This visit was conducted by Jennifer Davis, child care consultant with Alex Williams, on-site group leader. Currently the program operates with a Five (5) Star License issued 5/7/24. License restrictions include daytime care only; school age children only; meets enhanced ratios and space, reduced group size by one in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. This program is operated by Yadkin County Schools. The license was posted, and restrictions were met. The center’s fire inspection was conducted on 9/8/2025 . All indoor and outdoor areas were observed during the visit. Upon arrival staff/child ratio was observed and recorded on the attached worksheet. Supervision was adequate during the visit. I observed developmentally appropriate materials and activities were available to the children in the classroom. I observed positive interactions between staff and children. I observed children in a gross motor activity, homework and in center play. A current snack menu was posted. An allergy list was posted. Storage of hazardous items was monitored. There are no children currently on medications. The program does not provide routine transportation. They will go on field trips this summer. Staff and children’s records were monitored per DCDEE procedures. I observed DPI off-site verification forms for staff records, transportation. and children’s records. A fire drill was documented for 3/24/2026. A shelter-in-place drill was documented on 2/12/2026. A playground inspection was documented on 3/27/2026. Prior to the visit, the program's compliance history was 99%. All programs are required to maintain 75% compliance. The following violations were cited during the visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. A square wooden area next to storage had chipping paint and wood sections that need repair. The visit was conducted by Jennifer Davis, child care consultant, with Tammy Hutchens, on-site coordinator. Currently the program operates with a FOU4 (5) Star License issued 6/21/18. License restrictions include daytime care only; school age children only; meets enhanced ratios and space, reduced group size by 1 in each group from 7 pt. level; playground area does not meet child care playground safety guidelines. The program is operated by Yadkin County Schools. The program’s compliance history was 100% prior to today’s visit. Licensed programs are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least 75%. Any violation(s) documented may impact the compliance history score. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were observed in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. The outdoor play area is not monitored by DCDEE. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed staff reading with the children, children having free play and having snack. One new staff was reported at this program. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 11/17/23. A fire drill was recorded 11/21/23. A shelter-in-place drill for the facility was recorded 11/14/23. The most recent fire inspection was on 9/14/23. I observed a sanitation inspection was last conducted on 5/24/23 earning a superior classification. The program does not currently provide transportation. I observed the DPI off-site verification forms for transportation (5/23/23), staff records (5/23/23) and children's records (5/23/23). The following violation was cited at today’s visit. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One new staff did not receive 16 hours of orientation by June 28, 2023. .1101(a) Child Care programs are expected to achieve and maintain compliance at all times. At the completion of the visit, this visit summary was reviewed with and provided to you. If you state in your documentation that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance provided during today’s visit: I reviewed over the orientation requirements during the visit today to correct today’s violation. • For staff working with school-age children: (i) All staff shall receive on-site training and orientation as follows: (1) Within the first two weeks of assuming responsibility for supervising a group of children, each employee shall complete at least six clock hours of training on: (A) recognizing, responding to, and reporting child abuse, neglect or maltreatment pursuant to G.S. 110-105.4 and G.S. 7B-301; (B) the center's operational policies, including the transportation policy, identification of building and premises safety issues, Emergency Preparedness and Response Plan and the emergency medical care plan; (C) adequate supervision of children, taking into account their age, emotional, physical, and cognitive development; and (D) prevention and control of infectious diseases, including immunization; and (2) Within the first six weeks of assuming responsibility for supervising a group of children, each employee shall complete at least three additional clock hours of training on: (A) maintaining a safe and healthy environment and developmentally appropriate activities for school-age children; (B) firsthand observations of the program's daily operations and instruction in the employee's assigned duties; (C) instruction in the administration of medication to children in accordance with 10A NCAC 09 .0803; (D) successfully complete CPR and First Aid training appropriate for the ages of children in care; (E) prevention of and response to emergencies due to food and allergic reactions; (F) review of the program's handling and storage of hazardous materials and the appropriate disposal of biocontaminants; and (G) review of child care licensing law and rules, including an explanation of the role of State and local government agencies in the regulation of child care and the employee's obligation to cooperate with representatives of State and local government agencies during visits and investigations. (j) Staff in part-time, full day, or track-out school-age care programs required to complete BSAC training shall do so within three months of becoming employed.
