During today’s visit I completed a walk-through of the facility. Supervision, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The lunch menu met the meal pattern requirements. During the visit the group size and ratio were not in compliance and violations were documented. You stated space # 5 (toddler classroom) was closed today due to the teacher being absent. You had 6 (six) one year old children enrolled in space # 5 and three of the children were present today. You stated you had placed one child one child in space #2 (infant/toddler) the other two children in space #3 which already had a total of eight children present ranging from age two to three years of age in the classroom. I explained you could not have one year old children in a space with three-year-old children for the day. You stated you would correct this and move the two one year old children to space#2 (infant/toddler) and you would stay in the classroom until an extra staff member arrived. I observed in space #2 a total of seven children, 2 infants and 5 toddlers with two staff members present and you were in compliance now with your staff child ratios and group size. Children were observed during the visit playing in activity areas inside of their classrooms. The program currently operates with a four-star license issued March 8, 2023. During the visit I discussed the importance of reminding you to make sure all staff have sent their education to WORKS to be evaluated. Today two substitute files were reviewed and the staff and training worksheet was completed. Your last fire drill was completed 2/19/26, and your last monthly outdoor playground inspection was dated 2/20/26. Your last EPR drill was a shelter-in-place completed 1/26/26. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. NC Pre-K During today’s visit, I monitored the one NC Pre-K classroom requirements in section .3000 of the child care rules. The center uses Creative Curriculum and completes the Teaching Strategies Gold checklist to document the evidence of children’s ongoing process. The checkpoints are completed three times a year during the fall, winter, and spring. A parent teacher conference is held three times a year or more if needed. Newsletters are sent home monthly which include information on current themes, activities, and upcoming events. Upon arrival the children were playing in activity areas. The theme in the lesson plan was Black History Month. During the visit I reminded you would need to have the Environmental rating scales completed and score at least a 5.0 or higher to continue having an NC PreK classroom. A checklist was used to note the requirements I monitored today. During the visit the Administrator Ms. P. Outlaw accompanied me on the walk-through of the facility and assisted with the visit. Prior to the visit I reviewed your corporation status as current-active on the Secretary of State website. This program currently operates with a four-star license issued March 8, 2023. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. You stated you were currently enrolled in Adm. 1 and 2 through James Sprunt Community College. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, the children were observed playing in activity areas inside their classrooms. Today the lesson plans had were on colors and numbers. The lunch menu consisted of hotdogs with wheat bread, baked beans, watermelon, and milk. NC Pre-K The NC Pre-K monitoring was not completed during today’s visit. School in Duplin County just started August 25, 2025 and the NC Pre-K children are staggered in on the first week of school. Once all of the NC Pre-K children are enrolled a monitoring visit will be completed in the future. Your last fire drill and monthly outdoor playground inspections were dated 8/5/25. The EPR drill was a shelter-in-place completed 5/5/25 and you stated you had one scheduled for today. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last fire inspection was dated 7/23/25.Your last sanitation inspection was completed 6/18/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. A checklist was used to note the requirements I monitored today. During the visit the Administrator Ms. P. Outlaw accompanied me on the walk-through of the facility and assisted with the visit. Prior to the visit I reviewed your corporation status as current-active on the Secretary of State website. This program currently operates with a four-star license issued March 8, 2023. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. You stated you were currently enrolled in Adm. 1 and 2 through James Sprunt Community College. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, the children were observed playing in activity areas inside their classrooms. Today the lesson plans had were on colors and numbers. The lunch menu consisted of hotdogs with wheat bread, baked beans, watermelon, and milk. NC Pre-K The NC Pre-K monitoring was not completed during today’s visit. School in Duplin County just started August 25, 2025 and the NC Pre-K children are staggered in on the first week of school. Once all of the NC Pre-K children are enrolled a monitoring visit will be completed in the future. Your last fire drill and monthly outdoor playground inspections were dated 8/5/25. The EPR drill was a shelter-in-place completed 5/5/25 and you stated you had one scheduled for today. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last fire inspection was dated 7/23/25.Your last sanitation inspection was completed 6/18/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. During today’s visit I reminded you to use the staff and substitute checklist that is found on the DCDEE website under provider documents. We also discussed if you have a new hire, they must a negative TB test, medical assessment, and approved qualifying letter prior to employment. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law was posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The children were observed playing in activity areas inside their classrooms since it was raining lightly outside. During the visit one child aged three from space 3 was in space 6 today you stated to meet staff /child ratios. This is reflected on the enrollment worksheet. Limited monitoring of child care requirements occurred during today’s visit. The following violations were confirmed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On March 6, 2024, staff members failed to ensure a safe environment for children. A staff member brought candy to the facility from their personal residence and the candy was provided to some of the children at the end of the day. After leaving the facility, two four-year-old children ingested pills that were illegal narcotics contained in a candy wrapper that had been provided by the staff member. The children began exhibiting symptoms of a drug overdose and had to receive medical attention. 10A NCAC 09 .0601(a) 907 Discipline was related to food, rest or toileting. On March 6, 2024, staff members provided candy to children for good behavior. .1803(a)(4-6) 1810 There was a substantiation of child maltreatment.
