This visit was conducted with Valerie Johnson, Operator. Your program currently operates with a three-star license issued on 1/6/26. Restrictions include 1st, 2nd, and 3rd shift, children in care on ground level only, and no more than five (5) preschool aged children. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • Administering of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator to update required trainings. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked successfully. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children engaging in free play and routine activities. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 05/13/2026. A fire drill was recorded 05/11/2026. A shelter in place drill for the facility was recorded 03/12/2026. There is one (1) pet inside the home and all vaccinations and parent acknowledgement forms were monitored during the temporary time period visit on 11/17/25. The program does not provide transportation at this time. I did monitor your new vehicle that you plan to use for transportation in the future. I monitored a 2023 Chrysler Pacifica, MAH-1270. The vehicle was clean, in good repair, and had adequate tire tread. The vehicle currently has a temporary tag and is good on insurance until August 2026. The following violation was cited during today’s visit: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. During the visit, I observed that the ABCMS Provider Portal was not current and did not include the operator on the roster. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before 6/16/26. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Technical assistance discussed during today’s visit related to violations: •We discussed that the process of notifying the Division of new employees has changed and is now captured in ABCMS (Automated Background Check Management System). This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. This business NCID will be a separate NCID from the individual NCID you use to complete your background check. If you already have a Business NCID that you use in the Subsidy Portal for vouchers, you can use the same Business NCID as that one. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. This will include the operator, additional caregivers, and any household residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. You cannot transport children until I confirm you are approved for transportation. • When you send me your transportation policies, please send me a picture of the registration for your van, a picture of the Band-Aids in the van, and a picture of the secured fire extinguisher in the van. • If you do routine transportation picking the children up from home this permission can be for twelve months. You need to add the valid dates on your blanket routine transportation form. You will also need to have the parent fill out a Transportation Form that is found on the Provider Documents tab in addition to the blanket transportation form you have created. • If you do off premise activities, such as walks or field trips, you will complete the Off Premise Activity form for each activity that you take the child on and for each child going. If the activity is going to be a standing activity, you would include the valid dates on the Off Premise Activity Form. If the Off Premise Activity is a one-time event, you do not have valid dates. • We discussed the recognizing and responding training during the visit. This visit was conducted with Valerie Johnson, Operator. Your program currently operates with a temporary license issued on 11/14/2025. Restrictions include 1st, 2nd, and 3rd shift, children in care on ground level only, and no more than five (5) preschool aged children. The license was observed, and the restrictions were found in compliance. Ms. Johnson is a sole proprietor. During today’s visit, all areas of the home used for childcare were monitored, including the outdoor play area. All children’s, program, and operator records were reviewed. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. I observed materials and activities that were available to the children in the licensed space. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator. We discussed owner status of this facility. Program records and required postings were monitored. A fire drill and emergency drill have not been conducted as today is the first day of operation. An outdoor inspection was documented on 11/17/2025. There is one (1) pet inside the home and all vaccinations and parent acknowledgement forms were monitored. Operator file and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. The program does not provide transportation at this time. Ms. Johnson inquired about starting transportation. We had a discussion regarding requirements that must be met prior to transporting children. You stated that you want to do routine transportation for all ages of children. During today’s visit, I monitored emergency information and permission to transport forms for all children being transported. I observed first aid kit and fire extinguisher on the vehicle. I monitored one (1) vehicle today. I observed a 2020 Kia Optima LX/S, FFF-5170. This vehicle was clean, in good repair, had working seat belts, had adequate tire tread, had current car seats, and was current on inspection and insurance. Your next inspection date on this vehicle is due December 2025, and the insurance is valid through February 2026. The following violation were cited during today’s visit: Violation Number Comment Rule 2042 Swimming pool on the premises was not enclosed by a fence at least four feet high, or the fence was not locked, to prevent access by children. During the visit, I observed a fence that measure 3'10" enclosing a swimming pool. The fence was unlocked and included protrusions. I also observed a section of a fence measure 3'9" between the garage and privacy fence. .1730(i) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance discussed during today’s visit related to the violation: • During the visit, we discussed that all fencing must measure at least four (4) vertical feet tall. We also discussed that the fence cannot include any protrusions. If you decide to