Upon arrival, I was greeted by Kristen Cullen, and she accompanied me throughout the visit. During today’s visit, there were 109 children, and 20 staff members were present. CURRENT LICENSE STATUS: The program operates with a 3-star rated license issued on 5/31/17. They received 2 points in program standards, 4 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 5/5/25. The last fire inspection was completed on 8/7/25. The last outdoor play area inspection was completed on 8/15/25. The last fire drill was conducted on 7/23/25. The last lock down/shelter-in-place drill was completed on 3/13/25. The EPR plan was updated on 3/19/25. The last annual compliance visit was conducted on 9/10/24. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements and applicable enhanced requirements including but not limited to approved space, child interactions, required in-service trainings, indoor and outdoor materials and equipment, storage of hazardous items, storage of medication and medication forms, children records, Staff Criminal Record Checks, attendance records, CPR and First Aid certificates, ITSIDS certificates, activity plans and supervision. A sample of children's files and staff records were reviewed during today’s visit. Ten children’s files and six staff files were viewed. A summary of the review was left with you along with this summary. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of September 2024, through August 2025, was reviewed during today’s visit. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, THE NEW SCHOOL, INC., was current/active. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Space #9 did not have an activity plan posted. GS 110-91(12); .0508(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A plastic bag was stored on a hook in the hallway; accessible to children younger than three. .0604(q) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was conducted on 3/13/25. .0604(u);.0302(d)(8) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Ten children files did not have a signed statement. .0608(b)(1-6) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 83%. TECHNICAL ASSISTANCE The last emergency drill was conducted on 3/13/25. Emergency drills should be conducted quarterly. Please conduct a shelter-in-place/lock down drill by the end of this month. A plastic bag stored on a hook in the hallway was accessible to children younger than two years of age. Plastic bags, whether intended for storage, trash, diaper disposal, or any other purpose, should be stored out of reach for children that are two years younger. Soil clothes can be stored in paper bags and placed in the child cubby. CONSULTATION AND RESOURCES: During today’s visit you stated that you have started the process for American Montessori Society (AMS). You can visit the website [www.ncchildcare.nc.gov] under “What New” and go to QRIS Modernization section to learn more about the accreditation pathways. There is a section on education requirements if you would like to apply for five stars using education component. HEALTH AND SAFETY Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Staff should use their individual NCID username and password to access this training. All staff members shall complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. This training counts toward requirements for Health and Safety. Recognizing and Responding to Suspicions of Child Maltreatment training is available at www.preventchildabusenc.org. We conducted a walk-through of the indoor and outdoor learning environments. I observed children playing on the playground and in the infant classroom the babies were napping. INSPECTION/SAFTEY DRILL STATUS: The last sanitation inspection was conducted on 6/27/24. The last fire inspection was conducted on 8/26/24. The last outdoor play area inspection was conducted on 3/18/25. The last fire drill was conducted on 2/13/25. The last shelter-in-place/lock down was conducted on 3/13/25. Violations were observed and documented during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. The last annual compliance visit was conducted on 10/19/23. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements and applicable enhanced requirements including but not limited to approved space, child interactions, required in-service trainings, indoor and outdoor materials and equipment, storage of hazardous items, storage of medication and medication forms, children records, Staff Criminal Record Checks, attendance records, CPR and First Aid certificates, ITSIDS certificates, activity plans and supervision. A sample of Children's files and all Staff records were reviewed during today’s visit. A summary of the review was left with you along with this summary. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of October 31, 2023, through September 10, 2024, was reviewed during today’s visit. Please see the attached Staff-Training Worksheet for specific information on each staff member’s training including delinquent and excess in-service training hours. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, THE NEW SCHOOL, INC. was current/active. You visited all indoor and outdoor space with me. Children enrolled in space #8 were napping. Children in space#10, #6, #2, and space #9 was eating lunch and making a transition to nap time. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in each classroom. GS 110-91(12); .0508(a) 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. In space #1 and #9 staff purses were stored less than five feet from the floor. .0604(a) 847 Parent's medication authorization did not include required information. An authorization form for desitin cream did not have the amount to apply. 10A NCAC 09 .0803(4)(6-9) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic material used for packing was accessible to children in a two year old classroom. .0604(q) Please submit a corrective action letter for the above violations. Parents completing the consent form should include child’s name, name of medication, date/time to be given, dose amount, how to be administered, and the length of time the medication is to be administered. I met the new center administrator, Anita Miller, during today’s visit. Anita accompanied me on the walk through and assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments I observed groups of children eating lunch and beginning to transition to rest time. RATED LICENSE STATUS The program operates with a 3-star rated license issued on 5/31/17. They received 2 points in program standards, 4 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. Full rated license assessments will occur based on the program’s Cohort assignment; your program is in Cohort 1, therefore your assessment period would begin on or around July 1, 2024. You stated that you do not plan to complete an Environment Rating Scale. You should also ensure that all staff have registered and have their education assessed in WORKS. It is recommended that you complete this process as part of your onboarding process with new hires and as existing staff members complete additional coursework. INSPECTIONS The last sanitation inspection was conducted 6/20/23. The last fire inspection was conducted 6/2/23. The last documented fire drill was conducted 4/11/24. I reviewed emergency medications, and confirmed that all staff have current CPR, First Aid and ITS SIDS certificates, as required. There have been no new staff hired since the annual compliance visit, so I did not review staff files. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. There is soil and mulch build-up around the fence line, resulting in the fencing being less that 4 feet high. Soil and mulch had built up around the playground fence line, resulting in the fencing surrounding the playground to be less than 4 feet from the ground to the top of the fence line. This can be corrected in several ways. The build up can be moved from the fence line or lattice can be added to the top of the fencing. There was less than 2 inches of mulch surrounding the fall zone of the climbing structures on the playground for younger children and the playground for older, preschool children. The mulch could be redistributed to the fall zone areas or new mulch can be added. There should be at least 6 inches of mulch, extending at least 6 feet around all anchored climbing structures. CONSULTATION All child care facilities are required to test their drinking and cooking water for led every three years. It may be time for your facility to complete this testing again.
