Upon arrival, Jennifer Frink, administrator, assisted me with Today’s visit. CURRENT LICENSE STATUS: The program operates with a five-star accreditation issued on 7/25/25. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 7/21/25. The last fire inspection was completed on 1/5/26. The last outdoor play area inspection was completed on 1/5/26. The last fire drill was conducted on 12/7/25. The last lock down/shelter-in-place drill was completed on 1/12/26. The EPR plan was updated on 11/1/2025. The last annual compliance visit was conducted on 7/23/25. The NC Secretary of State's website was reviewed on 1/29/26 and was listed as active- current. You visited all approved indoor and outdoor spaces with me during today’s visit. Children were observed appropriately engaged in group-time activities, including reading books and participating in art activities within the classrooms. One group of children was also observed riding tricycles on the playground. ITEMS MONITORED: A partial monitoring of applicable child care requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Staff Records, Program Records, License Posted, and Permit Restrictions. The following violations of the Child Care Requirements were observed and documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Two classrooms did not have a current activity plan posted. GS 110-91(12); .0508(a) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. A staff member hired on 10/6/25 did not have a current TB test (12/29/23) on file. .0701(d) Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. TECHNICAL ASSISTANCE During today’s file review, it was noted that a staff member hired on October 6, 2025, has a TB test on file dated December 24, 2023. This date precedes the employee’s hire date and does not meet current onboarding or compliance requirements. CONSULTATION AND RESOURCES: ABCMS PROVIDER PORTAL North Carolina child care providers can now view and edit their facility’s staff roster in the ABCMS Provider Portal. You are required to list new employees within five days of hire and add/delete applicants on your roster as needed. To get access, you must complete the ABCMS Child Care Provider Portal Training in Moodle and submit the signed power form available at the conclusion of the training. FACILITY LEAD H2O, LEAD-BASED PAINT, ASBESTOS TESTING Child care facilities are now required to meet rule requirements (NC 10A NCAC 41C.1001-1007, NC 154A NCAC 18A.2816). Clean Classrooms for Carolina Kids are designed to ensure facilities meet all requirements to test lead and asbestos hazards in buildings. In viewing the website, I noticed that you did not complete all requirements, it’s possible that your facility could potentially receive violation in the near future. CURRENT LICENSE STATUS: The program operates with a 4-star rated license issued on 12/5/24. They received 2 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. ANNUAL INSPECTIONS: The last sanitation inspection was completed on 7/21/25. The last fire inspection was completed on 1/10/25. The last outdoor play area inspection was completed on 7/10/25. The last annual compliance visit was conducted on 7/30/24. You visited all approved indoor spaces with me during today’s visit. Children were participating in group activities, and outdoor play. A full assessment was conducted. A monitoring checklist was used to observe all minimum licensing requirements Eight (8) staff files and ten (20) children’s files were reviewed during today’s visit. A sample of the files reviewed for children/staff was left with you. ON GOING TRAINING HOURS Your center’s annual Inservice training for the period of July 2024 through July 2025 was reviewed during today’s visit. Prior to the visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Lightbridge Academy Holly Spring, LLC., was active/current. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 402 Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. The changing table in space#3 had a crack surface. 15A NCAC 18A .2819(a) & (b) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. A bottle and foods item in space #5 was not dated with the correct date. 15A NCAC 18A .2804(d) 847 Parent's medication authorization did not include required information. In space #2 several permission forms expired in June/2025. 10A NCAC 09 .0803(4)(6-9) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A plastic bag in a two year old classroom was stored in a child's cubby; which was accessible to children. .0604(q) Please submit a corrective action letter for the above violations. Plastic bags should not be accessible to children under 3 years of age. Please remove diapers from their plastic covers when storing them. One classroom was observed with an expired permission form. A good suggestion would be to have all the forms dated with the same ending date. Also, to have the medication labeled with the ending date. You could also have the permission form reduced in size and apply the form to the medication. Playground surfaces, especially those made of metal or dark-colored materials, can get dangerously hot in direct sunlight, posing a risk of burns to children. To mitigate this, consider shade structures, water features, and surfaces like engineered wood fiber or perforated rubber tiles, which are known to stay cooler. Today’s ambient temperature is high, and it is sunny, so we checked the temperature of the playground surfacing; it felt hot to the skin, so we discussed that finding with the director. Since your entire play areas are covered in artificial grass surfacing, we discussed the potential for burn hazards. Providing shade over play areas may help reduce direct sunlight exposure and keep surfaces cooler, especially for younger children that sit, crawl lay on the ground surfacing. Preventing burns: Test surface temperatures: Before letting children play, caregivers or administrators should touch surfaces like artificial surfacing, slides and metal platforms to check for excessive heat. Consider the time of day: Playground use should be limited during the hottest parts of the day, especially during peak sun hours. Water features: Incorporating water features like sprinklers or misters can help cool down surfaces and equipment. We left two handouts with you during today’s visit on monitoring hot playground surfaces. TECHNICAL ASSISTANCE ON