Upon arrival, the purpose of today’s visit was reviewed with you, Kristen Poarch, Lead Teacher. Your last annual compliance visit was conducted on November 17, 2023. Your program currently operates with a five (5) star license effective March 15, 2021. The permit restrictions were in compliance including day time care only, enhanced ratios reduced by one, certified developmental day, meets enhanced space, and meets enhanced ratios. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during independent center play, teacher lead group time activities, transitions, toileting, handwashing, lunch, outdoor gross motor free play, and rest time. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on November 7, 2024. Sanitation Inspection: The last sanitation inspection was completed on October 18, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills: The last fire drill was completed on October 31, 2024, at 1:39pm and 1:44pm both drills were recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: Two (2) staff files were reviewed today. Four (4) children’s files were reviewed today. There was one violation observed during today’s visit. I discussed this violation with you in detail at the end of the visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. On October 7, 2024 one child was not signed out. On October 8, 2024 one child was not signed out. On October 10, 2024 one child was not signed out. On October 16, 2024 one child was not signed out. 10A NCAC 09 .0302(d)(4) Technical Assistance: I suggested that you assign a staff member to review arrival and departure sheets daily to ensure that all children have been signed in and out each day. Consultation: Prior to today’s visit, your compliance history was ninety-eight (98) percent as of November 6, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Compliance Letter: You must correct the violation found during today's visit immediately. Please send me a letter verifying compliance by November 21, 2024. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. Upon arrival, the purpose of today’s visit was reviewed with Lead Teachers, Kristen Poarch and Kristi McDaris. Your program currently operates with a five star license effective March 3, 2021. The permit restrictions were in compliance including Developmental Day requirements. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios were in compliance. Children in care were observed during teacher directed activities, free play activities, outdoor gross motor activities, hand washing/bathroom routines, lunch and nap. Positive and nurturing interaction was observed. The file for one new staff was reviewed with no violations documented. One additional staff file was reviewed using the DPI verification form. The most recent sanitation inspection for your facility was conducted on November 3, 2023. A superior sanitation classification was issued with 4 demerits. The most recent fire inspection for your facility was conducted on October 5, 2023. The facility was approved for daytime care. A fire drill was conducted on November 6, 2023. NC Pre -K requirements were in compliance. Developmental Day Requirements were in compliance. Medication: one violation was documented. One violation was observed and documented today. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An inhaler was not in the original container and did not have a label from the dispensing pharmacy that included the child’s name, the name of the medication and the frequency of the dosage .0803(2)(a) The above uncorrected violations must be brought into compliance immediately. You shall submit a written, signed and dated statement to me at the address below detailing specifically how the above violation was corrected. Send a correction action letter to me by December 1, 2023. Include the facility name, ID# and the title of the person signing the documentation. As discussed, violations cited today and during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c). your compliance before today’s visit was 97%. Technical assistance: #844 Prescribed medications must be stored in the original containers in which they were dispensed with the pharmacy labels. If the medication is a pharmaceutical sample, it must be stored in the manufacturer's original packaging, be labeled with the child's name, and be accompanied by written instructions specifying: the child's name; the names of the medication; the amount and frequency of dosage; the signature of the prescribing physician or other health professional; and the date the instructions were signed by the physician or other health professional. Medications should not be accepted if they are not in the pharmaceutical packaging with pharmacy labels. An inhaler was not in the original container and did not have a label from the dispensing pharmacy that included the child’s name, the name of the medication and the frequency of the dosage.
