V. Garcia and B. Hall, Teachers, assisted me with the visit. C. Robinson, Staff Development Coordinator arrived and assisted. Your program currently operates with a five-star license, issued February 27, 2026, as part of the QRIS Pathway #3 for Accreditation/Head Start. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety three percent as of April 15, 2026. The NC Secretary of State website was reviewed on March 12, 2026, and Inc Choanoke Development Association of North Carolina, Incorporated. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; indoor and outdoor areas used by children were monitored. Children were adequately supervised and capacity was met. Children were transitioning from a teacher directed activity into free choice play, completing toileting and hand-washing routines, interacting with caregivers and eating lunch. Proper hand-washing procedures were met. Lunch consisted on bacon cheeseburgers, French fries, pears, and milk. Caregivers interacted appropriately with children and ensuring their developmental needs were met. The posted activity plans included developmentally age-appropriate activities and were current. Artwork was observed in the classroom that corresponded with the activities listed on the posted plans. The following violations were documented. We talked about providing floaters that work across multiple centers with a transportable file to ensure access is available upon request during monitoring visit. Staff contacted the administrative office to request copies of documentation from the file. MONTHLY OUTDOOR INSPECTIONS: Outdoor play inspections for months of December 2025 through March 2026 were not available for review. You contacted the administrator about location of the outdoor inspections and were told the inspections had been completed; however, they were with her. The administrator added she would bring copies of the inspections to the center tomorrow. I recommend making copies of program records and leaving them at the center. Best practice would be to designate a staff member to provide access to specific program records needed for monitoring and ensuring that person can provide the records during visits. QRIS STATUS: A permit for a five-star license as part of Pathway #3 Accreditation/Head Start effective February 27, 2026, was issued to your program; however, it does not reflect the head start ratios as a requirement to be met. I informed you a new permit packet was resubmitted to add the restriction for meeting head start staff/child ratios. Once the new permit is received, it should replace the current permit posted at the facility. Also, the head start staff/child ratio charts should be posted in the classroom. I encouraged you to review the head start ratio requirements and charts with staff to ensure understanding of the changes and implementation required. SURVEY FEEDBACK: Elizabeth City State University (ECSU) is exploring the possibility of offering their Early Childhood Education program in an online format, potentially as early as this fall. A survey was released today to gather interest and feedback from individuals who may want to take courses. The survey should be completed by this Friday. L. Riddick, Center Manager assisted me with the visit. Your program currently operates with a five-star license, issued March 13, 2020, earning 7 points in the education component, 7 points in the program standards component (meeting reduced ratios minus 1) and 1 quality point for offering a staff benefits package and infrastructure of parent involvement. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on January 8, 2025. The sanitation inspection was completed May 1, 2025, with a “Superior” classification. The last fire inspection was conducted on November 4, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. TECHNICAL ASSISTANCE/GUIDANCE: CPR/FIRST: First aid and CPR training certificates on file for two employees were issued by an agency that is not approved by the Division. I informed you that these certificates were not valid and training would need to be retaken through an approved agency. I also explained that online First aid/CPR courses are not permitted and therefore cannot be accepted. I informed on the days and hours you are complete administrative duties at the second center location, and the two teachers are the only other staff at the site, a plan must be implemented to ensure someone is always onsite with CPR/First Aid certification. You stated your schedule would be readjusted to ensure you are present until the training has been completed. While distance learning (pre-recorded training) is not permitted for CPR or First Aid, a blended learning course (including face-to-face and virtual instruction) is permitted; so long as the course received is from an approved training organization and the student’s competency has been evaluated by the instructor with a CPR/First Aid in-person test of applicable skills. For the course to be valid, meeting NC Child Care Requirements, a completion card/certificate is required that reflects successful completion of the course, following the approved organization’s standards for the specific course. Child care providers should always verify the instructor’s certification, obtain the complete address of the Training Center, and QR codes must be displayed on the certificate and cards. The statement “Taught according to AHA guidelines” or some variation of that statement does