HELPING TODDLERS ADJUST TO NEW CARE SETTINGS We observed a child in an older toddler group crying, probably due to new, unknown people entering her classroom and the teacher mentioned that she was new and entering childcare for the first time. The teacher quickly moved to the child and provided comforting reassurances to her. We also observed other groups of toddler-aged children where they all appeared happy in their environments, even during transition periods between outdoor play, meals and rest. Having children begin their enrollment in child care settings as toddlers can be challenging for them and their caregivers. It is typical for them to have trouble adjusting to the new environment and may experience periods of stress, crying and struggles with routine activities such as drop-off, transitions, napping and meals. Caregivers play a crucial role in helping toddlers adjust to child care by providing emotional support, building trust, and creating a nurturing environment. Here's some things you can do to facilitate this adjustment: -Emotional Support and Nurturing: Caregivers offer a warm and welcoming presence, providing comfort and reassurance to toddlers as they navigate the new environment. -Building Trust and Familiarity: Establishing a trusting relationship with caregivers helps toddlers feel safe and secure in their new surroundings. This can be achieved through consistent routines, positive interactions, and familiar items from home. -Individualized Attention: Tailoring interactions to each child's needs and temperament helps them feel seen, heard, and understood. -Comforting Items and Rituals: Familiar objects from home, like a favorite blanket or toy, can provide a sense of comfort and security during the initial days of daycare. -Patience and Understanding: It's important to recognize that adjusting to child care is a process, and caregivers should be patient and understanding throughout the child's journey. Four children files did not have verification of the law on file. Upon arrival, I was greeted by Jennifer Frink, and she accompanied me throughout the visit. CURRENT LICENSE STATUS: The program operates with a 4-star rated license issued on 6/17/24. They received 2 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. ANNUAL INSPECTIONS: The last sanitation inspection was completed on: 8/24/24. The last fire inspection was completed on 11/15/23. The last outdoor play area inspection was completed on 7/25/24. The director, Jennifer Frink, assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments I observed groups of children engaged in free play in their classrooms and outdoors. I also observed a diaper change, infant rest time, hand washing and face to name matches as groups of children transitioned from their classroom. RATED LICENSE STATUS The program operates with a 4-star rated license issued on 5/20/21. They received 2 points in program standards, 7 points in staff education and one quality point through the programmatic option of having a staff benefit package and an infrastructure of parent involvement. Full rated license assessments will occur based on the program’s Cohort assignment; your program is in Cohort 3, therefore your assessment period would begin on or around July 1, 2026. You should begin preparing and planning for your assessment on or after July 1, 2025, if you wish to complete the Environment Rating Scales. You should also ensure that all staff have registered and have their education assessed in WORKS. It is recommended that you complete this process as part of your onboarding process with new hires and as existing staff members complete additional coursework. INSPECTIONS The last sanitation inspection was conducted 8/31/23. The last fire inspection was conducted 11/15/23. The last documented fire drill was conducted 3/24/24. The last documented shelter-in-place or lockdown drill was conducted 2/13/24. The last documented playground inspection was conducted 3/18/24. The status of the owners, Lightbridge Academy Holly Springs, LLC., is listed as current and active on the NC Secretary of State website. Limited monitoring of child care requirements was assessed during today’s visit. I reviewed emergency medications, 8 new staff files and confirmed that all staff have current CPR, First Aid and ITS SIDS certificates, as required. The program does not transport children or enroll school aged children, therefore, these requirements were not assessed. During today’s visit, we discussed that the center’s indoor space calculations are based at 35 square feet per child, because there was not enough outdoor space to meet the requirement of 100 square feet per the total licensed capacity. A new rule, 10A NCAC 09 .2809 (a), states, “There shall be at least 30 square feet inside space per child per the total licensed capacity and 100 square feet outside space for each child using the outdoor learning environment at any one time.” This means that the center now has the option of reducing their indoor square footage to 30 square feet per child. When considering if this change will be beneficial to the center, we discussed the following factors. No class should exceed the number of students permitted in the space. No class should exceed the number of students permitted by the maximum group size, which is determined by the age of the children assigned to the class space. The center can not exceed the capacity listed on the license, which is currently 170, however, this could increase to 199 without having additional licensing considerations to the center. You stated that you would talk with your owners and district manager and will follow up with me if you decide to proceed with the lower indoor square footage. You may not make this change until a new license has been issued to your center, reflecting the changes made to your indoor square footage and/or capacity. It is helpful to designate a staff member who routinely checks the medication, allergy care plans and medication authorization forms to ensure that they are compliant with child care requirements. CONSULTATION All child care facilities are required to test their drinking and cooking water for led every three years. It may be time for your facility to complete this testing again. In addition, child care facilities are now required to test for lead and asbestos hazards in their buildings.
