A checklist was used to note the requirements I monitored today. This DPI afterschool program currently operates with a Three Star license issued March 27, 2019. Upon my arrival, I was greeted by Mr. Terrell Easter, (ACES Site Coordinator). Mr. Terrell accompanied me on a walk-through of the spaces and the outside play area. I full assessment of child care requirements was monitored. I completed a sampling of staff and children’s files. Today 46 children were present. I observed the children arriving to the cafeteria, completing handwashing and eating snack. The sanitation inspection was conducted on April 18, 2024 and received a classification of Superior with 14 demerits. A satisfactory fire inspection was completed on October 5, 2023. During today's visit, two violations of child care requirements were cited. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. One step to the climber is broken. The car shaped piece of equipment has broken slats. G.S. 110-91(6); .0601(b) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence is coming down in places and has a hole. GS 110-91(6); .0605((i) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me on or before September 25, 2024. The compliance letter should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation numbers. The date each violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. Thank you for your cooperation during today's visit. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). This is an ACES program located in an elementary school. The program operates with a Three Star license issued March 27, 2019. Upon my arrival, I was greeted by Terrell Easter, Site Coordinator. I stated the purpose of today’s visit. A checklist was used to note the requirements I monitored today. Mr. Terrell accompanied me on a walk-through of the spaces and the outside play area. I noted attendance and observed children in various activities. There were sixty-two children present today, ages five - eleven. Children were observed entering into the ACES space, routines/handwashing, the ACES opening, and transitioning to assigned areas. I completed a sampling of staff and children’s files. A sanitation inspection was conducted September 11, 2023, with a Superior classification. A satisfactory fire inspection was conducted on November 30, 2022 and I received a copy. Emergency drills and playground inspections were documented and current. There were items of non-compliance observed during today’s visit. We completed the staff and training worksheet, you will send me the copy via email by Monday October 2, 2023. Your signature on the form will serve as verification that the information provided is accurate and complete. I observed the following violations during the visit. Violation Number Comment Rule 717 Surfacing did not extend six (6) feet beyond the external limits of the equipment or 3 feet for equipment used only by children less than 2 years of age. The ground cover is mulch, The mulch has weeds growing in it and that makes the mulch very compacted. .0605(l)(1-2) 721 All equipment and furnishings were not in good repair. The climbing equipment has many places of peeling paint, the steps up the climber is broken and sharp. There is a car shaped piece of equipment that has broken slats. Some of the slate were replaced, but not all. This is an entrapment danger. G.S. 110-91(6); .0601(b) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The fence has large holes in it and is coming down in places. These areas are entrapments and has protrusions. GS 110-91(6); .0605((i) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The mulch around all of the climbing structures does not have the required depth of surfacing. It is not the required depth, nor is it the required 6 feet from the critical height of the equipment. Grass/weeds have crept into the mulch decreasing the required fall zone. .0605(k)(1-4) In order to maintain compliance with North Carolina child care laws and rules, the above violations must be corrected immediately. Please send me a correction letter by email stating how the violations were corrected and how you plan to maintain compliance in the future. It must be submitted to me by October 12, 2023. Some of the planks have been replaced, but not all of them and this is an entrapment issue. A barrier would help contain the mulch in the areas it is required. The perimeter fencing around the playground has holes in it and is coming completely down in places.