not make the course acceptable. You state the authorization form would be requested from the parent immediately. ADMINISTRATION CHANGE: You reported that you are the new administrator for West Bertie and Windsor Head Start programs. When asked about the required documentation regarding the change in administration, you stated that the notification and paperwork should have already been sent to me. I informed you the documentation had not been received and requested that it be submitted so the system could be updated. You stated that you would reach out to the Head Start Director to request submission of the information. I shared once received the information would be keyed to reflect the change. QRIS CONVERSTATION: Today we discussed the three pathways available. We reviewed in detail and discussed the requirements of your anticipated pathway choice as well as the resources, and the support you may need to assist you with the application process. During the conversation, we talked about developing a plan to complete the application process. You stated that Pathway 3Accerdiattion and Head Start is the pathway choice. We reviewed the requirements for this pathway and discussed a timeline for applying for a rated license transition to the Pathways. You stated the application would be completed and submitted in in January 2026. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. TECHNICAL ASSISTANCE/GUIDANCE: CPR/FIRST: First aid and CPR training certificates on file for two employees were issued by an agency that is not approved by the Division. I informed you that these certificates were not valid and training would need to be retaken through an approved agency. I also explained that online First aid/CPR courses are not permitted and therefore cannot be accepted. I informed on the days and hours you are complete administrative duties at the second center location, and the two teachers are the only other staff at the site, a plan must be implemented to ensure someone is always onsite with CPR/First Aid certification. You stated your schedule would be readjusted to ensure you are present until the training has been completed. While distance learning (pre-recorded training) is not permitted for CPR or First Aid, a blended learning course (including face-to-face and virtual instruction) is permitted; so long as the course received is from an approved training organization and the student’s competency has been evaluated by the instructor with a CPR/First Aid in-person test of applicable skills. For the course to be valid, meeting NC Child Care Requirements, a completion card/certificate is required that reflects successful completion of the course, following the approved organization’s standards for the specific course. Child care providers should always verify the instructor’s certification, obtain the complete address of the Training Center, and QR codes must be displayed on the certificate and cards. The statement “Taught according to AHA guidelines” or some variation of that statement does not make the course acceptable. QRIS CONVERSTATION: Today we discussed the three pathways available. We reviewed in detail and discussed the requirements of your anticipated pathway choice as well as the resources, and the support you may need to assist you with the application process. During the conversation, we talked about developing a plan to complete the application process. You stated that Pathway 3Accerdiattion and Head Start is the pathway choice. We reviewed the requirements for this pathway and discussed a timeline for applying for a rated license transition to the Pathways. You stated the application would be completed and submitted in in January 2026. North Carolina Rated License Assessment Project - NCRLAP’s mission is to promote the quality of child care by consistently and reliably assessing environments for the North Carolina Star Rated License. They collaborate with professionals in the early childhood education field to foster the development and learning of young children. Children throughout the facility were participating in group time, outdoor play, transitions and personal care routines. During circle time, the children learned about water safety. On the fenced playground they had access to slides, swings, bikes, and music. The caregivers were interacting and meeting the developmental needs for each of the children. Currently, this center operates with a Five-Star License, issued March 13, 2020, earning six (6) points in the Education Component, seven (7) points in the Program and one (1) Quality Point. The last annual compliance visit was completed on 2/28/2023. The sanitation inspection was completed on 9/8/2023, with a “Superior” classification. A fire inspection was completed on 9/13/2023 and yielded a "Satisfactory" result. The facility is approved for daytime care only. The fire inspector completed the inspection during the visit. You, Y. Demery, Behavior/ Family Service Specialist, was available and able to assist me with the visit. Ten (10) children age ranging three (3) to four (4) years old were present. Children were observed in free play in centers and completing handwashing and toileting routines. All staff and child interactions were warm and nurturing. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. At the completion of the visit, a computer-generated visit summary was reviewed, and a copy was left with you. The following violation was documented today.
