charge even if you completed it during your preparation year. Any current education needs to be posted in Works.
