A checklist was used to note the requirements I monitored today, including staff and children’s files. Child Care Consultant, Andrea Johnson accompanied and assisted me with this visit. The facility operates with a Notice of Compliance. The last annual compliance visit was conducted June 5, 2024. Your 18-month compliance history score is 94%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Fire Inspection – 02/04/2025 - Approved for daytime care only – Satisfactory Sanitation Inspection – 12/05/2025 - 6 Demerits – Superior The center is owned by First Baptist Church of Jacksonville, NC. The corporation is current and active with the NC Secretary of State. The Administrator, Kathryn Wessell, was present and accompanied Ms. Johnson as she visited each classroom and outdoor play areas. There are ten classrooms operating with seventy-eight children enrolled and sixty-five children present, ages 0-5. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants in space 14 received care based on individual needs, including diapering, feeding and napping routines. Infant feeding schedules and safe sleep checks were monitored and found to be in compliance. Children in spaces 9, 11, 13, and 15-were observed in the outdoor spaces. Outdoor activities included running, climbing, sliding, swinging, and riding in strollers. Children in spaces 8, 10, and 12 were engaged in a variety of age-appropriate fee play activities which included books, coloring sheets with shapes and colors, and books-on-tapes readings. Adequate supervision and positive interactions were observed. There was one violation observed today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is required at all times. Violation Number Comment Rule 608 Children in space #13 did not wash their after outside play. 15A NCAC 18A .2803(c) Comment regarding violation cited Handwashing - Child care rules states children must wash their hands upon arrival at the center, after each visit to the toilet, before eating, and before and after water play, after outside play, and/or after handling animals or animal cages . According to Center for Disease Control and Prevention, Clean hands save lives. Handwashing is like a "do-it-yourself" vaccine—it involves five simple and effective steps (Wet, Lather, Scrub, Rinse, Dry) you can take to reduce the spread of diarrheal and respiratory illness so you can stay healthy. Regular handwashing, particularly before and after certain activities, is one of the best ways to remove germs, avoid getting sick, and prevent the spread of germs to others. It's quick, it's simple, and it can keep us all from getting sick. Handwashing is a win for everyone, except the germs. For your facility to remain in compliance with NC Child Care Requirements, violations must be corrected immediately. On or before June 17, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected as well as how compliance will be maintained in the future. The time frame is established to allow you time for submitting your compliance letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Building Information – Facility is in the process of renovating and hope to be done by August 31, 2025. After completion of your renovations, you will to get statements from building, fire, and sanitation to verify that the changes are still approved for child care. There were no changes to the actual classroom spaces, just the entrance area and office space for staff. You alsos advised that you will be adding a new playground – you will need to contact me prior to allowing the children to utilize the space and/or any new playground equipment. You provided and updated floor plan during today’s visit. Other Information Health & Safety Training – All staff members who have been employed for at least one year, have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. New staff are in the process of completing these training courses. Health and safety trainings are required to be completed during the first year of employment and renewed every 5 years. Remember, the Recognizing and Responding to Suspicions of Child Maltreatment training must be taken at www.preventchildabusenc.org, within the first 90 days of employment. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Ensure this information in completed, recorded on the required form, and on file for review. CLEAN CLASSROOMS FOR CAROLINA KIDS - The registration and enrollment for all three testing programs have been completed – lead in water, lead-based paint and asbestos. I verified this information through the website Clean Classrooms for Carolina Kids. Ensure you keep all paperwork that you receive regarding this matter, and it may be requested to verify compliance. Use this link to enroll your program or view test results/status: https://www.cleanwaterforuskids.org/en/carolina/ Enrollment in the lead-based paint and asbestos testing programs is required for all facilities by May 31, 2025. This applies to private centers, FCCH and child care programs in the public schools. DPI Pre-K programs do not need to enroll separately as public schools will be tested as a whole. Criminal Background Check Information – This is a requirement and if it is not completed, it will be documented as a violation during your next visit. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. Ms. Wessell has completed the training and is currently using the ABCMS Provider Portal to update information at this facility by verifying the roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Rated License Reassessment Information – This facility does not have a rated license; they operate with a Notice of Compliance. You may apply for a rated license at any time. There are no longer any cohorts. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you may not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. Today I encourage you to review information about the new standards as it becomes available and to plan on training your additional caregivers on new assessment tools if you are still interested in Environment Rating Scales assessments. The new tools are the “3” versions, and I will share more information about training opportunities as they become available. Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. Limited monitoring of child care requirements occurred during today’s visit. The facility operates with a Notice of Compliance. The last annual compliance visit was conducted June 6, 2023. As of today, your 18-month compliance history score is 94%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Fire Inspection – 03/21/2024 - Approved for daytime care only – Satisfactory Sanitation Inspection – 12/05/2024 – 6 Demerits – Superior The center is owned by First Baptist Church of Jacksonville, NC. The corporation is current and active with the NC Secretary of State. The Administrator, Kathryn Wessell, and Assistant Administrator, Nicole Wilbur were present and available for consultation. There were one hundred children enrolled and seventy -five children, aged 0-7, present today. Infants received care based on their individual needs. Toddlers, preschool children, and school age children were engaged in free play activities indoors. The children completed bathroom and handwashing routines to prepare for lunch. Lunch consisted of chicken nuggets, green beans, peaches, roll, and milk. Staff/child ratios were in compliance. All children were adequately supervised and all staff members were actively engaged with the children. The allegation of this report specific to the classroom for three-year-old children. The allegation was discussed with Ms. Wessell, Ms. Wilbur, staff members for classroom for three-year-old children, Antavia (Ellae) Francis and Genette Chappell. Additional staff member for classroom for three-year-old children, Alice Joseph, was not present today. You each were given the opportunity to state your perceptions of the allegation and to share any pertinent information Allegation There is a concern that a staff member did not interact with a child in a nurturing and caring manner. During today’s visit, it appeared that all staff members interacted with children in a nurturing and caring manner. However, Ms. Wessell, Ms. Wilbur, Ms. Francis, and Ms. Chappell did recall an incident that occurred on January 9, 2025, regarding concerns that a parent reported a staff member grabbed and pulled a child by the left arm out of the classroom as she yelled at the child. Written statements regarding this incident were received and reviewed from Ms. Francis, Ms. Chappell, and Ms. Joseph. This facility does have cameras in the hallway, not the classroom, and the camera do not have audio. Ms. Wessell and I reviewed video footage of the hallway for January 9, 2025. The incident occurred in the morning between 810am-823am as children were being dropped off by their parents. I observed Ms. Francis leave the classroom with a coat in her left hand and holding the child by the left wrist/forearm area with her right hand as she walked abruptly with the child into a classroom next door. In the video, you can observe the child having to walk fast to keep up as the teacher walked swiftly down the hallway holding her by her wrist/forearm area. In the video, it also appeared Ms. Francis was frustrated and during my interview with Ms. Francis, she admitted to being frustrated because the child was having a hard time with drop off routine and parent entered the classroom and handed the child to her and child continued to kick and scream. She stated she did not yell at the child and just used a firm tone trying to calm and soothe the child but to no avail, so she allowed the child to go to the carpet, “the calm down spot” in the classroom to calm herself down and told her she could come talk to her when she was ready. Ms. Francis continued her normal classroom routines, and she stated within 4 to 5 minutes, the child calmed down and started to play with friends. Ms. Francis stated she interacts with and cares for children in a nurturing and caring manner daily; she “gives a healthy balance of love and redirection.” Based on observations. information received, video footage, and my investigation, this allegation is unsubstantiated. Each child must be attended to in a nurturing and appropriate manner, and in keeping with the child’s developmental needs. Nurturing care is essential for child development and lays the foundation for life-long health and well-being and builds human capital in the child today, the adolescent and adult tomorrow, and in the next generation in the future. Child care rules not only prohibit the use of inappropriate discipline but require all children be attended to in a nurturing and appropriate manner. This can only be achieved when staff members recognize and accept age-appropriate behaviors and respond in ways consistent with what is developmentally appropriate for each child. Understanding the stages of children’s development is critical; for example, those that cry may not have the verbal ability to express their needs and/or wants; or, they may not have the skills yet to do so. Recognizing this will minimize staff members’ tendencies to become frustrated and use harsh tones or verbal directives with young children who are simply engaging in age-appropriate behaviors. Once you understand that certain challenging behaviors are “typical,” it should allow you to respond in a nurturing and appropriate manner. You must keep your frustration in check by reminding yourself that children are not born knowing how to control and process their emotions; this is learned through caregiver interactions, guidance, and experience over time. Help toddlers navigate their emotions. Recognize and name feelings, then offer guidance on how to manage them. Suggest jumping up and down or visiting the cozy area. Empathize with their feelings and provide acceptable choices to help them feel in control. Comfort them with warm, physical contact, or give them a safe space to work through their negative emotions. Ms. Francis was disciplined by the facility. Staff discipline form was reviewed today. She was suspended without pay for three days and required to take two classes from One Place - Interactions Matter on January 16, 2025 and Deep Breathing Connection to Calm on January 28, 2025. Since this incident, Ms. Wessell held a meeting with all staff on January 20, 2025. Ms. Wessell shared the handwritten agenda with me during today’s visit. Ms. Wessell discussed with staff the proper tone of voice used with children, good interactions with children, and words matter – “be careful what you say to