WAYNE COLLIER ELEMENTARY Facility ID: 26002130 Consultant: TIFFANY REED Operation Type: Center Case Number: Visit Date: 3/7/2025 Number Present: 30 Completed Date: 3/7/2025 Age: From 4 To 5 Total Minutes: 275 Time In: 09:15 AM Time Out: 01:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to an Annual Compliance Visit, including compliance with requirements located in Child Care Rule Section .3000 in Spaces #38, #39, which serve children participating in the NC Pre-K program, and Health and Safety Training. The previous Annual Compliance Visit was completed March 14, 2024, and prior to today’s visit, the eighteen-month compliance history score was at 99%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a five-Star License issued February 19, 2020. The License was based on the program earning seven points in education standards, seven points in program standards, and one quality point by choosing to meet a programmatic option with has reduced group size decreased by at least one child per age group from the seven-point level as required in Rule .2818 (c). The information on record with DCDEE was verified as correct by Melinda Torrey, Lead Teacher. Inspections are as follows: Sanitation Classification of Superior was completed on September 09, 2024. Fire Classification of Approved for Day Time Care Only was completed on October 23, 2024. Melinda Torrey, Lead teacher, and Bessie McNeil, Lead Teacher, were present while I observed the indoor and outdoor areas. Upon my arrival, thirty (30) children and four teachers were present. Children had access to blocks, fine motor, dramatic play, creative art, and literacy materials. Children were observed participating in free-choice and outdoor play activities. In Space #38, children were observed building structures with magnetic tiles, practicing cutting with scissors and paper, operating on a baby doll in the dramatic play area, and exploring seashells in the science area. In Space #39, children were observed building structures with wooden blocks and Legos, drawing with markers, tracing their names with markers, cutting and glueing pieces of paper, and exploring with playdough. Both groups of children cleaned up materials, put on their coats, and lined up to transition to the outdoor learning environment at 10:35am. In the outdoor learning environment, children were observed running around the space, participating in a sack race, participating in red light-green light, playing on riding toys, and playing on the stationary playground equipment. The outdoor learning environment was monitored for playground safety requirements. At 11:45am, the groups lined up to return to their classrooms. When the group from Space #39 returned to their classroom, they put their coats into their cubbies, used the bathroom, washed their hands, and sat down on the carpet for story time. While the lead teacher read the children a story titled ‘Green Eggs and Ham’, the teacher assistant cleaned and sanitized tables to prepare for lunch. After the teacher read the story, the group had a discussion about times when they tried eating something for the first time. Routine care and handwashing practices were observed. Lunch consisted of chicken nuggets, waffles, emoji fries, cinnamon apples, and milk. Four staff files were reviewed. One staff member did not have documentation on file of completing the required Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of hire. Four children’s records were reviewed, and all documentation was in compliance. Program files were reviewed. The most recent fire drill was completed on February 25, 2025. The most recent shelter-in-place drill was completed on March 05, 2025. The most recent playground inspection was completed on February 25, 2025. Transportation requirements were reviewed and observed in compliance. I verified compliance by reviewing the physical files as well as the off-site verification forms for staff, children, and transportation. NC Pre-K requirements were reviewed. The lead teachers and teacher assistants that are currently listed in the NC Pre-K plan were present during today’s visit. The Creative Curriculum was implemented in the activity plan and the classroom activities. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in each child’s file. Hearing and Vision screenings were documented on each child’s health assessment form. Dental Screenings were placed in each child’s file. The teachers used Teaching Strategies to complete on-going formative assessments with children. The formative assessment results for each checkpoint were placed in each child’s file. At the completion of the visit, the Visit Summary was reviewed with M. Torrey. The following violations were observed: Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member (C.B.) did not have documentation on file of completing the required Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The staff member completed the training during today's visit, while the children were resting. