A checklist was used to note the requirements I monitored today, including staff and children’s files. Child Care Consultant, Heather Elliott, accompanied and assisted me with this visit. Your facility operates with a Four-Star License on August 1, 2024, earning 6 points in Program Standards, 4 points in Education, and a quality point - restrictions included daytime care only and meetings enhanced ratios and space. As of today, the facility’s eighteen-month compliance history was 96%. According to the Secretary of State’s website, the corporation’s status, New River YMCA Onslow, Inc., is current and active as of January 21, 2026. The last annual compliance visit was conducted on January 28, 2025. Sanitation Inspection – 11/24/2025 – Approved classification – 17 Demerits Fire Inspection – 01/21/2025 - Facility was approved for daytime care only Your fire inspection expires today. Child Care Rule 10A NCAC 09 .0304 (a) states that each operator shall schedule and obtain a fire inspection within 12 months of the center's previous fire inspection. The operator shall notify the local fire inspector when it is time for the center's annual fire inspection. I suggest noting the date of your most recent inspection on a calendar and setting a reminder for yourself to call and request your inspection at least one month in advance of the deadline. The original approved annual fire inspection report should be submitted to me within one week of the inspection visit on the form provided by the Division. Administrator, Seara Jarvis, stated she has contacted the fire inspector requesting and inspection. Ms. Jarvis accompanied Ms. Elliott as she monitored and conducted a walkthrough of the facility. There are five classrooms operating at your facility, including one Early Head Start (EHS) classroom. There were forty-seven children enrolled and twenty-eight children aged 0-5 years present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. In space 1b, infants were engaged in tummy time, exploring their environment by crawling and cruising the furniture, playing with age-appropriate materials, and one child was sleeping. Infant feeding charts are current and safe sleep checks were completed, recorded, and maintained as required. The caregivers were interacting with and meeting the developmental needs of each of the children. Proper handwashing techniques and diaper changing procedures were observed. In space 1a, and 3, children were engaged in free play activities. Children were playing in the dramatic play center, playing with puzzles, and reading a book with the teacher. Children in space 2b were participating in group time and then they transitioned to the outside playground. Space 2a houses Early Head Start (EHS); EHS was closed today. Space 4 is not being used for child care children; it is being used for storage. Staff/child ratios were in compliance, and all children were adequately supervised. For lunch, ham and cheese roll-up, green beans, applesauce, and milk were served. After lunch, children had nap/rest time. All children rested on individual cots with blankets, lights turned off, and soft music played making environment conducive for nap/rest time. Staff/child ratios were in compliance. All children were adequately supervised. Staff in all classrooms were observed interacting with children in a positive and nurturing manner Staff and Training Worksheets- There are seventeen staff members employed at this facility. All new staff and a percentage of veteran staff’s files were reviewed. Health and safety trainings were monitored, and requirements were met. The following violation was observed and documented today. All violations must be corrected immediately. The violation was corrected during the visit; therefore, a letter of compliance is not required. Continued compliance with applicable child care requirements is always required. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. In space 1B, four medication permission forms were missing child’s name and date. 10A NCAC 09 .0803(4)(6-9) Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always whether they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Technical Assistance regarding violation cited: Medication - All medications must be accompanied by written authorizations from parents with complete instructions for administration. Prescription medications must also be in the original container with the pharmacy label attached. Today I suggested that you review medication requirements with staff and periodically check behind them to ensure medication permission forms are accurately completed and all medications requirements are met. There were four medication permission forms for diaper cream in space 1b that did not include the child’s name and date. The caregiver corrected the medication permission forms during the visit; the medication was a diaper cream supplied by the facility. A parent may give a caregiver standing authorization for up to 12 months to apply over-the counter, topical ointments, topical teething ointment or gel, insect repellents, lotions, creams, fluoridated toothpaste, and powders, such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed. The authorization shall be in writing and shall contain:( the child's name, the names of the authorized ointments, repellents, lotions, creams, fluoridated toothpaste, and powders, the criteria for the administration of the ointments, repellents, lotions, creams, fluoridated toothpaste, and powders, the manner in which the ointments, repellents, lotions, creams, fluoridated toothpaste, and powders shall be applied, the signature of the parent, the date the authorization was signed by the parent, and the length of time the authorization is valid, if less than 12 months. Choosing a Pathway to the Stars: Your facility was issued a Four-Star License on August 1, 2024, and is not due a reassessment until August 1, 2027. Today, I completed a QRIS Conversation Template for your facility. You reported that your facility is interested in pursuing Pathway #2, Classroom and Instructional Quality Pathway, and you may apply for a rated license by July 2026. A copy of this template is attached to this visit summary. Ensure all information and education has been submitted to DCDEE WORKS for all staff members as you hire new staff or as existing staff complete additional coursework. This is of vital importance as the information in WORKS for your staff will determine the number of points that your facility earns in the education component of QRIS. Other Information Clean Classrooms for Carolina Kids: Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed on August 2, 2023. Water testing must be completed every three (3) years. • Lead-Based Paint-On site risk assessment completed, and report updated on November 5, 2025 – No lead paint hazards identified • Asbestos- On site inspection completed and inspected report updated on November 3, 2025 – No asbestos hazards identified Ensure you keep all paperwork that you receive regarding this matter, and it may be requested to verify compliance. The toolkit offers ready-to-share fact sheets, social media content, videos, posters and other materials to help communities, health departments, schools and employers share respiratory illness information. The materials explain how these viruses spread, symptoms to watch for and the benefits of getting updated vaccines. They also provide information on where to get vaccinated. NCDHHS encourages North Carolinians to get vaccinated for the strongest protection and follow everyday steps like handwashing and staying home when sick. For questions or concerns, talk to a health care provider. Resources and vaccine-finder information are available at MySpot.nc.gov. Reminders: The gate on the EHS playground area has started to collapsed. You stated that you are renovating the playground and fence will be taken down. Facility is installing