You, Yanzawolo “Kaima” Montoro, Director, assisted me with today’s visit. The last annual compliance visit was conducted on March 20, 2025. The last sanitation inspection was completed on September 30, 2025. The last fire inspection was conducted October 21, 2025, and your facility was approved for daytime care only. The program’s compliance history was 95% prior to today’s visit. The center's four star, NC Child Care law summary, safe arrival and departure procedures, emergency phone numbers, tobacco restriction, monthly menu, and Emergency Medical Care Plan were prominently posted. The following was monitored during today’s visit: supervision, staff/child ratios, permit restrictions, group size, capacity, adequate and approved space, and hazardous product storage. A walkthrough of the indoor and outdoor environment was completed. Children were observed engaging in free choice activities, meal time, rest time, and routine care taking. Fire drills are being completed monthly as required. Emergency drills are being conducted every three months as required. Incident logs were monitored during today’s visit. Incident reports are being filed in children’s individual files. Playground inspections are being conducted monthly. Your playground meets child care safety standards. Materials, toys, and equipment throughout the facility were developmentally appropriate, of sufficient quantity and in good repair. Emergency medications, permission to administer forms and medical action plans were monitored during today’s visit and found to be in compliance. A selection of staff files and children’s files were monitored during today’s visit. Today you stated your staff and training worksheet was not updated to include all new staff. I created my personal copy of the staff and training worksheet to include the staff files I reviewed today. You stated you will send an updated copy of the staff and training worksheet to me no later than one week from today, February 24, 2025. Today we reviewed TB test results for a staff member hired 11/24/25. The TB test documents did not include the year the test was completed. I reached out to the physician’s office where the TB was completed to confirm the date the test was placed and read and I am waiting to hear back. This facility does not provide transportation. We discussed the QRIS information today. I encouraged you to continue reviewing the QRIS Modernization information and documents on the DCDEE website. Also, please continue reviewing section .3200 in child care rules, which contains all the new rules related to QRIS. At this time this facility is not interested in going through the rated license process, due to not being due until February 2027. The following violation was cited today: Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. A staff member hired on 11/24/25 did not have documentation of receiving at least 16 hrs. orientation within first 6 weeks. .1101(a) Technical Assistance: Please ensure all required new staff orientation is completed within the two and six week timeframe. Please ensure all dates and hours are listed for topics discussed. Also, although you also utilize an orientation document customized to your facility, please continue ensuring that all required DCDEE orientation training topics are reviewed and documented. Ensure each new staff member also signs and dates the DCDEE orientation document, to confirm all DCDEE orientation topics are reviewed. Consultation was provided on the following: Please review and post the November 2025 Summary of the North Carolina Child Care Law and Rules brochure. Please continue ensuring that children are not sitting and waiting several minutes for their meals. During today’s visit the children in space four were sitting at the table when we arrived in the classroom waiting for their lunch that had not arrived at the facility yet. The children remained seated at the table throughout my time monitoring the classroom. You advised the classroom teachers to read a book to the children. I spoke with you and recommended not having children sit for meal time until their meal arrives in the classroom. I recommended teachers cleaning the tables around the time meals will be served, then once the meals arrive in the classroom, encouraging children to wash their hands and then come to the table. You stated meals are currently served family style. Please continue doing a walk through of the facility daily, and monitoring classrooms, ensuring teachers are speaking to and handling children in a nurturing manner. I encourage you to please keep your staff and training worksheet updated at all times to ensure items listed are not out of date and all staff files remain in compliance. You, Janique Little-Boyce, Director, assisted me with the visit. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions were monitored during today’s visit. The allegations are as follows: There are concerns regarding nurture and care. I visited each classroom during today’s visit. Children were observed engaging in free choice activities both indoors and outdoors, routine care taking, and staff/child interactions. I spoke with you and asked if you had any concerns with nurture and care. You stated you had concerns regarding nurture and care and relayed to me an incident that occurred in the toddler classroom on October 23, 2024 where a teacher roughly handled two children in the classroom on three different occasions. This incident was self-reported by you on October 29, 2024. I interviewed one staff member today and asked if they had any concerns with nurture and care. The staff member stated they had concerns regarding nurture and care and relayed to me an incident they witnessed that occurred in the toddler classroom on October 23, 2024 where a teacher roughly handled one child during rest time. I reviewed video footage of the alleged incidents and observed a child one year of age on a climber, and one staff member used the child’s arm to pull the child off the climber. Briefly while the staff