Upon my arrival I was greeted by Jamie Henderson, the center director. I completed a general walk-through of the indoor area consisting of seven (7) classrooms, the kitchen, and the outdoor play environment. I observed a total of fifty-eight (58) children present. The children were observed completing routine care tasks, outdoor play and preparing for nap time. Today’s meal was observed and in compliance with the Meal Patterns. ** License Status: This program currently operates on a GS 110-106 Notice of Compliance issued on September 27, 2012. ** Annual Inspections: The last sanitation inspection was completed on April 22, 2025, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on July 31, 2025. The last documented fire drill was conducted on October 30, 2025, the last documented lockdown drill was completed on October 31, 2025, and the last documented playground inspection was completed on October 30, 2025. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance logs, safe sleep logs, transportation, and I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A tube of Aquaphor had an expiration of March 2025. .0803(12) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were accessible to children in classroom designated for two(2) years old's. .0604(q) The violations cited during today’s visit were corrected therefore, a letter of compliance is not needed. Prior to today’s visit, the 18-month compliance history for this facility was 92%. During today’s visit we discussed the importance of keeping plastic bags and small pieces out of classrooms that are designated for children three and under. Plastic bags and small pieces are choking hazards. Ms. Henderson stated that she understands. During today’s visit, I observed a tube of Aquaphor that had an expiration date of March 2025. It is important that dates are checked when diaper cream is received and each month to ensure that expired medicines are not given to children. Upon my arrival, I was greeted by the director Jaime Henderson. Limited monitoring of the child care requirements occurred during today’s visit. This facility operates as a GS 110-106. I was accompanied as I completed a general walk-through of the indoor environment. A total of sixty-one (61) children were present. Twenty-eight school age children went on a field trip with three caregivers. Allegation(s): *There are concerns that a child was left unsupervised. During today’s visit, I discussed the self-report with the director. The director was given the opportunity to share her perspective regarding the information surrounding the allegations. I also reviewed children’s and staff files. Ms. Henderson stated that at 3:19 pm three caregivers were caring for three (3) infants and two (2) one year old children. They were preparing to line up for a group photo and to participate in the Independence Day parade. At 3:20 pm a one-year-old child was dropped with the classroom having a total of six (6) children at present. The last teacher walked out of the classroom at 3:24 pm. Leaving a child behind in a crib for 10 minutes. One of the caregiver’s notices one of the children’s parents’ car and it triggers her to ask the other caregiver where the child is. The third caregiver overheard the conversation and runs back to the classroom with a child in hand and doesn’t see anyone there. The child’s mother goes inside the classroom and sees her child sleeping in the crib. Based on my observations and statements made by the director, the allegations regarding there are concerns that a child was left unsupervised is CONFIRMED Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child was left unattended for 10 minutes. .1801(a)(1-5) Once Ms. Henderson found out what happened she met with the family and apologized for the incident occurring. She stated that she was going to ensure that something like this would never happen again. Ms. Henderson then met with the staff and the administration team to determine what happened, and to implement a plan so that this would not happen again. All violations must be corrected immediately. A compliance verification letter is due to me by August 5, 2025. In the letter, please address each violation, how the violations were corrected and how compliance will be maintained in the future. In addition, a return visit will occur within the next two (2) weeks to ensure compliance has been maintained. Technical Assistance/ Consultation: As a reminder, it is important to ensure that your staff know where the children are at all times. Face to name head count sheets are important during the transitions of the day. Please ensure that a review .1801(a) (1-5) Children shall be adequately supervised at all times in child care centers. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; and (5) staff must be able to see and hear children aged birth to five years old while the children are eating. Name of Operation: KENLY MISSIONARY BAPTIST CHURCH CHRISTIAN CCC Facility ID: 51000749 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: 0225-138L Visit Date: 2/17/2025 Number Present: 71 Completed Date: 2/17/2025 Age: From 0 To 9 Total Minutes: 90 Time In: 11:00 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to investigate a complaint received by the Division of Child Development and Early Education (DCDEE) on February 13, 2025. Upon my arrival, I was greeted by the director Jaime Henderson. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I was accompanied by Ms. Henderson and assistant director Hailey Newson as I completed a general walk-through of the indoor environment. I monitored staff/child ratios, adequate/approved space, supervision, interaction with children, discipline policy, and staff handbook. A total of seventy one children (71) were present. Allegation: *There is a concern that a child was not attended in a nurturing and appropriate manner or in keeping with child’s developmental needs in the classroom for one year old children. During today’s visit, I discussed the allegations with Ms. Henderson. She was given the opportunity to share her perspectives surrounding the allegation.Ms. Henderson stated that a parent came to her February 12, 2025, to report an incident that occurred in the classroom designated for one year old children on February 11, 2025. She stated that a parent reported observing her child restrained on a cot during naptime by the caregiver’s leg. Ms. Henderson informed the parent that she would investigate the matter and in the meantime move her child to another classroom. Ms. Henderson reviewed the video footage after work hours and determined that the incident did occur. Ms. Henderson reviewed footage with the administration team and determined to terminate the caregiver’s employment. Ms. Henderson self-reported this incident to DCDEE. During my review of the video footage for date in question, I observed a caregiver sitting down in a rocking chair with an iPad/tablet in her hand and a child attempting to get up from her mat. The caregiver picks the child up from the back of her pants and puts her back on the cot. The child then attempts to get back up. The caregiver pushes the child back down with her leg and holds her leg in place as the child tries to wiggle out of the restraint. The caregiver continues to hold the child down with her leg for approximately four (4) minutes. The parent is then seen going over to the child's cot to pick her up. I also reviewed a message in the ProCare application from the caregiver to the parents saying she restrained the child. I reviewed staff file and cell phone policy. Based on statements and observations through video footage, the allegation regarding a child not attending in a nurturing and appropriate manner or in keeping with child’s developmental needs in the classroom is SUBSTANTIATED. The following violations were cited during today's visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A caregiver restrained a one year old child on a cot with her leg during nap time. G.S. 110-91(10) 1877 A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk. At nap time a one year old child was trying to sit up from her cot when a caregiver restrained the child with her leg. .1803(a)(10) Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of the supervision violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. During rest time children should not be forced to lay down or take a nap. If children do not go to sleep a quiet alternative should be provided while other children are resting. Ms. Henderson reviewed the discipline policies, cell phone policy and the use of technology handbook policy. She stated that an addendum will be made to the cell phone policy and use of technology policy. She has scheduled an in house behavior management training on February 20, 2025. Ms. Henderson has reached out to the Johnston County Partnership for children to schedule Teaching with Heart training.