turf on the playground areas; the turf was observed on the premises ready to be installed. Staff members should inspect their classrooms daily to ensure a safe environment. Make sure all outlets are covered before children come into the classroom each morning. You should have a working appliance thermometer in your refrigerator to ensure it stays at or below 45°F for food safety, as built-in digital displays can be inaccurate, and they help during power outages. Place it in the middle of the fridge, check it daily, and adjust settings as needed to prevent bacterial growth, which speeds up between 45°F and 140°F. The built-in kitchen refrigerator thermometer was working. However, the appliance thermometer placed in the kitchen refrigerator was not working properly. You stated that the battery may have died, but it started working properly during the visit. Ms. Jarvis stated she planned to investigate new thermometer options for the refrigerators and/or replace them every six months. Limited child care requirements pertinent to this visit type were monitored. This facility operates with a Four-Star License on August 1, 2024, earning 6 points in Program Standards, 4 points in Education, and a quality point - restrictions included daytime care only and meetings enhanced ratios and space. As of today, the facility’s eighteen-month compliance history was 99%. According to the Secretary of State’s website, the corporation’s status, New River YMCA Onslow, Inc., is current and active. The last annual compliance visit was conducted on January 28, 2025. Fire Inspection – 01/21/2025 - Facility was approved for daytime care only Sanitation Inspection – 05/16/2025 - Superior classification – 0 Demerits New Administrator, Seara Jarvis, was not present when I first arrived at the facility. She was at a meeting at One Place and arrived approximately an hour into the visit. Assistant Administrator, Kimberly Jennings, was present and accompanied me as I visited each classroom. There are six classrooms operating at your facility, including one Early Head Start (EHS) classrooms. There were forty-five children enrolled and thirty-three children aged 0-5 years present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. All children rested on individual cots with blankets, lights turned off, and soft music played making environment conducive for nap/rest time. Staff/child ratios were in compliance. All children were adequately supervised. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. There have been five new staff members hired since the last annual compliance visit. New staff files were monitored today and found to be in compliance. There were four violations observed today. All violations must be corrected immediately. Continued compliance with applicable child care requirements is required. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. One staff member has not completed a health questionnaire form since July 22, 2024. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff member has not updated their emergency information since 07/22/2024. .0701(a) 1955 The child care center was not free of lead poisoning hazards as defined in G.S. 1130A-131.7(7). Infant feeding charts are current and up to date and safe sleep charts are being maintained. All children rested on individual cots with blankets, lights turned off, and soft music played making environment conducive for nap/rest time. Staff/child ratios were in compliance. All children were adequately supervised. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you may not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your additional caregivers on new assessment tools if you are still interested in Environment Rating Scales assessments. The new tools are the “3” versions, and I will share more information about training opportunities as they become available. Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. A checklist was used to note the requirements I monitored today. This facility operates with a Four-Star License on August 1, 2024, earning 6 points in Program Standards, 4 points in Education, and a quality point - restrictions included daytime care only and meetings enhanced ratios and space. As of January 24, 2025, the facility’s eighteen-month compliance history was 100%. According to the Secretary of State’s website, the corporation’s status, New River YMCA Onslow, Inc., is current and active. This is the first annual compliance visit since the program became licensed. Fire Inspection – 01/21/2025 - Facility was approved for daytime care only Sanitation Inspection – 12/20/2024 - Superior classification – 8 Demerits Administrator, Ashley Degen, was present and accompanied me as I visited each classroom. There are six classrooms operating at your facility, including two Early Head Start(EHS) classrooms. There were fifty six children enrolled and forty seven children aged 0-5 years present today. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. All other toddlers and preschool children were cleaning up from lunch and preparing for nap/rest time. Lunch consisted of chicken strips, green beans, applesauce, and rolls finish. All children rested on individual cots with blankets, lights turned off, and soft music played making environment conducive for nap/rest time. Staff/child ratios were in compliance. All children were adequately supervised. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. There was one violation observed today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is required. Standing water on a playground is a safety concern as it can cause the play surface to deteriorate, create slippery conditions, and promote the growth of mold and mildew; it can also compromise the structural integrity of the playground equipment, particularly metal components, due to rusting; the best way to address this issue is to ensure proper drainage. Other Information: Health & Safety Training – Some staff members have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. Ensure this information is accurate and maintained on file. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. As we discussed today, health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. CLEAN CLASSROOMS FOR CAROLINA KIDS - You stated that all the registrations and webinars for Clean classrooms for Carolina Kids have not been completed but will get them completed. This is a reminder and update about the REQUIRED testing for lead in water, lead-based paint and asbestos. To meet requirements, you must enroll your program through Clean Classrooms for Carolina Kids in ALL THREE TESTING PROGRAMS. Your facility may be exempt from testing for lead-based paint and/or asbestos based on the year built, however it is still required that you enroll in the programs and provide documentation to request exemption. See key points below: -Centers must have water tested for lead every three years. Check the date of your previous testing and ensure you request a new one if your three years are almost up. -Family Child Care Homes (FCCH) must have water tested by January 1, 2025. -Enrollment in the lead-based paint and asbestos testing programs is required for all facilities by May 31, 2025. This applies to private centers, FCCH and child care programs in the public schools. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you may not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. Today I encouraged you to review information about the new standards as it becomes available and to plan on training your additional caregivers on new assessment tools if you are still interested in Environment Rating Scales assessments. The new tools are the “3” versions, and I will share more information about training opportunities as they become available. Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position, and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within 2 business days.
