Upon arrival I was greeted by you, Mr. Christopher Tryon, Owner. I shared the reason for the visit you were able to assist me. Permit Information: Your program currently operates with a Five-Star Family Child Care Home License effective 7/6/22. The license was posted with the following restrictions in compliance: - 1st shift - Maximum of 5 preschool children at any time - Children in care on ground level only - Trampoline not in use during operating hours - Serves no more than two children under age one - Fireplace/woodstove not used during operating hours There is no longer a trampoline in the outdoor learning environment, please notify me if you would like this restriction to come off your license. Ownership: You Mr. Tryon are the Owner and you reside in the home. As a reminder, you must notify your consultant at least thirty (30) days prior to any changes occurring with ownership. The license cannot be bought, sold, subleased, transferred to another person or location or inherited I monitored the following items: supervision, capacity, staff qualifications and training, transportation, health, safety, program records, children’s records, indoor and outdoor learning environments. I monitored the following spaces used by the children: - childcare room - bathroom - outdoor learning environment Inspections: The last playground inspection was on 12/17/24. Indoor Learning Environment: I observed the indoor learning environment, materials for children to use were available in multiple learning areas/activity centers. I observed the children engaged in free play with transportation vehicles, meaningful and appropriate language were being used. During the visit, a specialist arrived to work with a child, I also observed drop off. Outdoor Learning Environment: The outdoor learning environment was monitored and in compliance. I reviewed that your wooden fence/barrier for the AC units needs to be flushed in order to prevent access to children. Program Records: The last fire drill was conducted on 12/17/24. The last emergency drill was conducted on 11/26/24. Staff Records: I reviewed your file and during the visit you completed the FCCH and CLIAR Verification form and the statement of responsibility. Two and a half (2.5) on-going training hours were carried over from last year’s annual compliance year. Two (2) on-going training hours were recorded, we reviewed that you need only half an hour (.5) on or before 1/31/25. If this isn’t complete a violation will be added to your visit summary. Children’s Records: All children’s files were reviewed, I observed the school-age children’s files with no updated signatures in the emergency care information section. One (1) was signed during the visit, please have the family review the form and have an updated signature. Nutrition: The facility was in compliance with child care meal pattern requirements. Medication: It was reported that medication is not administered. I observed topical ointments on a shelf next to the bathroom. Three (3) topical ointments were expired, I observed one (1) topical ointment with no permission form for administration, I reviewed the requirements with you and gave you a copy of the topical permission form. Transportation: It was reported that transportation is not being provided, a vehicle is on site for emergency purposes. Weapons: You reported that your facility was in compliance with child care requirements regarding firearms. Violation Number Comment Rule 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information. Three (3) topical ointments were expired, I observed one (1) topical ointment with no permission form for administration. 10A NCAC 09 .1720(c)(1&2) Corrective Action Plan: The violations not corrected during the visit must be corrected immediately. On or before 2/12/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. A template was shared with you to use for your compliance letter. The following items must be addressed: - the name of your facility - the facility ID number - the date the visit made - the date you are submitting the letter - citing each violation number - describing when AND how each violation was corrected AND how you will prevent the reoccurrence of each violation - close with your signature Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Rated License: Hold harmless has been extended until the new QRIS (Quality Rating and Improvement System) is implemented, this is also known as SB 425 (Senate Bill 425). Providers in Cohort #1 are not required to go any further with a rated license assessment unless requested, and providers in Cohort #2 do not need to start their preparation year unless a rated license assessment is requested. Lead in Water/Paint and Asbestos: You need to enroll their family child care home in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. I encourage you to reach out to Clean Classrooms for Carolina Kids with questions about this program and for additional support. Indoor Learning Environment: I observed the indoor learning environment and there were a variety of materials for the child children to use. I observed the child present building with magnetic blocks, the language interactions were developmentally appropriate. Outdoor Learning Environment: The outdoor learning environment was monitored. Program Records: I reviewed all the required records. Staff Records: I reviewed Mr. Tryon’s file and during the visit you completed the FCCH and CLIAR Verification form and the statement of responsibility. Three (3) on-going training hours will be carried over to next year’s annual compliance year. Two (2) on-going training hours are required on or before 1/31/25. Children’s Records: All children’s records were reviewed. Please ensure that the health care needs section is completed in its entirety. Medical reports need to be submitted to you within thirty (30) days of the child’s enrollment date. The medical report can be found on our website under provider documents and forms, a copy was left with you today. Nutrition: The facility was in compliance with child care meal pattern requirements. Medication: Medication is not administered. Transportation: Transportation is not being provided at this time; please ensure to have all the requirements reviewed on 6/30/23 completed for the vehicle approved for transportation. Weapons: You reported that your facility was in compliance with child care requirements regarding firearms. Two (2) violations were observed today.
