This visit was conducted with Gina Calhoun, new Director. Your program currently operates with a FIVE Star license effective 4/16/19. Restrictions include shift daytime care; meets all enhanced requirements and reduced ratios minus one; Certified Developmental Day. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center time, group time on the carpet, and outdoor play. Infants were having their individual needs met. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold in the NC Pre-K classroom in Space 5. The last annual compliance visit was conducted on 1/27/25. A sanitation inspection was completed on 12/12/25 with a Superior classification and seven (7) demerits. The last fire inspection was conducted 9/5/25 and a copy of this inspection was received during the visit today. Program records and required postings were monitored. A fire drill was conducted on 11/28/25 at 2:32 pm with two (2) minutes, fourteen (14) seconds to evacuate twenty-four (24) children and twelve (12) adults by R. McCoy. A lockdown emergency drill was documented on 11/28/25 at 3:42 pm with one (1) minute, forty-seven (47) seconds to lockdown. An outdoor inspection was documented on 6/10/25 ??. Ten (10) new staff files, one (1) staff file, and five (5) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. There was one (1) inhaler in Space 4 with required medication paperwork but no prescription box or label on the medication. The inhaler medication was already placed in the Flow vent tubing for use. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. I observed in Space 4 one inhaler with medication already placed in the Flow vent tubing. There was no prescription label on the medication nor a box with a medication label for this inhaler medication. .0803(2)(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. I observed one (1) staff who had no documentation of receipt of First Aid training within 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. I observed one (1) staff who had no documentation of receipt of CPR training with in 90 days of employment. .1102(d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. I observed one (1) staff with no documentation of receiving the Shaken Baby Abusive Head Trauma policy training prior to working with children. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. I observed two (2) staff who did not have documentation of receiving the Recognizing and Responding to Suspicions of child Maltreatment within 90 days of employment. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, January 27, 2026. Today we discussed each Pathway during the visit. • Between now and March of 2026, you will decide which Pathway is best for you and develop a timeline for applying. Today we discussed the three (3) Pathway options, and you stated that you are still undecided but leaning towards Pathway 1. Beginning in April, you will start to apply for your reassessment. Or you may apply early since you are already accredited or if you already know your Pathway. If you are choosing Pathway 1, which includes Program Assessment Self-Study, you should begin the self-study as soon as possible as it is a three-month self-study process for each classroom and must be completed prior to the rated license assessment. • The Application for Assessment for a Rated License is now live on DCDEE’s website under Provider Documenters. You are welcome to view this form and see all the required forms for each pathway. You can access the applications below for a Center. Once the application opens, you can click on the specific forms under each pathway to see the forms you will be using for the new QRIS process. Updated training modules are being added, including a Child Care Rule Rollout with new rules effective July 1, 2025, found under the Regulatory tab. Specifically, under the Orientation tab, I would encourage all staff to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. This visit was conducted with Gina Calhoun, new Director. Your program currently operates with a FIVE Star license effective 4/16/19. Restrictions include shift daytime care; meets all enhanced requirements and reduced ratios minus one; Certified Developmental Day. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 92% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center time, group time on the carpet, and outdoor play. Infants were having their individual needs met. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold in the NC Pre-K classroom in Space 5. The last annual compliance visit was conducted on 1/27/25. A sanitation inspection was completed on 12/12/25 with a Superior classification and seven (7) demerits. The last fire inspection was conducted 9/5/25 and a copy of this inspection was received during the visit today. Program records and required postings were monitored. A fire drill was conducted on 11/28/25 at 2:32 pm with two (2) minutes, fourteen (14) seconds to evacuate twenty-four (24) children and twelve (12) adults by R. McCoy. A lockdown emergency drill was documented on 11/28/25 at 3:42 pm with one (1) minute, forty-seven (47) seconds to lockdown. An outdoor inspection was documented on 6/10/25 ??. Ten (10) new staff files, one (1) staff file, and five (5) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. There was one (1) inhaler in Space 4 with required medication paperwork but no prescription box or label on the medication. The inhaler medication was already placed in the Flow vent tubing for use. The program does not provide transportation. Rebecca Rash, Director, and Crystal Scout , assistnat Director assisted me with the visit. Emily Lamquaye, child care consultant, accompanied me on the visit today to this certified developmental day program in Ashe County. Your program currently operates with a FIVE (5) Star license effective 4/16/19. Restrictions include 1st shift day time care; meets all enhanced requirements and reduced ratios minus one; Certified Developmental Day. The program’s compliance history was 90% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor areas were observed. Limited outdoor monitoring was conducted today due to a bear sighting in Jefferson today. The Sherriff’s department came out and to share that the bear was being aggressive and for children to go inside. Children came in immediately. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, having stories read to them, outdoor gross motor time and preparing for lunch indoors. One (1) new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. One inhaler medication and one diabetic medication as well as over the counter medications, including diaper creams and ointments were observed and reported. Storage hazardous items was monitored today. A playground inspection was recorded 7/12/24 with mulch being noted on the inspection form. Mulch was delivered and spread 8/1/24. A fire drill was recorded 7/29/24 @ 3:20 pm with one (1) minute , seven (7) seconds to evacuate thirty-one (31) children and fifteen (15) adults. A lockdown drill for the facility was last recorded 5/21/24. The most recent fire inspection was on 7/29/24. I observed a sanitation inspection was last conducted on 5/21/24 earning a Superior classification with four (4) demerits. You stated that you do not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 316 Children under one year of age were not kept separate from children two years and older. In Space 2, I observed an infant in the classroom with a two-year-old child. 10A NCAC 09 .0713(a)(5) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. The closet door in new Space 6 was off the hinges and propped up by a shelf. 10A NCAC 09 .0604(p) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. You are and have been in contact with Animal Control and you have alerted parents and staff of the bear sighting. • The Sherriff’s department and neighbors are keeping in contact with the director if a bear is sighted near the center. This visit was also used to monitor compliance with applicable childcare requirements located in Child Care Rule Section .3000 in space 6, where two (2) children participating in the NC Pre-K program are cared for. This visit was conducted by Karen Young, childcare consultant, with Rebecca Rash, Director, and Crystal Scott, Administrative Assistant. Your program currently operates with a FIVE (5) Star license issued 4/30/19. Restrictions include 1st shift daytime care only; meeting all enhanced requirements and reduced ratios minus one; Certified Developmental Day. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on 2/14/24 and Ashe Developmental Day School, Inc. was listed as current and active. The program’s compliance history was 93% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having outdoor gross motor play, free choice center time, eating lunch and having individual needs met in the infant room. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 3/1/23. A sanitation inspection was completed 4/17/23 with a Superior classification and nine (9) demerits. The last fire inspection was conducted 3/29/23 by B. Carson. Program records and required postings were monitored. A fire drill was conducted on 1/31/24 at 4:50 pm with forty-three seconds to evacuate thirteen (13) children and ten (10) adults by R. Rash. A lockdown drill was documented on 11/17/23 at 3:24 pm with two (2) minutes eight (8) seconds to lockdown by R. Rash. An outdoor inspection was documented on 1/26/24 by R. Rash with two (2) notations on the playground inspection sheet. Three (3) new staff and two (2) other staff files and five (5) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The developmental day requirements in section .2900 of the childcare rules were monitored for compliance. Children who receive developmental day services are currently cared for in all spaces. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .2902(f) were verified in compliance. Lead teachers assigned to the space three (3) have NC Birth-through-Kindergarten licenses and the administrator has a Level III NCECAC with over twenty-two (22) years’ experience. The program operates eight (8) hours per day, Monday through Friday, twelve (12) months per year. Children’s individual plans for care and developmentally appropriate activity plans are followed, and opportunities for inclusion with children who are typically developing are offered. I confirmed this facility provides the following five family involvement activities: quarterly parent education sessions, parent/teacher conferences twice a year; daily notes of progress provided to parents though Brightwheel; opportunities for parent volunteers to assist with special classroom activities, field trips and other learning experiences for children; and provides parents with referral information about other community programs and resources serving young children. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance for Space 6. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, K. Shatley and teachers, M. Weaver, and A. Johnson. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you were in the process of completing the Dial-4 screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/21/23. The classroom operates from 7:30 a.m. to 5:15 p.m. Parent conferences are held two (2) times per year. You stated that you use Brightwheel to communicate with parents. In addition, a report is sent home to the parents which includes any information about the child’s day and any reminders through Brightwheel. You verified that you incorporate family engagement by having open house and parent sessions. This program also has a parent advisory council. The environment rating scale score received on 2/12/19 was 6.54. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In the toddler room space 2, I observed a piece of the metal heat venting at the floor had a piece of metal sticking out. This violation was corrected during the visit today with duck tape until the maintenance man can repair. It was observed that standing water was in a small tub on the cement riding area of the playground. The water was removed during the visit today. Both violations were corrected during the visit today. