You, Holly Eason, Director, assisted with today’s visit. Your program currently operates with a four-star license effective August 8, 2018. The permit restrictions include the following: daytime care only, meets enhanced ratios, meets enhanced space, children under 2 ½ years old in rooms with direct exits only. The Secretary of State website was checked on October 16, 2025, and your business, GROGAN SMALL WORLD INC, was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment. Children throughout the facility were participating in routines, free play, gross motor play, lunch, rest time, and departure during the visit. Required Inspections: The last fire inspection was conducted on January 6, 2025. The last sanitation inspection was conducted on June 5, 2025. A “superior” classification was issued with four (4) demerits noted on the grade card. The last playground inspection was conducted on September 30, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on June 6, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on September 19, 2025. The last emergency drill documented was conducted on September 17, 2025. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. Your center is listed. No staff are listed under the roster. You are working to add current staff members included in your facility portal. Files Reviewed: Five (5) children’s files were reviewed during the visit. One (1) new staff file and one (1) existing staff file were reviewed during the visit. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility is listed in ABCMS. No staff are entered under the roster for the facility. G.S. 110-90.2 & .2703(r) The violation(s) documented must be corrected immediately. On or before October 30, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Technical assistance was provided on the following: ABCMS I discussed with you that you must notify the Division of all new child care providers who are hired or have moved into the child care facility within five business days by entering each staff member’s information into the Automated Background Check Management System (ABCMS) Provider Portal. I reviewed with you the ABCMS Provider Portal Access Guide document and provided you with a copy of the document during the visit. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. We discussed changes to the environmental rating scales and the implementation of the “3’s”. Additionally, please ensure all staff members have a WORKS account, that all transcripts have been submitted, and all WORKS accounts are up to date. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Upon arrival, the purpose of today’s visit was reviewed with you, Holly Moss, Administrator. Your program currently operates with a four star license issued on August 8, 2018. The permit restrictions were in compliance. The Secretary of State website was viewed today and Grogan Small World, Inc. was current and active. A walk-through of the facility was conducted. All indoor and outdoor areas were monitored. Children in care were observed during free play in activity areas, bathroom/handwashing routines, and nap. Staff interacted in positive ways with the children. Child/staff ratios were maintained. Two new staff files were reviewed. No violations were noted regarding staff files. The most recent sanitation inspection for your facility was conducted on June 8, 2023. A superior sanitation classification was issued with 0 demerits. The most recent fire inspection for your facility was conducted on January 11, 2023. The facility was approved for daytime care. Fire drills and lockdown/shelter in place drills had been conducted and recorded. One violation was observed today: Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #3, a permission slip was dated 7/19/23-7/21/23 giving permission to administer Hydrocortisone and Petroleum Jelly. The medication had not been sent home after the permission expired. .0803(12) The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 98% percent as of today August 21, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Corrective Action: The above uncorrected violation(s) must be brought into compliance immediately. You shall submit a written, signed and dated statement or email to me at the address below detailing specifically how the above violations were corrected.
