Jenna Ramsey, deputy director of DCDEE, accompanied me during today’s visit. Upon arrival, Jill Bellucco, administrator, assisted me with the visit. CURRENT LICENSE STATUS: The program operates with a 1-star rated license issued on March 12, 2025. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 9/3/25. The last fire inspection was completed on 2/10/25. The last outdoor play area inspection was completed on 1/15/26. The last fire drill was conducted on 12/9/25. The last lock down/shelter-in-place drill was completed on 12/20/25. The EPR plan was updated on 1/24/25. The last annual compliance visit was conducted on 7/10/25. The NC Secretary of State's website was reviewed on 1/20/26 and was listed as active- current. You visited all approved indoor and outdoor spaces with me during today’s visit. All groups were engaged in group-time activities in the classrooms. ITEMS MONITORED: A partial monitoring of applicable child care requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Staff Records, Program Records, License Posted, and Permit Restrictions. The following violations of the Child Care Requirements were observed and documented during today’s visit: Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) On child's feeding schedule was not posted. 10A NCAC 09 .0902(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Diaper cream in space #4 expired on 12/2024. .0803(12) All violations were corrected during the visit, no correction letter is required. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history score was 96%. TECHNICAL ASSISTANCE During today’s walk through we found diaper cream with an expiration date of 12/2024. North Carolina child care regulations r'equire that expired medications must never be administered. Unused or expired medications must be returned to parents or disposed of securely in accordance with FDA guidelines. Programs must follow established procedures for returning unused medications and properly discarding expired ones. CONSULTATION AND RESOURCES: Today, we discussed the available pathways, and you were given the opportunity to ask questions and share any concerns. Your current license is not due for renewal until March 2027. If you decide to increase your star rating, please visit the DCDEE website at www.ncchildcare.nc.gov. Upon arrival, I was greeted by Jill Bellucco, accompanied me throughout the visit. CURRENT LICENSE STATUS: The program operates with a 1-star rated license issued on 3/12/24. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 1/21/25. The last fire inspection was completed on 2/10/25. The last outdoor play area inspection was completed on 6/13/25. The last fire drill was conducted on 6/20/25. The last lock down/shelter-in-place drill was completed on 6/27/25. The EPR plan was updated on 1/24/25. The last annual compliance visit was conducted on 7/24/24. You visited all approved indoor spaces with me during today’s visit. Children were participating in group activities, and outdoor play. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements Eleven (11) staff files and ten (10) children’s files were reviewed during today’s visit. A sample of the files reviewed for children/staff was left with you. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of July 2024 through July 2025 was reviewed during today’s visit. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Vidi Vinci LLC., was active/current. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. One classroom did not have an activity plan posted. GS 110-91(12); .0508(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two bottles and one food item was not date. 15A NCAC 18A .2804(d) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Diaper cream and sunscreen were stored in an unlocked drawer accessible to children. 15A NCAC 18A .2820(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Expired medicine that was no longer used was not returned to the parent. .0803(12) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were accessible to children that were two year of age. .0604(q) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A teacher hired on 10/7/24 did not have verification on file. .1102(g) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. If I do not receive your letter by 7/24/25, and/or if you do not address all violations, I will be making a return visit to your program to monitor for compliance of violations and to obtain your compliance letter. Plastic bags should not be accessible to children under 3 years of age. Please remove diapers from their plastic covers when storing them. One classroom was observed with expired medication. Medication should not be used beyond the date of expiration. Unused medications should be returned to the parent/guardian for disposal. In the event medication cannot be returned to the parent or guardian, it should be disposed of according to the recommendations of the US Food and Drug Administration. The expiration date is so small and by putting a label on the medication with the expiration date will help staff to quickly recognize that the due date is quickly approaching, and the medication should be returned to the parent or discarded. CONSULTATION AND RESOURCES: HEALTH AND SAFETY Training in all the required health and safety topics must be completed every 5 years. Upon arrival, I was greeted by Jill Bellucco and she accompanied me throughout the visit. CURRENT LICENSE STATUS: The program operates with a 1-star rated license issued on 3/12/24. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 10/5/23. The last fire inspection was completed on 1/25/24. The last outdoor play area inspection was completed on 7/19/24. The last fire drill was conducted on 6/26/24. The last lock down/shelter-in-place drill was completed on 7/18/24. The EPR plan was updated on 1/5/24. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements and applicable enhanced requirements including but not limited to approved space, child interactions, required in-service trainings, indoor and outdoor materials and equipment, storage of hazardous items, storage of medication and medication forms, children records, Staff Criminal Record Checks, attendance records, CPR and First Aid certificates, ITSIDS certificates, activity plans and supervision. A sample of Children's files and all Staff records were reviewed during today’s visit. A summary of the review was left with you along with this summary. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of August 2023 through July 2024 was reviewed during today’s visit. Please see the attached Staff-Training Worksheet for specific information on each staff member’s training hours including delinquent and carryover hours. The following staff hours were not assessed because of the new starting date: T. Tarantini, R. Yates, T. Amy, C. Bragg, S. Breneman, P. Guevara, K. Hiel, M. Hiel, B. Escamilla, R. Higgins, and A. Kohne. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Vidi Vinci LLC. was current/active. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 615 Beds, cots and mats were not in good repair, properly handled, stored, or clean and sanitized between users. One cot in space #3 was unsafe for sleeping. 15A NCAC 18A .2821(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In one classroom the Bleach/water solution was stored less that 5 feet from the floor. .2820(b) 847 Parent's medication authorization did not include required information. In space #5 several authorization forms did not have required information. 10A NCAC 09 .0803(4)(6-9) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Two employee did not complete their yearly training hours. .1103(a) 1314 Emergency information did not name childs health care professional. One child file did not have a hospital preference listed. .0802(c)(2) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Any time authorization is given for over-the-counter cream the following information is needed from the parent: (A)the child's name. (B)name of medication/cream (C) the date to apply the cream. (D) the time given. (E) the amount and type of cream given. CONSULTATION AND RESOURCES: During our walk through the playground, we notice that there was a board used as a border to retain the mulch that is deteriorating from the weather. We also talked about perhaps adopting one brand of sunscreen; this will help staff to be able to organize their evening transitions and also to help eliminate missing information on the authorization forms. STAFF REMINDERS All staff members without early childhood experience and education will need to complete an additional 20 hours of training plus what they need to meet yearly requirements. Name of Operation: The Learning Experience Holly Springs Facility ID: 92003945 Consultant: ANGELA ALGER-WALKER Operation Type: Center Case Number: Visit Date: 9/14/2023 Number Present: 49 Completed Date: 9/14/2023 Age: From 0 To 4 Total Minutes: 240 Time In: 10:00 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's announced visit has been to conduct the first temporary time period visit. The temporary time period license was issued on 8/17/23 and expires on 2/17/24. Upon arrival, I was greeted by the center director, Jill Bellucco. She toured me through the center. We discussed the monitoring items for today's visit. We observed the center playground and conducted observation of the seven classrooms open for children during today's visit. We concluded the visit by reviewing this report and resource materials. Preschool-aged children were observed playing outside, washing their hands, participating in group time, doing workbooks in small groups, using the restroom, and transitioning from outside. Toddlers were observed listening to story, singing songs, and being diapered. Infants were observed being fed, freely playing with the available toys, and resting. The Children’s Records forms was completed during this visit. I monitored a sampling of the staff files. The director attests that all children’s file and facility/program records are current and contain all the required information. She will have the Staff and Training Worksheets prepared for the next visit. Forty-nine children were present during this visit. The center had the following items posted/located in a prominent place in the center for parents and available for me to review: --the center's license --a current and nutritious menu --a Safe Arrival and Departure Procedure --Summary of the NC Child Care Law posted The first fire drill was completed on 8/30/23. The outdoor inspection was completed on 9/11/23. The administrator must complete the Emergency Preparedness and Response training before 8/3/24 and complete the EPR Plan within four months of taking the training. She is scheduled to participate in the training on 10/7/23. She will then begin conducting and documenting the emergency drills quarterly. The center does not provide aquatic activities, transportation, or school-age services; thus, those requirements were not monitored during this visit. CHANGE OF OWNERSHIP PROCESS The building, fire, and sanitation inspections for the new owner must be completed, approved, and sent to me before 1/17/24. The Regional Manager stated that she will contact the various inspectors soon. I encouraged her not to wait too long but to give herself plenty of time to get any items they may require to be completed in time to submit the forms as requested. We also discussed that the building permit will need to be applied for with the City of Holly Springs Inspections Department before any renovations involving plumbing, electrical, demolition, or additional walls can begin. The policies were completed and approved on 8/17/23. The Following violations were observed during this visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Space # 3 and Space #7 had activity plans posted that were dated for the previous week (9/5-9/9/23). GS 110-91(12); .0508(a) 468 When three year old children and older were in care, the materials and equipment in the activity area was not in sufficient quantity to allow at least three children to use the area regardless of whether the children choose the same or different activities. The language and book centers in space # 7 did not have three related types of activities accessible to the children during today's visit. .0510(d)(1) 705 Equipment and furnishings were not sturdy, stable and free of hazards. Several mats throughout the center have sections that are beginning to tear or have open cracks exposing