Facility ID: 26001347 Consultant: SANDRA JOHNSON Operation Type: Center Case Number: Visit Date: 1/18/2024 Number Present: 40 Completed Date: 1/18/2024 Age: From 0 To 4 Total Minutes: 180 Time In: 08:30 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to an routine unannounced visit. The previous Annual Compliance Visit was completed, July 20, 2023, and prior today’s visit, the eighteen-month compliance history score was at 99%, which is above the 75% required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued August 20, 2021, The License was based on the program earning seven- points in education standards, six-points in program standards, and one quality point by choosing to meet an educational option with the child care administrator has at least 10 years of documented child care administrative work experience in a licensed program that can be verified by the Division. The information on record with DCDEE was verified as correct by Kimberly Malloy, and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed 6/22/2023. Fire Classification of Approved Daytime Only completed 7/5/2023. Ms. Malloy, Administrator, was present while I observed the indoor and outdoor areas. In space #1, #2, #3, and #5, children were observed participating in free choice activities. The children were observed playing with playdough, cookie cutters, stacking lego blocks, reading books, and playing with large and small balls. Lunch served today was chicken nuggets, corn, pears, wheat rolls, and milk. In space # 3, an observation was completed from 8:45a.m. to 9:00a.m., The children were observed moving about the space walking, crawling, and using push toys. One teacher sat on a small carpet and read stories and sang songs with the children while they played. The children would stop from time to time to look at the teacher and join her while she was singing. The other teacher also sang with the children while she changed their diapers and washed their hands and faces. The teachers worked well together. The children were engaged with the teachers as they talked to them and sang to them. In space#5, an observation was completed from 9:05a.m. to 9:20a.m., The children were observed participating in large motor activities due to the weather. The children played with small and large balls, they participated in red light, green light, game, and played a freeze tag game. They also played music and movement games. The teachers were very encouraging to the children, they enjoyed the games with the children. They laughed and played along with them, encouraging the ones who were not sure what to do. Two new staff files were monitored during today’s visit, see staff and training worksheet attached. The staff and training worksheet from the previous annual compliance visit was monitored for current information. One violation was cited. At the completion of the visit, the Visit Summary was reviewed with Ms. Malloy. The following violations were observed. Violation Number Comment Rule 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. One Staff member that work with infants did not complete ITS-SIDS training every three years. The staff members ITS-SIDS expired 9/29/2023 .1102(f) 1756 Enhanced staff/child ratios and group sizes were not met. In space #1, one staff member walked out of the classroom while children were having lunch and left on staff member with 13 children. This was corrected when the staff returned to the classroom after going to the restroom and remained in the classroom with the other teacher. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on February 1, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. Based on today’s classroom observation, I discussed with Ms. Malloy the positive interactions and conversations that were being had between the teachers and children as they played games and sang songs. The teachers were very engaged and encouraging to the children. I requested Ms. Malloy to share with the teachers the great work they are doing with the children to continue the positive interaction with the children. Do you have children who bite?
