Child Care Consultant, Andrea Johnson, accompanied and assisted me with this visit. This program currently operates with Five Star License, issued March 6, 2020, earning 7 points in program, 7 points in education and a quality point. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. The license was observed posted. As of December 3, 2025, the facility has an 18-month compliance history of 94%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Onslow County Schools operate this program. The last annual compliance visit was conducted at this facility on March 25, 2025. Sanitation Inspection – 04/11/2025 – 5 Demerits - Superior Fire Inspection – 10/22/2025 – PASS Observations and a walk-through of the facility were completed today. All approved indoor and outdoor spaces being used by the children were observed. There were sixty-eight children enrolled and fifty four children present, aged 3-5. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. The attendance for each classroom was accurately completed. Daily schedules and activity plans were available for each group of children. Preschool children assigned to indoor spaces #T3, #T5, and #T7 were engaged in a variety of free play activities which included magnetic tiles, wooden and hallow blocks, stuffed animals, Mr. Potato Head parts, letter and number recognition cards, animals, and playdough. Preschool children assigned to indoor spaces #T6 and #T8 were observed in the outdoor space running, climbing, sliding, playing musical instruments, and stepping on stepping stones. Handwashing routines were observed in preparation for lunch which consisted of pork chop sandwiches, stir fried vegetables, pears, and milk. Staff/Child ratios were in compliance, and all children were adequately supervised. Good interactions were observed between caregivers and children. NC Pre-K Site Monitoring - The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 8:00am to 2:45pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Ages and Stages Questionnaire (ASQs)screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were completed during the beginning of the school year and on file for review. Evidence of on-going assessments was observed in children’s files and on the Teaching Strategies GOLD website. The NC Pre-K Site Monitoring Tool for 2024-2025 was completed October 29, 2025 and was available for review. It was noted that all requirements were in compliance. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. Health & Safety Training - Staff members who have been employed for at least one year and some new staff have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. Ensure this information is maintained on file. Health and Safety training must be renewed every five years. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Children’s files were not monitored today. There have been no new staff members hired since your last annual compliance visit; one staff member transferred from another DPI site and file for that staff member was reviewed today. Required qualifying letter and trainings were current for existing staff. There was one violation observed today. Violation must be corrected immediately. Continue to comply with all applicable child care rules. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. Stationary equipment did not have adequate surfacing and grass was observed growing in the surfacing provided. .0605(j) For your facility to remain in compliance with NC Child Care Requirements, violations must be corrected immediately. On or before December 18, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected as well as how compliance will be maintained in the future. The time frame is established to allow you time for submitting your compliance letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Technical Assistance regarding violations cited - Today we discussed adequate mulch on the preschool playground. The mulch is not the appropriate six inches of loose surfacing. We discussed adding more mulch to the area to ensure that the depth of the mulch is a resilient six inches. We also discussed tilling or fluffing mulch often. For the safety of the children, you must ensure weeds and debris are removed and mulch is tilled or added to the appropriate depth to avoid potential injuries to children. You do have black resilient mats also for protection. Nationally, nearly 70% of playground related injuries are from falls. Fall injuries are the result of two things: 1) The height a child falls from and 2) The material/surface the child falls upon. Always provide adequate “soft” resilient surfacing under and around stationary playground equipment. Refer to 10A NCAC 09 .0605(h)(i). If mulch is compacted, decomposed, or has grass growing in the fall zone, it is no longer considered part of the resilient surfacing. When resiliency breaks down, the surfacing is no longer safe, and children can be injured. I encourage you to kill the grass immediately. The grass will not grow back through if the mulch is tilled routinely. Tilling aerates the mulch extending the life. There were also rust observed on the gas station play equipment; paint or replace. Also ensure the back side gate is secure and working properly. There concerns were listed on the monthly outdoor inspection forms for the months of September, October, and November. A work order has been submitted; however, the issues have not been corrected. Therefore, this violation and issues must be addressed and corrected immediately. A safe outdoor environment must be always maintained. Removing hazards will allow children to be free to safely explore and to make discoveries. Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of and periodically review all the licensing requirements that apply to your type of program. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. As of March 24, 2025, the facility has an 18-month compliance history of 97%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Onslow County Schools operate this program. The last annual compliance visit was conducted at this facility on April 16, 2024. NC PreK students were present in spaces #T3, # T5, #T6, #T7, and T8. There were seventy-two enrolled, and sixty-one children present, aged 3-5. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. Daily schedules and activity plans were available for each group of children. The theme for this week was, “insects.” The attendance for each classroom was accurately completed. When I arrived, children from spaces #T5 and #T6 were observed outside playing in fenced area. The children were observed playing with balls, riding trikes, sliding, sand play, playing on stationary equipment, and climbing on the structure. The children in spaces ##T3, #T7, and #T8 were observed participating in free choice activities. Free choice activities included playdough, drawing with markers, Legos, magnetic shapes, sand play, trucks, dramatic play, science, playing games, exercise bike, reading books, and dramatic play. I also observed children complete handwashing routines and prepared for lunch. Lunch consisted of cheese sticks, stir fry vegetables, pears, and milk. After lunch, children prepared for nap/rest time. Children were resting on appropriately spaced covered mats with blankets, lights were off, and soft music played in the background making the space conducive to rest. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. NC Pre-K Site Monitoring - The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed in children’s files and on the Teaching Strategies GOLD website. The NC Pre-K Site Monitoring Tool for 2024-2025 was available for review. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. As of March 24, 2025, the facility has an 18-month compliance history of 97%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. Daily schedules and activity plans were available for each group of children. The theme for this week was, “insects.” The attendance for each classroom was accurately completed. When I arrived, children from spaces #T5 and #T6 were observed outside playing in fenced area. The children were observed playing with balls, riding trikes, sliding, sand play, playing on stationary equipment, and climbing on the structure. The children in spaces ##T3, #T7, and #T8 were observed participating in free choice activities. Free choice activities included playdough, drawing with markers, Legos, magnetic shapes, sand play, trucks, dramatic play, science, playing games, exercise bike, reading books, and dramatic play. I also observed children complete handwashing routines and prepared for lunch. Lunch consisted of cheese sticks, stir fry vegetables, pears, and milk. After lunch, children prepared for nap/rest time. Children were resting on appropriately spaced covered mats with blankets, lights were off, and soft music played in the background making the space conducive to rest. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. NC Pre-K Site Monitoring - The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 8:00am to 2:45pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Ages and Stages Questionnaire (ASQs)screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed in children’s files and on the Teaching Strategies GOLD website. The NC Pre-K Site Monitoring Tool for 2024-2025 was available for review. Fall injuries are the result of two things: 1) The height a child falls from and 2) The material/surface the child falls upon. A checklist was used to note the requirements I monitored today, including staff and children’s files. This program currently operates with Five Star License, issued March 6, 2020, earning 7 points in program, 7 points in education and a quality point. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. As of March 24, 2025, the facility has an 18-month compliance history of 97%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Onslow County Schools operate this program. The last annual compliance visit was conducted at this facility on April 16, 2024. Sanitation Inspection – 11/08/2024 – 4 Demerits - Superior Fire Inspection – 02/27/2025 – PASS All approved indoor and outdoor spaces being used by the children were observed. NC PreK students were present in spaces #T3, # T5, #T6, #T7, and T8. There were seventy-two enrolled, and sixty-one children present, aged 3-5. Indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. Daily schedules and activity plans were available for each group of children. The theme for this week was, “insects.” The attendance for each classroom was accurately completed. When I arrived, children from spaces #T5 and #T6 were observed outside playing in fenced area. The children were observed playing with balls, riding trikes, sliding, sand play, playing on stationary equipment, and climbing on the structure. The children in spaces ##T3, #T7, and #T8 were observed participating in free choice activities. Free choice activities included playdough, drawing with markers, Legos, magnetic shapes, sand play, trucks, dramatic play, science, playing games, exercise bike, reading books, and dramatic play. I also observed children complete handwashing routines and prepared for lunch. Lunch consisted of cheese sticks, stir fry vegetables, pears, and milk. After lunch, children prepared for nap/rest time. Children were resting on appropriately spaced covered mats with blankets, lights were off, and soft music played in the background making the space conducive to rest. