    Name of Operation: Childcare Network #81 Facility ID: 67001220 Consultant: APRIL BOSSE Operation Type: Center Case Number: Visit Date: 3/25/2024 Number Present: 70 Completed Date: 3/25/2024 Age: From 0 To 10 Total Minutes: 110 Time In: 11:10 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance with applicable child care requirements for the facility’s the first temporary time period visit. The facility’s Temporary License was issued March 1, 2024, and is valid through September 1, 2024. Restrictions on the permit include a first shift capacity of 109 children ages 0 to 12 years old: meeting enhanced space and ratios minus one in each age group. Restrictions were in compliance today. The program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program uses High Reach Learning. According to the North Carolina Secretary of State website, your corporation Child Development Schools North Carolina, LLC is current and active. Your last fire drill was completed March 12, 2024, and your last outdoor inspection was completed March 13, 2024. Seven classrooms are operating with one hundred and fourteen children enrolled and seventy children present. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Activity plans were observed posted in each classroom. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Spaces 1 were observed sleeping in appropriate cribs and completing routine tasks with a caregiver. Toddlers in Space 2a and 2b were sitting at tables eating lunch while caregivers sat with the children and talked about what they were eating. Two-year-old children in Space 3 were cleaning up from a play doh activity and were reading books with caregivers as they waited for lunch to be served. Preschool children in Spaces 4 and 5 were engaged in activity areas with manipulative toys or engaged in group time pretending to be butterflies. School age children in Space 6 were cleaning up activity areas and sitting in a group reading poems out loud. Lunch served today consisted of pizza, green beans, pineapple, and milk. As this is a change of ownership, a new building, fire, and sanitation inspections must be completed by September 1, 2024. You will need to contact each inspector to request the inspection. Failure to obtain the required approved inspections prior to September 1, 2024, may result in administrative action against your license. Today you stated that you have requested or touched based with all three inspectors and are hoping to have it completed this month. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: the Temporary License, current menu, Safe Arrival and Departure Procedure, fire drill log, and the Summary of the NC Child Care Law. A sample of facility records were reviewed which included five (5) staff records, and eleven (11) children’s records. The Children’s Records worksheet was completed by me today and you have a copy of the completed form. The staff worksheets were completed for twenty-one (21) staff prior to our arrival. The indoor and outdoor learning environments were monitored for all pertinent safety and equipment related requirements. Enhanced space and ratio requirements are being met at this facility. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Children in NC Pre-K were not in attendance today due to Spring Break. The program follows the Onslow County school calendar. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies, is in place. Portfolios for each child are maintained electronically Online with Teaching Strategies Gold. The facility uses the developmental screening ASQ tool for prescreening and ongoing assessments. The following violation was observed today and must be corrected immediately: Violation Number Comment Rule 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The asthma action plan for one child in Space 6 was not updated on an annual basis. .0801(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before April 8, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance: The Asthma Action plan for one child in Space 6 expired as of March 17, 2024. These forms are valid for one year and was signed and dated for March 17, 2023. For any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services, a medical action plan shall be attached to the application. The medical action plan shall be completed by the child's parent or a health care professional and may include the following: (1) a list of the child's diagnosis or diagnoses including dietary, environmental, and activity considerations that are applicable; (2) contact information for the child's health care professional(s); (3) medications to be administered on a scheduled basis; and (4) medications to be administered on an emergency basis with symptoms, and instructions. RATED LICENSE: Today we discussed the two-component rated license process. As a change of ownership, you may choose to continue with the points awarded during the last rated license assessment. Under the previous ownership the facility earned 1 quality point for having a staff benefits package and infrastructure for parent involvement. The facility earned seven points in program for meeting minimum requirements, enhanced space, and ratios minus one in each age group and earning an average score of 5.45 across their three assessments. The facility earned five points in education. The facility earned a total of thirteen points which is a five-star license. You would like to continue with these points. This puts your facility in cohort 1. Your prep year began July 1, 2023, through June 30, 2024. Your reassessment year is July 1, 2024, through June 30, 2025. In your planning year, you will want to review resources available at www.ncrlap.org and contact One Place or Region 4 CCR&R for assistance with completion of the self-assessment component. Your planning year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. Also, be reminded that you may voluntarily request a reassessment at any time by contacting your consultant. NCRLAP outreach assessments are still being offered if you wish to “practice.” You may also wish to contact technical assistance specialists with One Place to help you with mock assessments. Related to education, ensure you are uploading your education. Education Standards: All staff need to ensure current education information has been submitted to WORKS and they have requested the appropriate evaluation for their position. It typically takes WORKS 4-6 weeks to complete an evaluation. Water Testing: Your water testing was completed through Clean Water for Carolina Kids under the previous owner on 10/19/2023. The testing for lead documentation has been completed. Space Calculations: Today new space calculations were not performed as spaces have not been altered and space calculations will remain the same. I will email you the new space calculations worksheet. Preservice form for Administrators: Please email me your preservice for administrators form as soon as it has been signed by the new owner. Reminders: Childcare licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times whether or not they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. DCDEE has launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to: • See applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster • See the real-time background check status of staff members • Run a printable report of the staff roster to assist with compliance visits To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within 2 business days.
