This center has a five star license and an 18 month compliance history score of 95%. The license and NC child care law summary were prominently posted. Upon arrival, I was greeted by B. Ingle, director. During the walkthrough, A. Fortier, director arrived at the facility. Ms. Ingle and Ms. Fortier accompanied me during portions of the walkthrough. There were 144 children present during the walkthrough of the facility. During the visit, I spent time observing space #3, the classroom for children one year of age. I observed the teachers sitting on the carpet with the children and helping them identify objects. I spent time observing space #8, the classroom for children 3 years of age. I observed the children participating in Stretch and Grow, an outside vendor company. The following items were monitored today: supervision, staff/child ratios, permit restrictions, license posted, approved space, storage of hazardous products, storage of medication, staff records and program records. There have been 6 new staff since my last visit. I monitored staff files. The staff and training worksheets were reviewed to confirm existing staff were still current with CPR, First Aid, ITS-SIDS training and criminal background qualifying letters. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-Down drills were documented quarterly as required. The playground safety inspections were reviewed. Reports were documented monthly as required. There were 2 violations cited during today’s visit. They were corrected during the visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A therapist was alone with a child is space 11b. The ABCMS system was monitored and D. McDaniels was not in the system. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A therapist was alone with a child in space 11b without a qualification letter. G.S. 110-90.2(b) & (d) & .2703(e) Technical Assistance: -I discuss the ABCMS Portal with the administrator. -Hold Harmless has been extended until the new QRIS is implemented (SB 425). -Use this time for staff to update their information in the WORKS database. The facility currently has a five-star license and an 18 month compliance history of 92%. The license and NC childcare law summary were prominently posted. The Secretary of State database was monitored and the KRKLN INC. is admin dissolved. I contacted the owner regarding this matter. He sent me documentation that the fee and paperwork has been completed. I sent a screenshot of the Secretary of State showing admin, dissolved to Ebony Duncan, Supervisor. Traci Meyer Carpentar, Child care consultant assisted with today’s visit. We were greeted by B. Ingle, director of operations. Ms. Ingle accompanied us during the walkthrough of the facility. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-down drills were documented quarterly as required. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 4/17/23. The center received 11 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 10/25/22. There were 129 children in attendance today. The children were observed participating in free play and outdoor play. The playground was monitored today. The facilities transportation vehicles were monitored today. All new staff files and 10 percent of existing staff files were monitored today. Ten percent of the enrolled children’s files were monitored at random today. There were 3 violations cited today. All were corrected during the visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. In Space #6 there was not a weekly schedule posted. GS 110-91(12);.0508(a) 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In Space #3 three bottles were not dated correctly. 15A NCAC 18A .2804(d) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Spaces #8, #10 and #11a outlets were not covered with safety plugs. 10A NCAC 09 .0604(c) Technical Assistance: I shared with the administrator to make sure all topical medication forms are completely filled out. I shared with the administrator to make sure the trash can in classrooms that have excessive amount of plastic hanging from them to tie them up.