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. NC Pre-K Site Monitoring - The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 8:00am to 2:45pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Ages and Stages Questionnaire (ASQs)screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed in children’s files and on the Teaching Strategies GOLD website. The NC Pre-K Site Monitoring Tool for 2024-2025 was available for review. It was noted that all requirements were in compliance. There were two violations observed today. Violations must be corrected immediately. Continued compliance with all applicable child care requirements is always required. For the safety of the children, you must ensure weeds and debris are removed and mulch is tilled or added to the appropriate depth to avoid potential injuries to children. You do have black resilient mats also for protection. Nationally, nearly 70% of playground related injuries are from falls. Fall injuries are the result of two things: 1) The height a child falls from and 2) The material/surface the child falls upon. Always provide adequate “soft” resilient surfacing under and around stationary playground equipment. Refer to 10A NCAC 09 .0605(h)(i). As protruding bolts were observed on the black mulch barrier, I suggest that you check them on a regular basis to determine if they need to be pushed back down in the ground. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. Onslow County Schools operate this program. The last annual compliance visit was conducted at this facility on April 16, 2024. As of December 2, 2024, your 18-month compliance history is 98%. Sanitation Inspection – 11/08/2024 – 4 Demerits - Superior Fire Inspection – 10/23/2024 – PASS All approved indoor spaces being used by the children were observed. Due to active precipitation, the fenced outdoor play area was observed but not monitored. I observed new playground equipment had been installed. Prior to this visit, I had no knowledge of new equipment on the fenced outdoor play area. I contacted Hannah Bradshaw, NC PreK Coordinator, regarding my concerns of the fall zones and the surfacing for three pieces of stationary equipment on the outdoor play area. I advised Ms. Bradshaw that the stationary equipment is not approved. Ms. Bradshaw immediately got the appropriate persons involved and made the three pieces of stationary equipment inaccessible to the children by roping them off until fall zones and depth of surfacing are corrected. Ms. Bradshaw stated she would keep me updated on the progress of this project. Information for the stationary equipment must be on site and available for review. Once corrected, I will need to make a visit for approval prior to children being allowed to use these three pieces of equipment. Daily schedules were available for each group of children. Activity plans were observed posted; the theme for the week was “Trees.” A variety of age-appropriate learning materials were observed in classrooms. Family child care homes, as of now, are only required to do it once, and the water testing must be completed by January 1, 2025. Rated License Reassessment Information - - Your facility’s three-year rated license reassessment was due to be completed in March 2023. There are no longer any cohorts. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you may not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your additional caregivers on new assessment tools if you are still interested in Environment Rating Scales assessments. The new tools are the “3” versions, and I will share more information about training opportunities as they become available. Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025. To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position, and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within 2 business days. If you have general questions, please continue to use DHHS.CBC.Unit@dhhs.nc.gov. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Pursuant to Executive Order 319, NCDHHS will accept report submissions no later than January 15, 2025, but submissions may be completed at any time prior to that date and are now open. Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county. Submit a report even if no children are attending your child care center or your child care center has closed. Restrictions on the permit include: a capacity of 118 during first shift with an age range of 3-5, meets enhanced space, meets enhanced ratios minus 1 child per group, and T3, T5, T6, T7, T8, T12, K8 @18 children each. Onslow County Schools operate this program. The last annual compliance visit was conducted at this facility on April 18, 2023. As of April 15, 2024, your 18-month compliance history is 98%. Sanitation Inspection – 11/07/2023 – 6 Demerits - Superior Fire Inspection – 02/27/2024 – PASS All approved indoor spaces being used by the children and outdoor spaces were monitored today. Daily schedules were available for each group of children. Today was picture day so daily schedules were adjusted. Activity plans were observed poste; the theme for the week was “Gardening.” A variety of age-appropriate learning materials was observed in classrooms. NC PreK students were present in spaces T3, T6, and T8. There are seventy-three children enrolled and thirty-eight children, ages 3-5, were present. Children from all spaces were observed playing outside in fenced play area with caregivers. Children enrolled in space #T3 returned inside, completed handwashing and bathroom routines, participated in free choice activities, group time, and prepared for lunch. Lunch consisted of chicken nuggets, roll, sweet potato casserole, applesauce, and milk. Two groups of NC PreK students from space #T5 and #T8, were on a field trip to the Children’s Museum and the park for lunch. There were twenty-six students on the field trip and accountability information, including the names of the children on the trip, was available for review. Accountability information also included attendance sheet and a bus seat chart of where each child and caregiver were seated. Staff/Child ratios were in compliance and all children were adequately supervised. Good interactions were observed between caregivers and children. There was one violation observed today.
