Name of Operation: C. WAYNE COLLIER PRIME TIME Facility ID: 26001617 Consultant: YOLANDA PRIDGEN Operation Type: Center Case Number: Visit Date: 2/26/2025 Number Present: 27 Completed Date: 2/26/2025 Age: From 5 To 10 Total Minutes: 120 Time In: 02:40 PM Time Out: 04:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to an Annual Compliance Visit including health and safety training requirements. The previous Annual Compliance Visit was completed August 29, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 100%. This program currently operates with a Three-Star License issued February 24, 2020. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by choosing to meet a programmatic option for: Staff benefit package and Infrastructure of parent involvement. The information on record with DCDEE was verified as correct by Tiffany Payne. Current Inspections are as follows: Sanitation Classification of Superior completed September 9, 2024. Fire Classification of Approved completed October 23, 2024. Tiffany Payne, traveling program coordinator, and NeKeyah Bryant, group leader, were present and accompanied me while observing the indoor areas. The outdoor area was not monitored due to the license restriction: “Playground area does not meet child care playground safety standards. Upon arrival, two staff members were observed setting up the tables in the approved child care space. A section of the child care space was being used by the school for an afterschool event therefore all interest centers were not used due to space. Children arrived at approximately 3:05pm, washed their hands and put their belongings in their individually assigned bin. Staff members took attendance as children separated into various age groups and lined up to go the the bathrooms located in the hallway. Children were served snack which consisted of Scooby Graham Cracker Snacks and chilled 100% fruit juice. Following snack, I observed children playing with tabletop board games before going outdoors for free play. Families arriving to pick up their children were greeted and information was shared during departure. A checklist was used to note the requirements monitored during the visit. A completed Staff and Training worksheet was on file and updated during the visit. All staff files were monitored including the traveling program coordinator. The Children’s Record form was completed and three (3) files were monitored. The most recent fire drill was completed on February 25, 2024 and the lockdown emergency drill was conducted on January 28, 2025. The outdoor inspection form is not required due to the permit restriction for outdoor play area. At the completion of the visit, the Visit Summary was reviewed with T. Payne. The following violation was observed: Violation Number Comment Rule 528 Food substitution was not of comparable food value or recorded on the menu prior to the meal or snack being served. The posted menu stated Poptarts would be served however I observed children being served Scooby Graham Crackers. The violation was corrected with the traveling program coordinator made the correction on the posted menu. 10A NCAC 09 .0901(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violation observed and documented today was corrected immediately therefore a compliance letter is not required for today’s visit. Facility Specific TA/Consultation/Reminders: School-aged Professional Development Opportunities: The Cumberland County Partnership for Children is sponsoring the “Let’s Get Connected” The School Aged Connection. The in-person event focuses on creating a safe and inclusive community for sharing and learning new concepts. This learning event is for directors and school-age staff only (newly hired and/or seasoned Group Leaders). Training topics to be covered are: growth and development, classroom management, learning environments, building relationships, safety and wellness, positive behaviors, activity planning, and sanitation.
