Your program currently operates with a one-star license, issued September 13, 2018. Restrictions on your permit include: a capacity of 91 for first shift care, age ranging in 2-12 years old, no cooking allowed, and children under 2 ½ years old in rooms with direct exits only. The license was posted, and the restrictions were in compliance. Your last annual compliance visit was conducted on August 6, 2025. The center’s compliance history was reviewed with the operator. The program’s compliance history was 94% as of today. The NC Secretary of State website was reviewed today and Milestones Preschool, LLC was listed as current/active. The last Sanitation Inspection was completed on November 24, 2025, with a Superior rating with 4 demerits. Your last fire inspection was on August 13, 2025, and your facility was approved for daytime care. Jamie McRacken and Elizabeth Elks, administrators, were present and available for consultation. A walk-through of the facility was completed today and all approved indoor and outdoor learning environments were monitored. Supervision and staff/child ratios were found to be in compliance. There were 92 children enrolled and 72 children present, age ranging from 3-5 years old. In spaces 1, 3, 4, and 5 children were participating in group activities. The activities included but not limited to singing songs, talking about who was present at school today, conversations about the weather, and discussing and reading the book If You Give a Mouse a Cookie. In space 2, the children were playing outside and transitioned to their classroom for morning snack. Each indoor classroom environment had age-appropriate materials, and the space was divided into centers including but not limited to blocks, art, manipulatives, books, and science. Teachers were engaging with children at eye level and asking engaging questions. All activity plans were current and aligned with NC Early Childhood Foundations. Daily schedules were available for each group of children. Files for the two new staff members have been reviewed and found in compliance. During today’s visit child care requirements were monitored for compliance including supervision, staff/child ratios, CPR and First Aid, Criminal Background Checks, storage of hazardous materials, storage of medications, approved space, and other applicable requirements. The following violations were observed and documented today. All violations must be corrected immediately. Two permission forms for albuterol, one is Space 1 and one in Space 3. Child Care rule .0803 (6) states a parent may give a caregiver standing authorization for up to six months to administer prescription or over-the-counter medication to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. The authorization shall be in writing and shall contain: (a) the child's name; (b) the subject medical conditions or allergic reactions; (c) the names of the authorized over-the-counter medications; (d) the criteria for the administration of the medication; (e) the amount and frequency of the dosages; (f) the manner in which the medication shall be administered; (g) the signature of the parent; (h) the date the authorization was signed by the parent; and (i) the length of time the authorization is valid, if less than six months. Today I suggest going through all the rooms on a regular basis to ensure that all forms, including medical action plans and permission forms, are up to date. Medical Action Plans should be updated yearly and permission forms for prescribed medication are for up to 6 months. Expired medications must be returned to the parents within 72 hours and new medication received must be accompanied by a new permission to administer form as this form will contain the medication expiration date. Playground Surfacing Today I suggested refreshing, adding and raking the mulch to playground 1 with all the recent inclement weather and the compacting due to the children to ensure that there is sufficient surfacing around the climbing structure. Today we discussed the new QRIS modernization. You stated that you were going to stay at a one-star center and at this time were not interested in the different pathways. CLEAN CLASSROOMS FOR CAROLINA KIDS - Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed on March 8, 2024. Water testing must be completed every three (3) years. The Lead Paint Testing and Asbestos Testing was completed February 24,2024, indicating the center is exempt. ABCMS-Today we discussed that information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This program currently operates with a 1 Star License, issued September 13, 2018. The restrictions listed are as follows: 1st shift only, no cooking, and children under 2 1/2 years old in rooms with direct exits only. Age range is 2 through 12 years with a capacity of 91. The NC Secretary of State website was reviewed on April 12, 2024, and Milestones Preschool, LLC was listed as current- active. The last annual compliance visit was conducted July 25, 2023. The center completed the last Fire Inspection on August 1, 2023, and was approved for daytime care only. The last Sanitation inspection was completed on September 12, 2023, with a Superior Classification. As of April 12, 2024, the compliance history is 96%. Today’s visit was conducted by Child Care Consultant April Bosse on behalf of your Child Care Consultant Kim Sherry. Jaime McRacken, administrator, was present during today’s visit. All indoor and outdoor spaces were monitored today. The center was clean and organized. There were five classrooms operating including the half day program. There are ninety-three children enrolled and sixty-five children ages 3-5, were present today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. A variety of age-appropriate learning materials was observed in each classroom. Children in Space 1 were engaged in activity areas with playdoh, bristle blocks, and animals. Children in Space 2 were engaged in activity areas with linking blocks, tracing letters, and using glue for an art activity. Children in Space 3 were engaged in a large group activity coloring dinosaurs and gluing on the letter of their names. Children Space 4 were engaged in painting watermelon art and making words with magnet letters. Children in Space 5 were engaged in pretending to cook, dot marker art, a bug sensory table, and tracing letters with a caregiver. All children were adequately supervised. Caregivers and children interacted in a positive and appropriate manner. Snacks and lunches for all children are brought from home. Four new staff members have been hired since the last annual compliance visit. Files for all new employees were reviewed today. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An Epi-Pen in Space One was not in the original labeled container. .0803(2)(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Prescription medications must also be in the original container with the pharmacy label attached. Today I suggested that you review medication requirements with staff and periodically check behind them to ensure all requirements are met. Permission to administer medication is only valid for six months. Today you stated that you will ensure all permission forms are updated and that the requirement for six months’ permission to be renewed will be noted on the spreadsheet that you use to ensure those dates are met. You also stated that you will discuss with staff and parents to ensure that all medications that come into the facility are labeled with a pharmacy label. Technical Assistance: Today we discussed plastic bags for dirty clothes hanging in cubbies. We discussed that this is permitted as those classrooms with plastic bags hanging in cubbies did not have children under the age of three enrolled in them. If a child that is two years old did enroll in those classrooms, caregivers will need to be aware that any plastic will need to be at least five feet off the floor in those classrooms. Reminders: Childcare licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times whether or not they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable.
