The program currently operates with a 4-Star License, issued September 12, 2019. The restrictions on your permit include a capacity of 8 children for first and second shift care, age ranging from 0-12 years old, maximum of 5 preschool children at a time, fireplace/woodstove not used during operating hours, and no more than 2 children under 1 year of age. Your last annual compliance visit was on May 20, 2025. The facility’s compliance history was reviewed with the operator. The program’s compliance history is 100% as of April 17, 2026. You, Rudella Gary, owner was present and available for consultation today. There are 4 children enrolled and 3 children present today, ages ranging from 1-4 years of age. All approved indoor and outdoor spaces were monitored today. You were engaging with the children during free play when I arrived. One child was finishing up morning snack, and one child was completing personal care routines. Morning snack consisted of waffles, blueberries, and milk. Toileting procedures and handwashing procedures were observed and found in compliance. One child arrived later, where you greeted the child and parent. The children transitioned into the childcare space, where the children were participating in free play. One child was playing with a puzzle and other manipulative toys, one child was reading a book with you, and one child was pretending to cook in the kitchen. For lunch, beef meatloaf, whole grain brown rice, green beans, mixed fruit and milk were served. Staff/child ratios were in compliance, and all children were adequately supervised today. Staff and all four (4) children’s files were monitored and found in compliance. Health and Safety trainings was monitored, and requirements were met. Emergency drill logs and playground inspections were monitored. The following violations were observed and documented during today’s visit. All violations must be corrected immediately. Violation Number Comment Rule 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. No quarterly lockdown or shelter-in-place drill has been recorded this year. .1719(a )(16) & .1721(e )(7) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. Today dish cleaner and other cleaning supplies were located in an unlocked cabinet accessible to children. .1719(a)(6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 11, 2025. I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Items labeled “Keep out of reach of children” with no other warnings must be stored at a minimum of five feet off the floor. Hazardous cleaning supplies, including those kept under pressure in an aerosol can, as well as medications must be kept in locked storage. The definition of locked storage requires that the lock be unlocked with some type of device such as a key or combination. Regular childproofing locks do not meet this requirement. Ensure that all cleaning supplies and other items labeled “Keep out of reach of children” are kept inaccessible while children are in care. Today I suggested checking the lock cabinet on the way to the door when the first child arrives in the morning to ensure that it is locked. Today hand sanitizing wipes were found stored not 5 feet off the floor in the child care space and your locked storage with cleaning materials was not locked. Shelter-in-place/ Lockdown drills Today, I observed no shelter-in-place drill has been performed yet this year. Shelter-in-place/ lockdown drills must be performed and recorded once every three months, and records shall be maintained as required. Records of shelter-in-place/ lockdown drills documenting the date and time of each drill, the length of time taken, and the signature of the person who conducted the drill shall be available for review. Today I suggested marking a date on your calendar so that you can ensure that a shelter-in-place/ lockdown drill is performed quarterly. It may also help to plan these drills for different times of the day so children and staff can be prepared to evacuate no matter what activity or routine is taking place. Today we discussed the QRIS and you stated that you were going to go with Pathway 1-2. Today we reviewed the requirements for Pathway 1-2, Program Assessment- Classroom and Instructional Quality. Today you stated that you are wanting to go Pathway 2- Classroom and Instructional Quality. We walked through the Individual and Facility Continuous Quality Improvement (CQI) forms (individual and center). You will complete the complete the center form for your facility and an Individual CQI annually. (This will also count as your Professional Development Plan) Maintain these in your staff files. We reviewed the Family and Community Engagement Plan. We reviewed the Foundational Practices. Ensure that they are included in the parent handbook Consider creating a notebook to collect documentation of the practices including pictures, flyers, newsletters, emails, etc. Pathway 2: Classroom and Instructional Quality We reviewed the list of approved curriculums and formative assessments as an approved curriculum and formative assessments will be required for pathway 2. You will need to provide verification that you have received training on the selected curriculum and formative assessment tool. We discussed options for five hours of additional training (on-going training or CEUs) or coaching/mentoring option. Reminders: Your health questionnaire will expire on May 20, 2026. You have received 8.5 on-going training hours. You must complete 1.5 credit hours by May 20, 2026. Please send confirmation of completion.