children and how you say it,” and reacting versus responding – which she discussed in depth the importance of the teacher having self-regulation before intervening in a challenging situation with a child. There was one violation observed and cited today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is required at all times. Children should be spoken to in a calm, respectful voice at all times. No form of corporal punishment or rough handling is ever acceptable. No child may be yelled at, threatened, shamed, humiliated, bullied or subjected to sarcastic remarks or profanity. it is important to remember to engage in one-to-one interactions with children, get on the child’s level for face-to-face interactions, use a pleasant, calm voice and simple language, provide warm, responsive physical contact, help children understand classroom expectations, redirect children when they engage in challenging behavior, listen to children and encourage them to listen to others, and acknowledge children for their accomplishments and efforts. Positive interactions between children and the caregiver are fundamental to quality child care. Caregivers can help a child feel safe, secure, and supported by acknowledging and responding sensitively to children's cues/signals and supporting children's secure attachment through consistent and warm nurturing relationships. A return visit will be conducted to verify compliance with the violation documented today. Violations of this nature directly impact the safety of the children while in your care. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. Limited child care requirements pertinent to this visit type were monitored. Child Care Consultant, Andrea Johnson, accompanied and assisted me with this visit. The facility operates with a Notice of Compliance. The last annual compliance visit was conducted June 6, 2023. As of December 31, 2024, your 18-month compliance history score is 97%. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Fire Inspection – 03/21/2024 - Approved for daytime care only – Satisfactory Sanitation Inspection – 12/05/2024 – 6 Demerits – Superior The center is owned by First Baptist Church of Jacksonville, NC. The corporation is current and active with the NC Secretary of State. The Administrator, Kathryn Wessell, was present and accompanied Ms. Johnson as she visited each classroom and outdoor play areas. Infants assigned to indoor spaces #110 and #112 were receiving routine care based on their individual needs. Diaper changing and safe sleep practices were monitored, and requirements were met. Toddlers and young preschool children assigned to indoor spaces #114, #116, and #118 were engaged in free play activities which included reading books, playing with food in the dramatic area, painting handprints, stacking toys, and playing musical instruments. All other children were observed in the outdoor spaces. Outdoor gross motor activities included climbing, sliding, swinging, running, and jumping. The children transitioned to the indoor space for lunch which consisted of corndogs, hashbrowns, peaches, and milk. All staff members were actively engaged with the children. Routine toileting, handwashing, and food service procedures were monitored and found to be in compliance. During today's visit child care requirements monitored for compliance included supervision, staff/child ratio, CPR and First Aid, Criminal Record Checks, storage of hazardous substances and medications, approved space, posting of the license, and other applicable requirements. There have been six new staff members hired since the last annual compliance visit was conducted. Files for new staff members were monitored during this visit. Health & Safety Training - Staff members who have been employed for at least one year and some new staff members have completed Health and Safety training as outlined in Child Care Rule 10A NCAC 09 .1102. Ensure this information is maintained on file. New staff must complete this training within one year of hire. Health and Safety training must be renewed every five years. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. There were three violations observed today. All violations must be corrected immediately. Continued compliance with all applicable child care requirements is required at all times. This is a reminder and update about the REQUIRED testing for lead in water, lead-based paint and asbestos. Your facility may be exempt from testing for lead-based paint and/or asbestos based on the year built, however it is still required that you enroll in the programs and provide documentation to request exemption. See key points below: -Centers must have water tested for lead every three years. Email the certificate along with your full name, position, and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within two business days. Technical Assistance regarding violation cited: Lockdown or shelter-in-place drills should be practiced at least every three months as part of your facility's EPR plan. These drills should be documented on the Emergency Drill Record. Your facility has not completed an emergency drill since 08/06/2024. Frequent practice will allow children and staff members to be familiar with the steps to take to keep everyone safe in an emergency. Today I suggested you plan the dates and times of your drills in advance and use calendar reminders to ensure they are completed at least every three months. The trained staff shall review the Emergency Preparedness and Response Plan (EPR) annually, or when information in the plan changes, to ensure all information is current. All staff shall review the center's EPR Plan during orientation and on an annual basis with the trained staff. Documentation of the review shall be maintained at the center in the individual's personnel file or in a file designated for EPR documents. The date of review should be noted on the Staff and Training Worksheet. Today I suggested you plan to conduct the review at a staff meeting held at the same time every year to help you remember. Recognizing and Responding to Suspicions of Child Maltreatment One staff member did not complete the Recognizing and Responding to Suspicions of Child Maltreatment Training within 90 days of hire. This is a required training that must be completed within the