The facility’s Four Star License was issued May 12, 2025. Restrictions on the permit include daytime and overnight with a maximum capacity of 8 children ages 0 to 12 years old; No more than 5 preschool children present at any time; fireplace not used during operating hours. Restrictions were in compliance today. Heather Elliott, Child Care Consultant assisted with today’s visit. All main child care spaces and your fenced backyard were monitored today. The following rooms are identified as rooms used for child care: the living room/ child care room, and bathroom located in the hallway. Children enter through the front door. The child care space is divided into activity areas that include but are not limited to art, dramatic play, blocks, fine motor, books, and music/movement. The fire extinguisher was checked and charged. A first aid kit was available. The provider is not currently approved for transportation. Children are not currently enrolled during second and third shift. Three children, ranging in age from two to four years old are enrolled and were present today. Upon arrival children were engaged in free play, accessing a variety of age-appropriate materials. Adequate supervision and positive interactions were observed. Breakfast consisted of oatmeal, mandarin oranges, and milk. One violation was observed and must be corrected immediately. Violation Number Comment Rule 1712 Operator did not develop and adopt a written plan of care for completing routine tasks to ensure routine tasks did not interfere with the care of children during hours of operation. Written plan of care forms were not completed and on-file for the three children enrolled. .1712(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 31, 2025. I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. TECHNICAL ASSISTANCE RELATED TO VIOLATION CITED: Child Care Rule 10A NCAC 09 .1712 WRITTEN PLAN OF CARE states that each family child care home operator (operator or operators) who intends to complete routine tasks while children are in care shall develop and adopt a written plan of care for completing routine tasks. For purposes of this Rule, routine tasks include running errands, meeting personal and family demands, attending classes, and attending medical appointments. Operators who complete routine tasks with enrolled children shall limit these tasks to no more than two hours per week. If you do not plan to complete any routine tasks with the children, you will need to mark the box stating that you do not perform any routine tasks. We also discussed today that if you do plan to take the children out, for example to the library or the park, you will need to have an off-premise permission slip form. Both forms were emailed to you today. REMINDERS: The ABCMS training must be completed through Moodle. Following, you will be provided with a verification form and specific steps to follow in order for you to connect your information to the criminal background portal. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. Be reminded that you have five (5) business days from the date of hire/termination to add/remove staff from the portal. A fire drill and playground inspection are due in the month of October. A professional development plan is due annually. The health questionnaire form should be updated annually. Health and safety trainings should be completed within one year of employment. Today, I printed a copy of the log, as well as the log for on-going training hours for you. I reminded you that the health and safety training topics must be completed by the end of November 2025. The secretary of the state website is showing your LLC as current but not active. You provided me with a postcard you received recently from them, and told me you contacted them recently. They told you the payment was not due until November 2025.