member had the child’s arm the child’s feet were not touching the ground. On another occasion I observe the same staff member using the same child’s clothing to abruptly pull the child in an effort to make the child lay flat on their cot during nap time. On another occasion I observed the same staff member force a two year old child to pick up toys by grabbing the child’s arms and pushing the child forward, forcing the child to grasp the toy and place it on the shelf. During today’s visit I was able to confirm the allegation based on information received from interviewed staff and observations. Therefore, the allegation regarding nurture and care was substantiated. The following violation was cited during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On three different occasions I observed by video a staff member roughly handle two different children, ages one and two. G.S. 110-91(10) Technical Assistance was provided on the following: We discussed the administrator going over the discipline policy during the next staff meeting to ensure each staff member knows about appropriate and inappropriate discipline practices. You indicated today that the facility has a plan for the staff member in question to complete additional trainings before returning to the classroom. The following trainings have been completed: 1. Tackle Behaviors that Challenge with Kinesthetic Teaching 2. Unleashing the Leader Within You The following training will be completed: 1. Safeguarding training through Save the Children 2. Risk Mitigation 3. Recognizing & Responding to Suspicions of Child Maltreatment The staff member in question has also updated their professional development and receives a weekly check in from the Director. Consultation was provided on the following: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You, Janique Renee- Little, Director, completed the walkthrough with me and assisted with today’s visit. Your facility currently operates with a temporary license issued on August 10, 2023, and expires on February 10, 2024, with the following restrictions: Daytime care only. The last sanitation inspection was completed on October 4, 2023, and you received a superior rating and zero demerits. The last fire inspection was completed on January 5, 2023. I observed adequate supervision, staff/child ratios maintained, licensed capacity maintained, and approved space used during today’s visit. I observed sign-in and out sheets in each classroom documenting children’s arrival and departure times. I observed attendance records to be current as required. I observed your Temporary License, Safe Procedures for Arrival and Departure, current menu, sanitation placard, Emergency Numbers, Emergency Medical Care Plan, First Aid information sheet, tobacco free policy signage, Summary of NC Child Care Law dated September 2023, evacuation plans, current daily schedules, current activity plans, staff-child ratio sheets, and allergies posted as required. I observed children engaged in free choice activities, outdoor play, lunch, and rest time. I observed lunch to consist of meatloaf, green beans, peaches, a whole wheat roll, and milk. I observed monthly outdoor inspections documented on the outdoor inspection log. I observed your most recent outdoor inspection was conducted on November 27, 2023. I observed monthly fire drills documented on the Emergency Drill Log and Report form. I observed the most recent fire drill was conducted on November 29, 2023, at 1:30pm. I observed the most recent emergency drill in the form of a lockdown was conducted on September 20, 2023, at 12:05pm. I observed your incident log to be current and complete as required. Incident reports are being filed in children’s individual files. I monitored a selection of staff and children’s files during today’s visit. Rated License Summary: Program Standards The Application for Assessment for a Two Component Star Rated License form was reviewed and signed by the director on December 5, 2023. The program points will be determined by your score earned on the Environmental Rating Scale (ERS) assessment as well as enhanced ratios and space. Education Standards: Several staff members will need to submit official transcripts to the workforce unit for evaluation. Education points will be determined upon receipt of evaluation results from the workforce unit. Quality Option: The facility has chosen to earn a quality point by 75 percent of teachers having an AAS or higher in Early Childhood Education or Child Development and by using a curriculum as defined in Rule .0102(10). The facility uses Creative Curriculum as the approved curriculum. Total points will be determined upon the receipt of ERS scores. The facility will be issued a star rated license after the temporary license expires on February 10, 2023. Please remember that programs with less than a three-star rated license will not be eligible to receive subsidy funding. The Environmental Rating Scale (ERS) will be requested and a representative from NCRLAP will be in touch with you soon. The following violation was observed today: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space 3 and space 4, a bottle of Benadryl was observed in the red emergency bag. You removed the Benadryl and placed it in locked storage during today's visit. 15A NCAC 18A .2820(d) The following Technical Assistance (TA) was provided during today’s visit: 1. We discussed that Benadryl for children with emergency medications should be stored in locked storage at all times. The following Consultation was provided during today’s visit: 1. We discussed the approved agency list for First Aid and CPR training. 2. We discussed that due to the Summary of Child Care Law being updated, parents should receive the revised law and sign a statement that they have received it. If this is in your parent handbook, I advise that this law be updated. 3. We discussed ensuring that trash bags are tight fitting around the trash can or the excess is tucked in rooms caring for children under 3 years of age.
