You, Melissa Annas, Program Coordinator, assisted with today’s visit. Your program currently operates with a five (5) star license effective October 4, 2023. The permit restrictions were in compliance including Daytime care only; School age children only; Playground area does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, rest time, outdoor gross motor play, snack, and departure during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on April 15, 2025. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The most recent fire drill was conducted on December 30, 2025. The most recent emergency drill was conducted on January 8, 2026. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. No new staff members have been hired since the last visit. The following violation was observed/documented during the visit: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One child enrolled on August 12, 2025, had an Auvi-Q Epinephrine injector USP 0.3mg Auto-injector on site that expired on January 8, 2026. One child enrolled on August 12, 2025, had an Epinephrine Injection USP, single dose 0.3mg auto-injector on site that expired on November 30, 2025. .0803(12) The violation(s) documented must be corrected immediately. On or before February 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to dinah.brown@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Medication: Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization or medication has expired must be returned to the child's parents. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, should be discarded. Standing permission to administer medications for chronic illnesses may be given for up to six (6) months. I suggested you assign a staff member to each room to check all medications and Permission to Administer Medication Forms at least once a month to prevent further non-compliance regarding medication. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Upon arrival, the purpose of today’s visit was reviewed with you, Melissa Annas, Program Coordinator. Your last annual compliance visit was conducted on May 21, 2024. Your program currently operates with a five (5) star license effective October 4, 2023. The permit restrictions were in compliance including Daytime care only; School age children only; Playground area does not meet child care playground safety standards; Meets enhanced ratios; Meets enhanced space. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during independent center play, transitions, snack time, toileting, handwashing, gross motor indoor play, and departure. Required Inspections: The last fire inspection was completed on March 10, 2025. The last sanitation inspection was completed on May 22, 2024. A “superior” classification was issued with six (6) demerits noted on the grade card. Emergency Drills: The last fire drill was completed on April 29, 2025, at 4:30pm and recorded. The last lockdown drill was completed on April 27, 2025, and recorded. Lead Water Testing: The most recent lead water testing results were completed on August 10, 2024. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios and meets enhanced space. The facility was approved for daytime care only. Your sanitation inspection was conducted on May 9, 2023, with 0 demerits and a Superior rating. Your last monthly fire drill was conducted on April 29, 2024. I conducted a review of nine (9) children’s files during today’s visit. No Staff and Training Worksheet was available today for a review of staff files during today's visit. I will return tomorrow May 22, 2024 to conduct a review of staff files. Your last emergency shelter-in -place/lockdown drill was conducted on April 29, 2024. Children were observed during transitions, snack, independent tabletop activities and departure. A checklist was used to note the requirements I monitored today. There was one violation observed during today’s visit. Your last annual compliance visit was conducted on September 12, 2023. You, Jody Andrews, Lead Assistant, assisted with today’s visit. Your program currently operates with a five (5) star license effective October 4, 2023. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios and meets enhanced space. A walk-through of the facility was completed today, Space #1 and 3 were used during the visit. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Your last annual fire inspection was conducted on April 22, 2024. The facility was approved for daytime care only. Your sanitation inspection was conducted on May 9, 2023, with 0 demerits and a Superior rating. Your last monthly fire drill was conducted on April 29, 2024. I conducted a review of nine (9) children’s files during today’s visit. No Staff and Training Worksheet was available today for a review of staff files during today's visit. I will return tomorrow May 22, 2024 to conduct a review of staff files. Your last emergency shelter-in -place/lockdown drill was conducted on April 29, 2024. Children were observed during transitions, snack, independent tabletop activities and departure. A checklist was used to note the requirements I monitored today. There was one violation observed during today’s visit. Upon arrival, the purpose of today’s visit was reviewed with you, Melissa Annas, Program Coordinator. Your last annual compliance visit was conducted on October 10, 2022. Dinah Brown, Child Care Consultant, accompanied me on the visit. Your program currently operates with a five (5) star license effective December 20, 2019. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios and meets enhanced space. You have earned seven (7) points in education, six (6) points in program standards for a total of thirteen (13) points. A walk-through of the facility was completed today, Space #1 and 3 were used during the visit. You were unable to conduct the walk-through with me. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during snack, handwashing, indoor free play activities and departure. Compliance History: The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was ninety-six (96%) percent as of September 11, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Sanitation Inspection: The most recent sanitation inspection for your facility was conducted on May 9, 2023. A superior sanitation classification was issued with zero (0) demerits noted on the grade card. Fire Inspection: The most recent fire inspection for your facility was conducted on March 15, 2023. The facility was approved for day time care only. The following violation was observed today: Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Four (4) incident reports were reviewed. Four (4) reports did not contain the parent's signature, one (1) report did not have the signature of the person completing the report, and one (1) report did not identify the type of injury the child received. The date of the incidents were May 17, 2023, May 31, 2023, and two on June 7, 2023. .0802 (e) The violations documented must be corrected immediately. On or before September 26, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected.