foam to the children. The teacher storage cabinet in space # 8 is missing a hinge at the bottom right section which does not allow the door to close properly. .0601(c) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. The gate has a large opening at the bottom with metal links protruding where children could be injured. GS 110-91(6); .0605((i) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. One child. L. Adams, did not have a medical action plan for his Epipen that was on file at the center during today's visit. .0801(b) The violations documented during today's assessment were reviewed with the operator and must be corrected IMMEDIATELY. Evidence of correction must be sent to me no later than September 21, 2023. Compliance verification must be a dated and signed letter that includes statements addressing how and when the violations were corrected. Please include the facility's name and ID number on the letter of compliance. The letter may be emailed to angela.algerwalker@dhhs.nc.gov. If you email the compliance letter, you must include a signature block, which contains your name, position, facility name, and facility ID number. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 COMPLIANCE HISTORY When you receive your Child Care License from the State of NC, you are agreeing to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand and to monitor your staff to ensure that all requirements are maintained at all times. TECHNICAL ASSISTANCE AND CONSULTATION The following items were discussed during today' visit: 1) Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, ITS-SIDS training, child maltreatment training, and health and safety training requirements. 2) Please refer to the What’s New Section of the DCDEE website for the new rules and other updates. 3) The NC Early Childhood Equivalency Certificate Exam is now available on Moodle (directions: Moodle – CCDF – NC Early Childhood Equivalency Exam). This exam is a pathway option comparable to a NC Early Childhood Credential or EDU 119. This equivalency exam introduces the foundations of culturally responsive, equitable and inclusive early childhood education, planning intentional developmentally appropriate experiences, learning activities, and teaching strategies for indoor and outdoor environments for all young children, guidance techniques, and professionalism. Exam Outline: What is the criteria to take the equivalency exam option? • at least 18 years of age or older • a high school diploma or equivalent Types of questions (format)and how many questions? • 100 questions. • true/false and multiple choice What would the passing criteria be? • achieve an 80% or higher. • If the person does not pass the first time, there will only be 2 attempts allowed within a 6-month period. • If they haven’t passed after 2 attempts/6 months, the individual will be advised to enroll in EDU 119 or pursue one of the other equivalencies. Once the certificate is generated in Moodle, the early childhood educator will upload the certificate in their WORKS account to be verified. If an early childhood educator have any questions or concerns about the exam , please have them contact dcdee.equivalency.exam@dhhs.nc.gov. 4) Emergency Preparedness and Response - It is important to consider how to prepare for and respond to emergency or natural disaster situations and develop written plans accordingly. You are required to have procedures in place to address natural disasters that are relevant to your location (such as tornados, flash floods, and hurricanes) and all hazards/disasters that could occur in any location including acts of violence, bioterrorism/terrorism, exposure to hazardous agents, facility damage, fire, missing child, power outage, and other situations that may require evacuation, lock-down, or shelter-in-place. The procedures must be updated and reviewed annually. The only way to prepare for disasters is to consider various worst case or unique scenarios, and to develop contingency plans. By brainstorming and thinking through a variety of “what if...” situations and developing records, protocols/procedures, and checklists, facilities will be better able to respond to an unusual emergency or disaster situation. EPR training is required to be completed within one year of licensure. 5) The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. Proper maintenance is a key factor when trying to ensure a safe play environment for children. Each playground is unique and requires a routine maintenance check program developed specifically for that setting. 6) A current dated activity plan is required for each group of children. We reviewed completing at least two weeks of activity plans and posting the plan for the next week at the end of the day the Friday before. This will ensure a current activity plan is posted for the next week. 7) Growing up healthy is every child's right and every parent's dream for their children. Significant advances in improving the overall health and development of children with chronic illnesses have been made. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. RATED LICENSE APPLICATION INFORMATION Once you have achieved a full six (6) months of operation, your program will be eligible for a Voluntary Enhanced Rated License. The award of this Rated License will occur at the end of the 6-month Temporary License Time Period. The Regional Manager stated that the facility will only be applying for a one star rated license at the end of the temporary time period. The following information will need to be completed to begin the program's Rated License Assessment: Once the approved inspections are submitted, the facility can be issued a Four Star Rated License effective 2/18/24 based on previously earning 4 points in staff education, 7 points in program standards, and one quality point. You can be issued a Rated License based on a previous assessment due to being in “Hold Harmless” status. A full Rated License Assessment will occur based on the program’s Cohort assignment; your program is in COHORT #1, therefore the assessment period would begin on or around July 1, 2024. You should begin preparing for your assessment now as the cohort system includes a planning/preparation period prior to the assessment period.
