Currently this program operates with a Five Star license issued on February 25, 2019 earning six (6) points in staff education, seven (7) points in program standards and one (1) quality point. The last sanitation inspection was completed on May 12, 2025, earning a “superior” rating. The last satisfactory Fire inspection was completed on October 24, 2024. The last annual compliance was completed on March 6, 2025. Lead Teacher, A. Smith was available and assisted with today’s visit. This program consists of three (3) NC Pre-K classrooms. The fourth classroom is currently not being used. Thirty-one (31) children were in attendance ranging in age four (4) to five (5) years. Children were observed in free play in centers, teacher directed activities, and completing transitions and routines such as handwashing and toileting. All staff interacted with the children in a positive and nurturing manner. This center is owned and operated by Northampton County Public Schools. I verified the owner and program contact information is correct as documented with DCDEE. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. Fire Drills/Emergency Drills, Fire inspection, Sanitation report, First Aid/CPR Poster, Playground Outdoor Inspections, EMC Plan and License were reviewed and current. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. The following violations of child care requirements were documented today. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First Aid training for R. Barnes-Simmons expired on 3/10/2025 .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. CPR training for R. Barnes-Simmons expired on 3/10/2025 .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Shelter in place/lock down drills have not been completed and documented since 5/2025 .0604(u);.0302(d)(8) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Barnes-Simmons expired on 3/10/2025. PROGRAM RECORD KEEPING The facility’s shelter in place/lock down drill have not been completed and documented since 5/2025. REMINDERS • Recognizing and Responding to Suspicions of Child Maltreatment training for K. Deloatch will expire 10/31/2025. The training can be completed at https://preventchildabusenc.org/resource-hub/recognizing-responding-to-child-maltreatment/ • The playground inspections are being completed monthly, however the issues are not being addressed effectively. The issues that are reflected on the playground inspections should be addressed and corrected promptly, to maintain compliance. QRIS/STAR RATED LICENSE We discussed the different pathway options for the new QRIS system. You decided to complete Pathway 1-Program Assessment. We completed an in-depth review of the forms specific to Pathway 1 and you were given the opportunity to ask questions. Children were observed in free play in centers, teacher directed activities, and completing transitions and routines such as handwashing and toileting. All staff interacted with the children in a positive and nurturing manner. This center is owned and operated by Northampton County Public Schools. I verified the owner and program contact information is correct as documented with DCDEE. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records, and all program records were monitored. All children and program records were found to be in compliance. Two (2) violations were documented for staff record keeping/ training. NC Rated license scale results were also were also reviewed during the visit. The outdoor play area was monitored and violations were documented. All off-site forms were reviewed and found to be in compliance. The following violations of child care requirements were documented today. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. STAR RATED LICENSE (SRL)/ ENVIRONMENT RATING SCALES (ERS) Your facility had the ERS assessment completed on January 20, 2025. A copy of the assessment was given to you. The ERS results were reviewed with Lead teacher, A. Smith during the visit. An opportunity to ask questions and discuss the results was provided. The following information pertains to your rated license. PROGRAM STANDARDS Your facility meets minimum requirements, enhanced space and reduced ratios. Also, your facility scored an overall score of 5.3 on the ERS assessment. Policies and procedures have been previously submitted and approved. Based on this information, your facility meets seven (7) points in program standards. EDUCATION One administrator, three lead teachers and three teacher assistants are required for this program. Education cannot be determined at this time. The administrator’s qualifications were not in the DCDEE WORKS system. Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Currently this program operates with a Five Star license issued on February 25, 2019 earning six (6) points in staff education, seven (7) points in program standards and one (1) quality point. The last sanitation inspection was completed on March 27, 2024, earning a “superior” rating. The last satisfactory Fire inspection was completed on October 25, 2023. The last annual compliance was completed on May 31, 2023. You, T. Robinson, Administrator, were available and Lead Teacher, Q. Boyd assisted with today’s visit. This program consists of four (4) NC Pre-K classrooms. Children were observed in free play in centers, teacher directed activities, outdoor play, and completing transitions and routines such as handwashing and toileting. All staff interacted with the children in a positive and nurturing manner. All off-site forms were reviewed and found to be in compliance. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Ten percent of staff records and ten percent of children’s records were monitored. All staff records monitored were found to be in compliance. One (1) violations were documented for children’s record keeping. One (1) violation was written for safe outdoor environment due to four (4) active ant mounds in the outdoor play area. Three (3) violations were written regarding medication. The following violations of child care requirements were documented today. