Kim Sherry, Licensing Supervisor, assisted during today’s visit. Your program currently operates with a 5-star license, issued September 2, 2024. Restrictions on your permit include a capacity of 120 children, ages 0-12 for daytime care only, meeting enhanced ratios minus one in each age group, and enhanced space. Your last annual compliance visit was conducted on February 12, 2025. The sanitation inspection was completed on December 17, 2025, with a “Superior” classification of 14 demerits. The last fire inspection was conducted on August 6, 2025, and your facility was approved for daytime care only. The center’s compliance history was reviewed with the operator. The program’s compliance history is 100% as of January 26, 2026. The NC Secretary of State website was reviewed today, and Child Development Schools, LLC, was listed as current-active. Jennifer Lopez, administrator, was present and accompanied me during the walk through and observations. Today 108 children were enrolled and 80 children were present, ranging in age from 0-5 years of age. Children throughout the facility were participating in group time, free play in activity areas, transitional activities, and personal care routines. Daily schedules were available for each group of children. Current activity plans were observed posted in each classroom. In space 1, infants were engaged in tummy time, exploring their environment by crawling and cruising around the furniture and playing with age-appropriate materials. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting with and meeting the developmental needs of each of the children. Staff/child ratios were in compliance, and all children were adequately supervised. The one-year-old children were playing with appropriate age materials. The staff were engaged with the children in a game of tossing the beanbag into a flower cardboard cutout. In the classroom with two-year-olds, the children were engaged in a teacher lead activity, where they were painting with paintbrushes using a variety of colors. The three-year-old children were playing in centers including but not limited to dramatic play, art, manipulatives, and blocks. Staff had a shiny art activity where the children would color with crayons then put oil on top to make the paper and drawing shine. Preschool aged children were participating in free play activities in the learning centers including but not limited to blocks, manipulatives, books, and art. Proper handwashing techniques and diaper changing procedures were observed. The children’s lunch time was observed, and their meal consisted of diced turkey, fresh broccoli, pears, whole grain pasta with cheese and milk. Twelve newly hired staff member’s files, along with a selection of veteran staff files, were reviewed today. Fourteen children’s files were reviewed today and found to be in compliance. NC Pre-K Requirements- The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Monitoring Tool was reviewed today. On all the playgrounds, the 4X4 post have exposed rough edges and have started to splinter. 10A NCAC 09 .0604(p) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff member, N. Rucker, did not have her 16 hours of orientation available for review. .1101(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. One staff member, L. Axtell, ITS-SIDS training expired August 2025. .1102(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Staff members L. Axtell and N. Rucker did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 10, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Soft materials and activities are needed in the cozy area. The art shelf needs extra materials. The shelf had some construction paper but many of the shelves were empty. The playground used by infant children needs additional materials available for the children to explore. Materials were available in a locked cabinet and/or in the classroom; however, they were not accessible to the children. Please verify in writing additional materials have been added. Technical Assistance: A violation was documented related to Child Care Rule .0604 (p) SAFETY REQUIREMENTS FOR CHILD CARE CENTER. On the playground for the infants, the fence is not aligned. The tension band has loosened resulting in the lower half of the fence beginning curl exposing sharp and rusty protruding edges accessible to children. The overhang over the infant classroom outside door and over the one-year-old classroom is starting to deteriorate, having nails protruding and a nest is visible with items hanging down from the ceiling. On the one-year-old children’s playground, there were broken plastic toys accessible to children. On the two-year-old children’s playground, there was a broken bench that needed to be removed from the playground. On the playground for the NC Pre-K and aftercare children, there was trash on the playground, the shelf that stores the toys and materials and the green plastic planter were broken. There was a hole between the fence and concrete that poses a tripping hazard. All 4 X4 posts are starting to splinter on all playgrounds used by children. Before children enter the outdoor area each day, you must take time to check the outdoor area for hazards to ensure all areas are safe for children. Child care centers licensed to care for infants 12 months and younger shall require child care administrators and all staff scheduled to work with infants, including volunteers counted in staff/child ratios, meet the following training requirements: Complete the ITS-SIDS training within 2 months of assuming responsibilities in the infant room, or within 90 days of assuming responsibility as a child care administrator. Complete the ITS-SIDS training again no more than 3 years from their previous ITS-SIDS training. L. Axtell’s ITS-SIDS has expired August 2025 and is in need of renewal. Staff members L. Axtell and N. Rucker have not taken Recognizing and Responding to Suspicions of Child Maltreatment and have been employed for more than 90 days. The area around the sinks in space 3, 4, and 5 were not in good repair. The caulking around the sinks had a collection of dust, dirt and grime. In classroom space 4, the front of the sink started to deteriorate and wood particles were assessable to the children. In classroom space 3, the corner of the sink has sharp glue that is in need of removal. In classroom 2, the cabinet above the sink is broken and is in need of repair. Clean Water for Carolina Kids websites states that your facility’s last lead water test was completed on October 10, 2023, and the facility’s asbestos report was completed on September 21,2024. Your facility was exempt from the lead-based paint inspection. Reminder these tests must be completed every three years. At the completion of the visit, this summary was printed, reviewed, and a copy was left with you.
