Upon arrival I was greeted by Ms. P. Mathews -Administrator/Co-Owner who accompanied me on the walk through of the facility and assisted with today’s visit. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The preschool children were observed playing outside on the playground. You stated you had shifted some children to the two and three’s year old classrooms for today. This will be reflected on the children’s enrollment worksheet. The lunch menu for today consisted of pepperoni pizza, peaches, carrot stripes, and milk. This program currently operates with a four-star license issued May 1, 2019. Please be reminded to have all staff’s education sent to WORKS to be evaluated. Your last fire drill and monthly outdoor playground inspection were completed 8/15/25. Your last EPR drill was a lockdown completed 8/14/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. Violations documented were corrected during the visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. I observed three bottles for one infant with a date of 8/3/25 during the visit. 15A NCAC 18A .2804(d) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Name of Operation: THE RIGHT START FAMILY SOLUTION Facility ID: 31000234 Consultant: CARLA HANCHEY-EDWARDS Operation Type: Center Case Number: 0725-324L Visit Date: 7/30/2025 Number Present: 24 Completed Date: 7/30/2025 Age: From 0 To 11 Total Minutes: 80 Time In: 09:00 AM Time Out: 10:20 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to investigate allegations of non-compliance related to safe indoor / outdoor environment and discipline. These allegations were received by the DCDEE on 7/22/25. Upon arrival I was greeted by one of the Owners/Administrator Ms. P. Mathews. The nature of the complaint was reviewed with you. Based on our conversations and on the information provided, the following was determined: During today’s visit I observed indoors the vinyl flooring was torn in some areas of the facility. While observing the outdoor playground areas I observed a small broken plastic tunnel and the fence gate hanging off the top hinge that separates the infant/toddler from the preschool playground. Based on the information provided and on observations made the allegation pertaining to safe environment indoor/outdoor was confirmed. During the visit I spoke with Ms. Mathews and five staff members about anyone screaming or yelling at the children in care. The classrooms are right next to each other and if anyone was screaming or yelling the staff members would be able to hear it clearly. Ms. Mathews, along with the staff members, stated they had not heard any staff members screaming or yelling at any child in their care. During today’s visit I did not hear anyone screaming or yelling at any child in care. Based on the information provided and the observations made, the allegation pertaining to discipline was not confirmed. I completed a walk-through of the facility, and the electrical outlets were covered that were not being used. Limited monitoring of the child care requirements occurred during today’s visit, there was a total of 24 children present ranging from ages 0-11 years old. The following violation was documented during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The vinyl flooring was torn in some areas of the facility which could cause a potential tripping hazard. In the outdoor playground area I observed a gate hanging by one hinge that separates the preschool playground from the infant/toddler playground and one small broken plastic tunnel on the preschool playground. 10A NCAC 09 .0601(a) The violation must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By August 13, 2025, please send me a letter addressing the violation, how it was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov . If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. An extension from management will need to be requested. In addition, an administrative action against the facility may be recommended. A checklist was used to note the requirements I monitored today. Today Ms. Mathews, Owner/Administrator, accompanied me on the walk through and assisted with today’s visit. During the visit you stated you had space#4 closed today and had moved the children to space #3 and space #5 for today. This will be reflected on the children’s enrollment worksheet. This program currently operates with a four -star license issued May 1, 2019. Please be reminded to have all staff’s education sent to WORKS to be evaluated. The hold harmless has been extended until further notice. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files and staff files were reviewed, and the children’s record worksheet was completed. Two infants were observed in their cribs, one was sleeping, and one was awake. The toddlers were observed playing with toys. The preschool children in spaces #3 and #5 were observed playing in activity areas inside their classrooms. The school-aged children were not present during the time of the visit. You stated the schools were closing today at lunch and would be closed on Wednesday due to the inclement weather coming. You stated you would be closing today at 2pm due to inclement weather. The theme in the lesson plans was the alphabet and the sounds they make. The lunch menu for today consisted of chicken with noodle soup, kidney beans, strawberries, and milk. Your last fire drill was completed 12/13/24 and your monthly playground inspection was dated 12/13/24. Your last EPR drill was a shelter-in-place dated 11/18/24. I reminded you an EPR drill must be completed every three months and to keep all documentation on file for review. Your last fire inspection was dated 1/26/24. Your last sanitation inspection was completed 9/26/24 with an approved rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. During the visit you stated you transport children with 2023 Jeep Grand Cherokee and the inspection expires 7/31/25 and your insurance expires 4/20/25. The Toyota 2005 mini van inspection expires 2/28/25 and your insurance expires 4/29/25. You stated some of the school aged children ride the bus and get off at the facility. Today three violations were documented and corrected during the visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member with a hire date of 2/8/24 had not completed first aid training until 1/14/25. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member with a hire date of 2/8/24 had not completed CPR training until 1/14/25. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff members with hire dates of 2/8/24 and 9/11/24 did not complete training within 90 days of employment. The training has been completed as of 12/12/24. .1102(g) Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. Please be reminded if a new employee is hired, they must complete CPR, First Aid, and the Child Maltreatment training within 90 days of employment. Today violations were documented and corrected during the visit; although the staff had completed the trainings they were not completed within 90 days of employment with the facility.
