The program currently operates with Five Star License, issued September 2, 2025. Restrictions on the permit include: a capacity of 36 for first shift only with an age range of 3-5 years old, meeting enhanced ratios and enhanced space, #200 and #712 at 18 children each, meeting enhanced ratios minus 1, and certified developmental day. Your last sanitation was on November 14, 2025, with a superior rating with 10 demerits. Your last fire inspection was on October 28, 2025, approved for daytime care. Your last annual compliance was on May 5, 2025. The compliance history was reviewed today and as of March 24, 2026, your 18-month compliance history is 97%. The facility is operated by Onslow County Schools. Today 30 children are enrolled and 24 children are present today, ages ranging from 4-5 years of age. Two mobile units on the Dixon Elementary campus used for NC Pre-K. Today all approved indoor and outdoor spaces were observed today. All indoor spaces are arranged into activity areas that include but are not limited to language, blocks, art, math, science, sensory, dramatic play, and manipulatives. A variety of age-appropriate learning materials were observed in each classroom. Daily schedules and activity plans were available for each group of children Today children in space 1 were observed engaged in free play. Children were observed playing in centers including but not limited to playing with a doll house, coloring a boat with markers, reading books. The children transitioned to group time where the teacher explained deep breathing, emotions and did a dance. The children transitioned outside where they were observed playing on the rock climber, the climbing structure, riding bikes, playing in the sand box, and making music. In space 2, children were observed engaged in free play. The children were playing in the sensory bin, coloring pictures, playing on a tablet, playing with Legos, and playing in housekeeping. The teacher used a timer for the children on the tablet and recorded their screen time. Staff/Child ratios were in compliance, and all children were adequately supervised. Staff were interacting with children on the child’s level and appropriate open-ended questions were being asked. Morning snack was observed. For morning snack, French toast sticks, applesauce and milk was served. Proper handwashing and food preparations were observed. NC Pre-K Site Monitoring - The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 8:00am to 2:45pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Ages and Stages Questionnaire (ASQs)screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of ongoing assessments was observed in children’s files. Portfolios were observed in notebooks in the classrooms. Family engagement activities include but are not limited to newsletters, stuffed animal pet adoption, and open house. It was noted that all requirements were in compliance. Six children’s files and three staff files were monitored, including new staff files. The following violations were observed and documented during today’s visit. The checklist shall be signed by the person who conducts the inspection and shall be maintained for 12 months in the center's files for review by a representative of the Division. The playground inspection includes a checklist of items related to safety, surfacing, and equipment quality. Your March playground inspection also needs to be conducted. With the inclement weather, today I suggested to fluff and add mulch if needed around your climbing structures to ensure there is adequate depth of surfacing. Nationally, nearly 70% of playground related injuries are from falls. Fall injuries are the result of two things: 1) The height a child falls from and 2) The material/surface the child falls upon. CLEAN CLASSROOMS FOR CAROLINA KIDS Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed on October 10, 2023. Water testing must be completed every three (3) years. • Lead-Based Paint- exempt • Asbestos- no results reported You will need to follow up to ensure you have completed all steps for these two items. The facility currently operates with a Five Star License issued April 17, 2020, earning 7 points in Staff Education, 7 points in Program Standards and 1 Quality Point in the Programmatic Option. This facility’s eighteen-month compliance history is 100%. This program is operated by Onslow County Schools. The last annual compliance visit was conducted May 8, 2024. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. This facility’s eighteen-month compliance history is 100%. This program is operated by Onslow County Schools. The last annual compliance visit was conducted May 8, 2024. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was 100% as of December 11, 2023. This program is operated by Onslow County Schools. There are two mobile units used for NC Pre-k. Ann Jones and Sandra Hatcher were the classroom teachers in space #712 and Ms. Clark and Ms. Bridges were the classroom teachers in space #200. Upon arrival to Ms. Clark’s classroom (space #200), I greeted the staff and quickly realized it was unusually cold inside the classroom. The classroom staff stated it was 54 degrees indoors as I also observed on the digital thermostat. The classroom staff stated there had not been any heat in the classroom since they arrived today, but a work order had been submitted at 7:30am this morning. I discussed the concern with Ms. Reyes, Assistant Principal and we worked together on a temporary solution. We planned to temporarily transition the children from the mobile unit to an unused classroom inside the school. As Ms. Reyes and I entered the classroom to discuss the temporary plan with the staff, we heard the heating system operating. I could feel warm air coming from the ceiling vents. The temperature on the thermostat began to rise. I observed a maintenance employee actively working on the outside heating unit. Later, the maintenance employee shared the compressor was having issues and he didn’t know how long it would continue to operate. Ms. Davis, Director of the NC Pre-K program and Michael Singleton, Onslow County Environmental Health Inspector were notified of the classroom temperature concern. There were twenty-one children present, ranging in age from 3 years to 5 years of age. Observations were conducted in the two classrooms. In classroom space #200, the children were engaged in indoor free play. One staff member was working with a small group of children on a writing activity while the other staff member assisted children with a snowman craft activity.