first 90 days of employment, no exceptions. Other Information CLEAN CLASSROOMS FOR CAROLINA KIDS - You stated the registration, and webinars have been completed for Clean classrooms for Carolina Kids. This is a reminder and update about the REQUIRED testing for lead in water, lead-based paint and asbestos. To meet requirements, you must enroll your program through Clean Classrooms for Carolina Kids in ALL THREE TESTING PROGRAMS. Your facility may be exempt from testing for lead-based paint and/or asbestos based on the year built, however it is still required that you enroll in the programs and provide documentation to request exemption. Check the date of your previous testing and ensure you request a new one if your three years are up. -Family Child Care Homes (FCCH) must have water tested by January 1, 2025. -Enrollment in the lead-based paint and asbestos testing programs is required for all facilities by May 31, 2025. Reminder - CRIMINAL BACKGROUND CHECK REQUIREMENTS A valid qualification letter was on file for all employees. However, there are several employees whose qualifying letter will expire in 2025. Criminal Background Check qualification letters can be renewed six months prior to the expiration date of current qualification letter. Criminal Background Check Information - DCDEE launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. To gain access to the Provider Portal complete the ABCMS Child Care Provider Portal Training on DCDEE's Moodle. The training consists of watching a short video then taking a test about the video. A certificate will be issued upon successful completion of the test. Email the certificate along with your full name, position, and Facility ID to DCDEE_ABCMS_Provider@dhhs.nc.gov to get access to the portal. Access will then be granted within two business days. If you have general questions, please continue to use DHHS.CBC.Unit@dhhs.nc.gov. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Pursuant to Executive Order 319, NCDHHS will accept report submissions no later than January 15, 2025, but submissions may be completed at any time prior to that date and are now open. Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county. Submit a report even if no children are attending your child care center or your child care center has closed. A checklist was used to note the requirements I monitored today. The facility operates with a Notice of Compliance. The last annual compliance visit was conducted June 6, 2023. Your 18- month compliance history score is 96%. The center is owned by First Baptist Church of Jacksonville, NC. The corporation is current and active with the NC Secretary of State. Today’s visit was conducted by Child Care Consultant April Bosse on behalf of your Child Care Consultant Tammy Ross. Fire Inspection – 04/17/2023 – Approved for daytime care only Satisfactory Sanitation Inspection – 05/23/2023 – 2 Demerits – Superior Eleven classrooms are operating at your facility. Eighty children ages 0-5 years were present today. All children were appropriately supervised. All approved indoor and outdoor spaces were monitored today. Daily schedules were available for each group of children. Infants received care based on their individual needs. Infant feeding charts are current and up to date and safe sleep charts are being maintained. During today’s visit, infants in Space 110 were observed sleeping in appropriate cribs and being held by caregivers for bottle feedings. Toddlers in Space 114 and 116 were engaged in activity areas with baby dolls and stacking rings or outside on the playground riding on toys, sliding and pushing toys. Two-year-old children in Spaces 103, 111, and 118 were outside on the playground engaged with slides, bikes, and wagons and engaged in art class making owls out of torn paper and glue. Preschool children in Spaces 104 and 106 were observed engaged in a variety of activities including blocks, a matching game, making ornaments and holiday cards and engaged in group time singing songs. Staff in all classrooms were observed interacting with children in a positive and nurturing manner. Lunch consisted of chili with veggies, cornbread, applesauce, and milk. New staff files were monitored today and found to be in compliance. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. 2 diaper creams in Space 111 were not returned to the parent after the authorization had expired. .0803(12) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Regarding medication authorizations, I encouraged you today to make lists of medications brought in by parents which include children’s names, types of medications, expiration dates of medications and expiration dates of authorization forms. This will allow staff members to easily see when medications or authorization forms are about to expire so they can return them to parents or get new authorization forms within 72 hours as required. Technical Assistance: Today technical assistance was given regarding the balance of feeding infants and attending to those infants that are engaged in free play or exploring the environment. Quality child care consists not only of adequate supervision but of interaction, stimulation and engagement of children by their caregivers. Research shows that infants’ brain development is dependent on activities and interactions. Freedom of movement and tummy time activities are also critical to their health and physical development. Confining children in equipment such as car seats, bouncers or swings for extended periods of time is not developmentally appropriate and does not allow for such valuable learning activities to occur. If they fall asleep in car seats or bouncers or swings, they must immediately be placed in cribs or on cots/mats as safe sleep rules require. Reminders: Child Care Rules were revised July 1, 2023, and a new rule book was posted to the DCDEE website. As discussed, you should discard any old versions of the rules and ensure you are referring to the most current rule book when reviewing or researching requirements. Sanitation rules were also readopted July 1, 2023. Child care licensing requirements are established to ensure a safe and healthy child care environment. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. There have been changes to the 2023 licensing fees this year.