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The violations observed and documented today may impact the compliance history score. Facility Specific TA/Reminders Special Training A violation was cited today for a staff member not having documentation of completing the required Recognizing and Responding to Suspicions of Child Maltreatment training. According to child care rule 10A NCAC 09 .1102(g), the child care administrator and all staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training shall count toward requirements set forth in Rule .1103 of this Section. McNeil, while the teacher assistant interacted with the other children in center play. Staff engaged in frequent, positive, and interactive language with individual children and groups of children by asking questions, responding to questions, and discussing their activities. Today’s breakfast was breakfast pizza, juice, and milk. Two new staff files were reviewed, one violation(s) were cited. At the completion of the visit, the Visit Summary was reviewed with M. Torrey. McNeil encouraged children to take their time using the scissors safely. B. McNeil repeated the words” open and close them” as the children were cutting out their octagon picture. Children were encouraged to clean their spaces as they transitioned to different centers. In activity centers, children were engaged in building structures with unit blocks and vehicles, playing with dramatic play materials, sand play with sand toys, and drawing with markers. In space #39, During outdoor play, teachers talked to children about playing safely and encouraged positive with other children. As children made good choices, teachers encouraged children. Throughout the visit, teachers monitored children completing personal care routines and hand washing. Both classroom teachers promoted increased creativity and exploration in the environment and trying new things, which are the building blocks for higher-order thinking. Positive interactions keep children engaged and encourage them to seek out and try new things. I encouraged teachers to continue to provide opportunities for exploration. TA: A violation were cited due to one staff member I.A. employed 2/10/2023 has not completed Recognizing and Responding to Suspicions of Child Maltreatment. Equipment and materials that are hazard free and in good repair are required for children. Additional information regarding indoor materials and equipment is in Sections .0500, .0600, and .2500 of the Child Care Rules, as well as .2822 of the Sanitation Rules. We discussed options for updating the equipment including: sealing with clear coat to extend the life of the current paint; stripping all paint off and sealing in the natural state of the wood; or replacing. Reminders: July 2023 Child Care Rule Changes: The DCDEE is excited to share information about the July 2023 child care rule changes. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. Two groups of children enrolled in the NC Pre-K program were observed participating in indoor and outdoor activities. Before transitioning to outdoor play, the children in Space #39 were participating in morning group time to discuss activities planned for the day. While outdoors children played with balls, climbed, and slid on the stationary equipment. Children in Space #38 completed an art project with lead teacher B. McNeil, while the teacher assistant interacted with the other children in center play. Staff engaged in frequent, positive, and interactive language with individual children and groups of children by asking questions, responding to questions, and discussing their activities. Today’s breakfast was breakfast pizza, juice, and milk. Two new staff files were reviewed, one violation(s) were cited. At the completion of the visit, the Visit Summary was reviewed with M. Torrey. The following violation(s) were observed. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member I.A. employed 2/10/2023 has not completed Recognizing and Responding to Suspicions of Child Maltreatment within 90 days of employment. .1102(g) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation(s) observed and documented today must be corrected immediately. Additionally, by 5:00pm on October 19, 2023, you must submit a written, dated, and signed statement to me . The statement must describe accurately and in detail how and when the violations were corrected. McNeil encouraged children to take their time using the scissors safely. B. McNeil repeated the words” open and close them” as the children were cutting out their octagon picture. Children were encouraged to clean their spaces as they transitioned to different centers. In activity centers, children were engaged in building structures with unit blocks and vehicles, playing with dramatic play materials, sand play with sand toys, and drawing with markers. In space #39, During outdoor play, teachers talked to children about playing safely and encouraged positive with other children. As children made good choices, teachers encouraged children. Throughout the visit, teachers monitored children completing personal care routines and hand washing. Both classroom teachers promoted increased creativity and exploration in the environment and trying new things, which are the building blocks for higher-order thinking. Positive interactions keep children engaged and encourage them to seek out and try new things. I encouraged teachers to continue to provide opportunities for exploration. Equipment and materials that are hazard free and in good repair are required for children. Additional information regarding indoor materials and equipment is in Sections .0500, .0600, and .2500 of the Child Care Rules, as well as .2822 of the Sanitation Rules. We discussed options for updating the equipment including: sealing with clear coat to extend the life of the current paint; stripping all paint off and sealing in the natural state of the wood; or replacing. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements.