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I observed an unlocked cabinet with hazardous items in the bathroom of the two's room, space 7. The teacher locked the cabinet and this violation was corrected during the visit today. .2820(b) 865 A child was given over the counter medication without written authorization from a parent to follow the direction of the NC State Health Director during a public health emergency and/or the authorization did not contain the required information. I observed an over the counter Desitin ointment in Space 7 that stated the amount as "as needed". The parent was notified and scanned a copy of the permission to administer medication during the visit. The parent scanned the permission to administer back to the school during the visit today. The permission was taken to the classroom and placed with the medication. Children’s individual plans for care and developmentally appropriate activity plans are followed, and opportunities for inclusion with children who are typically developing are offered. I confirmed this facility provides the following five family involvement activities: quarterly parent education sessions, parent/teacher conferences twice a year; daily notes of progress provided to parents though Brightwheel; opportunities for parent volunteers to assist with special classroom activities, field trips and other learning experiences for children; and provides parents with referral information about other community programs and resources serving young children. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance for Space 6. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, K. Shatley and teachers, M. Weaver, and A. Johnson. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. You stated you were in the process of completing the Dial-4 screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/21/23. The classroom operates from 7:30 a.m. to 5:15 p.m. Parent conferences are held two (2) times per year. You stated that you use Brightwheel to communicate with parents. In addition, a report is sent home to the parents which includes any information about the child’s day and any reminders through Brightwheel. You verified that you incorporate family engagement by having open house and parent sessions. This program also has a parent advisory council. The environment rating scale score received on 2/12/19 was 6.54. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In the toddler room space 2, I observed a piece of the metal heat venting at the floor had a piece of metal sticking out. This violation was corrected during the visit today with duck tape until the maintenance man can repair. It was observed that standing water was in a small tub on the cement riding area of the playground. The water was removed during the visit today. Both violations were corrected during the visit today. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. I observed an unlocked cabinet with hazardous items in the bathroom of the two's room, space 7. The teacher locked the cabinet and this violation was corrected during the visit today. .2820(b) 865 A child was given over the counter medication without written authorization from a parent to follow the direction of the NC State Health Director during a public health emergency and/or the authorization did not contain the required information. I observed an over the counter Desitin ointment in Space 7 that stated the amount as "as needed". The parent was notified and scanned a copy of the permission to administer medication during the visit. The parent scanned the permission to administer back to the school during the visit today. The permission was taken to the classroom and placed with the medication. The vioaltion was corredted during the visit today. 10A NCAC 09 .0803(9)(a-d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Thursday, February 29, 2024. All violations were corrected during the visit today. No compliance letter is required. Rash stated she will update and review with staff the changes to the EPR plan. • Keep a watch on the blue chairs in toddler room as they are starting to tear at the end of the arm rest. • When toddlers are sitting with nothing to do, waiting for lunch, sing songs, or read a book to engage the children so they are not sitting without an activity for more than three (3) minutes. • Add signs to all areas of interest in all classrooms to prepare for ITERS-R and ECERS-R. * We disussed the reopening of Space 8. Rebecca Rash, Director assisted me with the visit. Your program currently operates with a FIVE (5) Star license effective 04/30/2019. Restrictions include 1st shift daytime care; meets all enhanced requirements and reduced ratios minus one; Certified Developmental Day. The NC Secretary of State website was reviewed on 8/22/23 and Ashe Developmental Day School Inc. was listed as current and active. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having individual needs met, free choice play, and having circle time. One (1) new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items was monitored today. A playground inspection was recorded 7/28/23. A fire drill was recorded 7/31/23. A lock down drill for the facility was recorded 5/30/23. A drill needs to be conducted in August of 2023. The most recent fire inspection was on 3/29/23. I observed a sanitation inspection was last conducted on 4/17/23 earning a Superior classification. Ms. Rash stated that she is aware of the new sanitation rules ,and she plans to attend one of the September virtual meetings. She also has her clean-up team ready to go. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. I observed one child's baby bottles brought from home today labeled with a name but no date. The staff added the date on the bottles during the visit.
