This is the FCCH’s annual compliance visit. I used a checklist to assist me with the visit. This FCCH earned a Five Star License on May 6, 2022. As of March 5, 2024, the compliance history for this FCCH is 100%. Fire drills and Playground inspections were current and accessible. I used the Annual Compliance Monitoring Checklist for FCCH and the FCCH Item Number Listing (DCD 0375) as basic tools to assess compliance with applicable child care requirements pertinent to this FCCH. Upon my arrival, I was greeted by you, Ms. Lisa Brockman, owner of this family child care home. You told me you do not have guns or weapons in the home. You do not provide any type of transportation. You have hired Ms. Pam Wilkes as additional caregiver due to an upcoming surgery. The childcare room has centers, books, toys, and activities. The playground is well shaded, and you have toys for outdoor play. There are two children enrolled, and two children were present today. The children were painting an art project when I arrived. The children washed their hands and transitioned to lunch and rest time. All interactions I observed were warm and caring. I reviewed Ms. Brockman and Ms. Wilkes Verification of Information. Both need to take Child Maltreatment training. I helped Ms. Brockman locate the website for the training. Ms. Wilkes, the additional caregiver must have a negative TB test on file. To verify compliance with specific requirements, provider and additional caregiver information was recorded onto the Verification Form for FCCH and both forms were signed. Ms. Brockman completed the FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form. I received the signed forms during today’s visit. Your signature on these forms will serve as verification that the information provided is accurate and complete. I reviewed the children’s files and both children need a copy of the medical report for their file. I reviewed program files, inspections, and emergency drills. I observed two items of non-compliance today. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two children do not have a health assessment on file in the FCCH. GS 110-91(1); 10A NCAC 09.1721(a)(1) 939 Individuals who provide care for 5 hours or more a week, during planned absences of the operator, did not have proof of negative TB test or screening results completed within the last 12 months prior to the first day providing care. The additional caregiver does not have a negative TB test on file. GS 110-91(7)(b) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. The owner nor the additional caregiver have current certification of Suspicions of Child Maltreatment training. .1703(a)(5) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by March 20, 2024. Include the following information in your correction letter: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number, the date the violation was corrected, information regarding how the violation was corrected and how you plan to maintain compliance with the item number in the future. Your FCCH is in Cohort #1 for the Rated License. Your Preparation year began July 1, 2023 – June 30, 2024, and your Implementation year July 1, 2024 – June 30, 2025. I reviewed your Operational Policies and suggested some changes. We discussed the Rated License and the Self-Assessment for FCCH’s. Check for information concerning FCCERS at https://www.ncrlap.org. This website is a great resource to prepare for the FCCERS Assessment. There is also information ON THE Self-Assessment that all FCCH’s are to complete.
