A rated license assessment visit was completed during today’s visit. The checklist was used to note the requirements I monitored today. Upon arrival the Family Specialist, J. Williams, was not present. Ms. S. Lisane of Duplin County Prekindergarten was on site and assisted with the visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. A random selection of staff files was monitored for compliance. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. The lunch menu met the meal pattern requirements. LICENSE STATUS: Currently this facility operates with a five-star license issued March 5, 2018, earning seven (7) points in education component, seven (7) points in program standards, and one (1) quality point based on programmatic options of having staff benefits package and infrastructure of parent involvement. Your last fire drill was completed 1/15/26 and your last monthly outdoor playground inspection was dated 1/7/26. Your last EPR drill was a lockdown completed 1/16/26. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. Your last fire inspection was dated 3/13/25. Your last sanitation inspection was completed 12/2/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. NC-PREK During today’s visit, I monitored the two NC Pre-K classrooms for requirements in section .3000 of the child care rules. The center uses Creative Curriculum and completes the Teaching Strategies Gold checklist to document the evidence of children’s ongoing process. The checkpoints are completed three times a year during the fall, winter, and spring. A parent teacher conference is held three times a year or more if needed. The classrooms operate from 8:00am-2:30pm. Newsletters are sent home monthly which include information on current themes, activities, and upcoming events. Both classrooms were observed where the children were playing in activity areas. The lesson plans were on Valentine’s Day activities and space. Prior to today’s visit, the 18-month compliance history for this facility was 97%. The following violations were documented and corrected during the visit. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. Verification the EMC plan was had been reviewed annually was not available for two staff members during the visit. 10A NCAC 09 .0802(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Verification the EPR plan had been reviewed annually for two staff members was not available during the visit. .0607(e) Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. RATED LICENSE ASSESSMENT: Prior to the visit I received your completed rated license application paperwork on 2/10/26. The facility operates as an office of Head Start Grantee site and has chosen Pathway 3: Accreditation and Head Start Pathway for a five -star license. The program meets the Head Start and Early Head Start standards including the staff/child ratios. Please be reminded your rated license application will be submitted to my supervisor pending final approval, this facility will receive a five-star license. If you do not receive your new license within a couple of weeks, please contact me. Technical assistance regarding the following items were reviewed with you today. I reminded you the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. During the visit violations were documented for the annual review of the emergency preparedness plan and for the emergency medical care plan. The person responsible for that paperwork was not on site but was called during the visit. She stated the paperwork had been completed and reviewed with the two staff members; however, it could not be located during the visit. The paperwork was signed today, and the violations were marked corrected during the visit. Upon arrival I was greeted by Ms. Williams, PreK Family Specialist who assisted with the visit. Today Ms. S. Lisane Eligibility Recruitment Selection Enrollment and Attendance Manager from Duplin County Schools was also present during the visit. Ms. S. Lisane stated she works with all of the Prekindergarten sites in Duplin County. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The preschool children in space #107 were observed playing outside on the playground. The children in space #108 were observed playing in activity areas inside their classroom. The lesson plans for both classrooms were on transportation. The lunch menu for today met the meal pattern requirements. This program currently operates with a five-star license issued March 5, 2018. Your last fire drill was completed on 10/31/25. The monthly outdoor playground inspection was completed 11/3/25. Your last EPR drill was a shelter-in-place completed 9/26/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. The following violation was documented and corrected during the visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).Two employee's qualification letter expired on 6/12/24 and 8/13/25. This program currently operates with a five-star license issued March 5, 2018. Please be reminded to have all the staff’s education sent to WORKS to be evaluated. The hold harmless has been extended until further notice at this time. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, and summary of NC Child Care Law, were posted at the time of the visit. Newsletters are sent home monthly which include information on current themes, activities, and upcoming events. The last rating scale assessment overall score received was 5.69. Upon arrival the children in space #107 were playing in activity areas. The children in space #108 were preparing to go outside on the playground to play. The theme in the lesson plans were dental health and Kindergarten. The lunch menu consisted of spaghetti with meat balls, green beans, rolls, sliced oranges, and milk. Your last fire drill was completed 1/29/25, and your monthly playground inspection was dated 2/24/25. Your last EPR drill was a shelter-in-place dated 9/11/24. A violation was documented due to another EPR drill was not completed within the three-month timeframe. I reminded you an EPR drill must be completed every three months and to keep all documentation on file for review. Your last fire inspection was dated 3/13/24. Your last sanitation inspection was completed 5/6/24 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. Two violations were documented during today’s visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last EPR drill documented on file was a shelter-in-place completed on 9/11/24. .0604(u);.0302(d)(8) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff members with hire dates of 9/2023 and 3/21/24 did not complete the Child Maltreatment training until 1/12/24 and 1/31/25. .1102(g) The violation must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violation was corrected. The compliance letter must be received no later than March 11, 2025. Contact me if you have questions regarding correction of the violation documented during today’s visit. If you are unable to correct the violation within the specified timeframe, send a compliance letter regarding the violation that was corrected. An extension will need to be requested from management. During today’s visit I explained the importance of completing the EPR drills within the timeframe to meet compliance with childcare requirements. Please be reminded that an EPR drill must be completed every three months. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
