Your program currently operates with a 3-Star license issued December 5, 2019. Childcare Consultant, Natalie Pond, assisted with today’s visit. Due to travel time, we arrived at your facility at 9:15am. The last annual compliance was conducted on September 18, 2024. The sanitation inspection was completed December 13, 2024, with an Approved classification. The last fire inspection was conducted on December 13, 2023. Your facility was approved for 1st, 2nd, and 3rd shift care, the facility meets enhanced , and outdoor play in the fenced area only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two (92) percent as of September 10, 2025. The Administrator, Mia Pickett, was present and available for consultation. Four (4) children, between the ages of one (1) and four (4) years old are enrolled and two (2) were present today. There is one (1) approved indoor space and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. The children were engaged in free play with vehicles and toy animals, caring for a baby doll, and working on individualized art with markers. The teacher interacted with the children as they played, answering their questions and facilitating activities. Supervision and staff-child ratios were observed and found to be in compliance. Lunch consisted of breaded chicken, whole wheat bread, tossed salad, blueberries, and milk. Health and safety trainings were monitored, and requirements were met. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Lunch consisted of breaded chicken, whole wheat bread, tossed salad, blueberries, and milk. Health and safety trainings were monitored, and requirements were met. Needs Completing Immediately: Water Test- It’s time to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. Steps: 1. Sign Up – Pre-enrollment webinars are available at cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar into your web browser to see available dates and times for the next month. 2. Evaluate Hazards - The program will ship your facility a water sample kit and may coordinate an on-site visit by a professional to assess for lead-based paint and asbestos hazards, if needed. 4. Receive Results – Receive your results, recommendations, and water mitigation support, if needed. 5. Request Reimbursement - If you choose to conduct lead-based paint or asbestos mitigation, you can request reimbursement from NC DHHS. The program currently operates with a 3 Star License, issued December 5, 2019 earning 4 points in Education, 2 points in Program, and 1 quality point. Restrictions on the permit include: a first, second, and third shift capacity of 11 children 0-12 years of age, meets enhanced space, and outdoor play is limited to fenced area. Prior to this visit, the facility’s eighteen-month compliance history was 98%. The last annual compliance visit was conducted September 29, 2022. The last sanitation inspection was completed December 29, 2023, with a “Superior” classification. The last fire inspection was conducted December 19, 2023, and your facility was approved for daytime care only. Owner/operator Mai Pickett was present and available for consultation today. All indoor and outdoor spaces were monitored today. There are seven children enrolled and seven children ages 2-4, were present today. Daily schedules were available for children. Activity plans were observed posted in the child care area. A variety of age-appropriate learning materials was observed. Children in Space 1 were sitting at the tables upon arrival engaged in group time singing about the alphabet and days of the week. After group time, children engaged in activity areas with puzzles, musical instruments, dress-up clothing, manipulatives and interlocking blocks. All children were adequately supervised. The caregiver and children interacted in a positive and appropriate manner. The snack today consisted of goldfish. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A aerosol can of Lysol was observed on a metal shelf in the bathroom accessible to the children in care. .2820(b) A violation was documented and was corrected during the visit. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violation(s) documented will impact the compliance history score.
