Name of Operation: JAMESTOWN ELEMENTARY NC PRE-K PROGRAM Facility ID: 41002142 Consultant: SHERRY PROCTOR Operation Type: Center Case Number: Visit Date: 2/22/2024 Number Present: 15 Completed Date: 2/22/2024 Age: From 4 To 5 Total Minutes: 150 Time In: 09:45 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. I used a checklist to note the requirements I monitored today. This NCPK currently operates with a Five-Star license, effective January 15, 2019. This elementary school has one NCPK classroom. The class was on the playground; I was greeted by Ms. S. Baskerville, LT and Ms. C. Kennedy, TA when I walked to the playground. Today fifteen children were present. I observed the NCPK classroom playing outside, and I gathered attendance. Children returned to the classroom, and everyone washed their hands and prepared for lunch. The children eat in the school cafeteria. When the children returned from lunch, the children had a bathroom break, and everyone washed their hands. Ms. Kennedy put out mats and Ms. Baskerville read a story to the children, transitioning them to rest time. All interactions I observed were warm and caring. The playground has a climbing structure, and they have plenty of mulch in the fall zone and under the playground equipment. The area has room for gross motor play and vigorous play. The playground equipment has rusted areas and needs painting. A Fire Inspection was conducted on June 21, 2023, no violations were cited, and I have a copy. The latest Sanitation Inspection was conducted September 11, 2023, with a Superior score. I reviewed a sampling of children’s files. I reviewed staff files and the staff and training worksheet. Your signature on these forms will serve as verification that the information provided is accurate and complete. The center’s compliance history is 100% as of February 20, 2024. I observed one violation of childcare rules during the visit today. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The metal playground car has several rusty areas. The areas need to be sanded and painted. G.S. 110-91(6); .0601(b) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately.
