Laney Clark, Group Leader, assisted me with the visit. You, Fanisha Watson, Lincoln County Regional Program Director, arrived at 4:05pm during the visit. The most recent annual compliance visit was conducted on December 4, 2025. This program currently operates with a Three star rated license issued on December 5, 2025, meeting daytime care only, school age only, and meets enhanced space. This facility has earned two points in program standards, five points in staff education, and one quality point for having a staff benefits package and an infrastructure of parent involvement. The North Carolina Secretary of State website was reviewed on May 6, 2026, and Young Men’s Christian Association of Catawba Valley, Inc. was listed as current/active. Limited monitoring was conducted. Upon arrival, children were engaged in homework and quiet activities. Children were observed during art activities and free play activities indoors. You stated you utilize the cafeteria and gymnasium for indoor child care space. The program’s compliance history was 96 percent prior to today’s visit. One new staff member has been employed since your most recent annual compliance visit. Applicable records for existing staff were reviewed. All staff were listed on the ABCMS Provider Portal roster for this facility. You stated that the previous program coordinator, Christie German, was no longer employed as of January 30, 2026. You stated children are not transported by the licensed program during the school year. This facility does not operate during the summer. The most recent monthly fire drill was conducted on April 27, 2026, at 4:35pm. The most recent quarterly lockdown/shelter-in-place drill was conducted on April 13, 2026, at 7:05am. This facilities EPR Plan for this facility was created on December 17, 2025. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter-in-place drill was completed on October 10, 2025, and the next lockdown drill was not completed until January 29, 2026. .0604(u);.0302(d)(8) The violation documented above must be corrected immediately. On or before May 20, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Per Child Care Rule 10A NCAC 09 .0607(d) The Emergency Preparedness and Response Plan shall include: (10) the location of a Ready to Go File. A Ready to Go File means a collection of information on children, staff and the facility, to utilize, if an evacuation occurs. **The file shall include, a copy of the Emergency Preparedness and Response Plan**, contact information for individuals to pick-up children, each child's Application for Child Care, medication authorizations and instructions, any action plans for children with special health care needs, a list of any known food allergies of children and staff, staff contact information, Incident Report forms, an area map, and emergency telephone numbers. -10A NCAC 09 .2510(b) At least one individual who is responsible for planning and ensuring the implementation of daily activities for a school-age program (program coordinator) shall: (1) Be at least 18 years old and have a high school diploma or its equivalent prior to employment; (2) Have completed two semester credit hours in child and youth development and two semester credit hours in school-age programming. Each individual who does not meet this requirement shall enroll in coursework within six months after becoming employed and shall complete this coursework within 18 months of enrollment. An individual who meets the staff requirements for administrator or lead teacher shall be considered as meeting the requirements for program coordinator, provided the individual completes Basic School-Age Care (BSAC) training as defined in 10A NCAC 09 .0102(4) of this Chapter, and (3) Be on site when children are in care for programs offering before and after school care only. You, Madalyn Wells, Program Coordinator, assisted with the visit until 4:15pm. After your departure, there was not a legal designee on site to review and sign the visit summary. I reviewed the violation that was observed prior to your departure. The Secretary of State website was checked prior to the visit, and the business, Young Men’s Christian Association of Catawba Valley, Inc., is still active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Currently this center operates with a temporary license issued on January 22, 2025, with the following restrictions: Daytime care only; School age children only, and Playground area does not meet Child Care Playground Safety Standard. The last monthly fire drill was conducted on March 21, 2025, at 3:30pm. The last sanitation inspection was completed on April 22, 2025, and you received a superior classification and zero (0) demerits. No new staff have been hired since the last visit was conducted on April 9, 2025. I contacted Erica Simmons, Administrator, via email to discuss the transition from a temporary license to either a one star or a two through five-star license. An email reply was not received prior to the conclusion of the visit. I discussed with you that you will not be eligible to receive subsidy funding after your temporary license expires since you will not achieve at least a three-star rated license. Your temporary license will expire after July 22, 2025. An extension of time is not given, and your rated license points will be processed with the current points you have earned. Tables must be cleaned with soapy water then wiped dry. Then spray with sanitizing solution and let sit for at least two minutes before wiping dry. Follow this procedure before and after eating. Make sure no children are near the table when spraying sanitizing solution. Sinks must be disinfected between changes of use. The staff should spray the sink with the disinfectant spray and allow the disinfectant to stay for at least two minutes before turning on the water and using the sink. Schedule - At least one fine motor/language activity must be scheduled daily. Staff need to read with children each day. This would be a free choice activity for children to choose to listen to the book.