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. MEDICATION One (1) child in space A102 had a rescue medication that did not have a prescription, medical authorization, nor an action plan signed by the physician on file for review. All medications that are at the facility should be stored in original container with original pharmacy label including, the child’s name, name of medication, amount, and frequency of dosage. The medication should also have written permission from the parent, and documentation from the physician, in accordance with the child care rule. NC PRE K SERVICES Two (2) children’s records, J. Gregory and S. Deloatch did not have documentation of a dental screening on record for review. The dental screening shall be included on the child’s health assessment and/ or separate documentation for the dental screening should be obtained within 30 days of the child’s enrollment in the program. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Angela Ramsey, Pre-K Coordinator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Tameka Robinson and six additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On March 15, 2024, a bus driver dropped off a five-year-old child at a location alone and unsupervised. .1801(a)(1-5) 807 A safe indoor and outdoor environment was not provided for the children. On March 15, 2024, the bus driver created an unsafe environment when a five -year-old child was dropped off alone and unsupervised, crossed a road, and was picked up by two adult males that were unknown to the child. 10A NCAC 09 .0601(a) 1106 Each adult and child was not restrained by appropriate individual belt/device. On March 15, 2024, a bus driver instructed a child to move from to another seat, across the aisle, while the bus was in motion. GS 110-91(13) 1127 For routine transport of children to and from the center, staff did not have a list of the children being transported. On March 15, 2024, the bus monitor did not use a list to document when children boarded the bus. 10A NCAC 09 .1003(l) 1810 There was a substantiation of child maltreatment. On October 22, 2024, a substantiation of child maltreatment was determined related to supervision needs. GS 110-105.6(a) 1910 The driver or other staff member in the vehicle did not ensure children were transferred to an individual who is indicated on the application. Currently this program operates with a Five Star license issued on February 25, 2019 earning six (6) points in staff education, seven (7) points in program standards and one (1) quality point. The last sanitation inspection was completed on November 9, 2023, earning a “Superior” rating. The last satisfactory Fire inspection was completed on October 25, 2023. The last annual compliance was completed May 31, 2023. You, T. Robinson, Administrator, was available and Lead Teacher, Q. Boyd assisted with today’s visit. This program consists of four (4) NC Pre-K classrooms. Thirty-eight (38) children were observed in attendance. Children were observed in free play in centers, teacher directed activities, and completing transitions and routines such as handwashing and toileting. All staff interacted with the children in a positive and nurturing manner. All off-site forms were reviewed and found to be in compliance. New staff records were reviewed and a violation was cited. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. Any violations observed were reviewed and a computer-generated visit summary was left with you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. New staff, T. Jacobs, did not completed Recognizing and Responding to Suspicions of Child Maltreatment training within the required timeframe. .1102(g) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all child care facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply to your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 13, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is accurate documentation. If any information provided in the letter is not accurate, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 95%. TECHNICAL ASSISTANCE/ CONSULTATION STAFF RECORD KEEPING New staff member, T. Jacobs, did not have Recognizing and Responding to Suspicions of Child Maltreatment training on file for review. We discussed the training should be completed within 90 days of hire date. Ms. Jacobs’ date of hire is September 8, 2023. Information was left with you on where to complete Recognizing and Responding to Suspicions of Child Maltreatment training. The training can be completed via the Prevent Child Abuse NC website at www.preventchildcabusenc.org. RESOURCES STAR RATED LICENSE I informed you information regarding the preparation for the star rated license assessment process is located on the Division of Child Development (https://ncchildcare.ncdhhs.gov/) and North Carolina Rated License Project website. I recommend you and staff visit ncrlap.org to obtain detailed information about how the process will be implemented and review prep year activities and ideas offered. More information will be provided regarding your programs rated license assessment timeframe. 2023 ANNUAL LICENSE FEES There are a few changes to the Annual License Fee process this year: • All invoices will be emailed, and no paper copies will be sent through the U.S. Postal Service. • All invoices will be emailed by December 1, 2023 and will include the invoice number and total amount due. • All payments are due no later than December 31, 2023 Payments must be submitted online only through the Division’s website using a Mastercard or Visa debit or credit card, or by e-check. Providers will need their Facility ID number and their 2023 Invoice number to complete the payment process. Annual license fees do not apply to state or public-school operated child care facilities, or to religious sponsored facilities operated pursuant to a Notice of Compliance. Failure to pay the license fee is a violation of NCGS § 110-90(1a) and can result in a revocation of your child care license. In addition, unpaid license fees will be assessed a 10% late fee and accrued interest as established by NCGS § 147-86.23.