remove the protrusions from the current fence, make sure you enclose the openings with a cap and add something to the top to ensure the height is at least four (4) vertical feet tall. The fence around the swimming pool must be locked at all times as well. We discussed that you would need to add a lock to the gate where the swimming pool is enclosed. We also discussed that the small fence separating the garage and the privacy fence must measure four (4) vertical feet tall as well. Additional Information/Consultation: •You must notify your consultant 30 days prior to changing the ownership status of your facility. •During the visit, we discussed the QRIS (Quality Rating Improvement System) modernization plan. I completed a template documenting the pathway you plan to pursue, identifying needs, documenting a timeline for completion, resources, next steps, etc. Over the next few months, I’ll be checking in with you, ahead of your next visit to check on progress and determine if additional assistance is needed with WORKS, NCRLAP/ERS, accreditation, etc. •We discussed that your background check is due for renewal by December 8, 2025. I recommend completing this now. We discussed during the visit that you need to create an Individual NCID to complete this process rather than using your Business NCID that you use for your subsidy portal. You will use this link to find the Background Check Portal: https://ncchildcare.ncdhhs.gov/Home/DCDEE-Sections/Criminal-Background-Check-Unit For any questions about your background check, please call, (919) 814-6401. This needs to be completed prior to completing the ABCMS Provider Portal. •Child Care Centers with a Temporary License in good standing may be eligible for participation in the NC Child Care Subsidy System during their first 6 months of operation. Legislative changes, however, will only allow centers with a 3 Star or higher license to continue to participate in the Subsidy System after the first 6 months of operation. You should begin making plans now to obtain at least a 3 Star License by the expiration of your Temporary License if you intend to participate in the NC Subsidized Child Care System. •Make sure you have an updated WORKS letters on file available for review. When evaluating education, we can only use the WORKS letters to determine your star rating. •Make sure you add the directions from the facility from a central location to your Emergency Preparedness and Response Plan. •If you have not, you must register with the Department of Justice to receive e-mail or phone call notification if a sex offender moves into your area. Contact the local Sheriff's Department if a sex offender email or phone notification is received from the NC Sex Offender Registry. •We discussed that you could use a divider near an outlet to designate a separate area in the childcare space for a parent who may be breastfeeding if you enroll any infants. •We discussed that if you use screen time, the screen time must be linked to NCFELD (North Carolina Foundations of Early Learning and Development) goals. It also must be documented on a screen time log. Screen time must not be accessible to children ages two (2) years old and younger. We discussed these specific forms during the visit to ensure you are using the most current forms: summary of the childcare law, safe sleep policy, and the child file checklist. Pam emailed you a link to the Provider Forms tab during the visit. •Childcare applications must be updated annually. If no changes have been made, the parent can sign and date the application verifying that it has been checked annually. •We discussed the Ready to Go File during your visit. A ready to go file must consist of a copy of the Emergency Preparedness and Response Plan, contact information for individuals to pick-up children, each child's Application for Child Care, medication authorizations and instructions, any action plans for children with special health care needs, a list of any known food allergies of children and additional caregiver, additional caregiver contact information, Incident Report forms, an area map, and emergency telephone numbers. •We discussed that you want to provide transportation. Prior to providing transportation, you need to implement transportation policies outlining Section .1723 in the Child Care Rules and verifying how you will meet these rules. Once you send me these, I will look at them. Once I confirm your policies are completed, you would be eligible to transport children. You cannot transport children until I confirm you are approved for transportation. •If you do routine transportation picking the children up from home this permission can be for twelve months. You need to add the valid dates on your blanket routine transportation form. You will also need to have the parent fill out a Transportation Form that is found on the Provider Documents tab in addition to the blanket transportation form you have created. •If you do off premise activities, such as walks or field trips, you will complete the Off Premise Activity form for each activity that you take the child on and for each child going. If the activity is going to be a standing activity, you would include the valid dates on the Off Premise Activity Form. If the Off Premise Activity is a one-time event, you do not have valid dates. •We discussed that you could go to DCDEE’s website and open the Childcare Rules (Chapter 9) and save the document as a PDF. Once the document is saved as a PDF to your computer, you can open the file and click Ctrl and F. A search bar will appear in the upper right corner of the document. You can use that search bar to search for a word or number to help you navigate the childcare rules. •Make sure K. Ferguson has the top part of the medical form completed by the parent. •We discussed that when children are present, if they are receiving subsidy for that shift, they must be in licensed space. We discussed that if you have children present on the weekend that are enrolled at your facility and receive subsidy for any shift, all children must be in licensed spaces only. •Pam emailed you a copy of the NC Foundations of Early Learning and Development digital version during the visit.
