Your program currently operates with a Five Star license. Restrictions include Daytime Care Only; Meets enhanced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 90% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during group time, free play, and playing in their outside learning environment. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on February 27, 2025. A sanitation inspection was completed on November 19, 2025, with a Superior classification. The last fire inspection was conducted August 13, 2025. Program records and required postings were monitored. A fire drill was conducted on January 30, 2026. A lockdown drill was documented on December 22, 2025. An outdoor inspection was documented on January 20, 2026. Staff files and children’s records will be monitored at a later date. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation for children enrolled in your program. I observed first aid kits and fire extinguishers on the vehicle. I monitored one vehicle today. The other vehicle was unavailable. I will monitor this vehicle during my follow-up visit. I observed a 2012 Chevy Bus, Tag #TLS3. This vehicle was clean, in good repair, had working seatbelts and was current on inspection and insurance. Your next inspection date on this vehicle is due August 31, 2026, and the insurance is valid through August 6, 2026. The following violations were cited during today’s visit: Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. In Space Twos the lesson plan was dated February 2-6, 2026. GS 110-91(12);.0508(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the 3's space two outlets in the surge protector did not have an outlet cover. 10A NCAC 09 .0604(c) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. This visit was conducted by Turkessha Casterlow, Childcare Consultant. I was greeted and accompanied through the facility by Cole Kenworthy, Assistant Director. Your program currently operates with a Five Star license effective May 17, 2017. Restrictions include daytime care only; Meets enhanced ratios; Meets enhanced space. The NC Secretary of State website was reviewed on August 18, 2025, and The Little School of Hillsborough at Waterstone, LLC was listed as current and active. The program’s compliance history was 87% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during outside play, free play, and routine care. Eight new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. A playground inspection was recorded July 25, 2025. A fire drill was recorded on July 31, 2025. A lock down drill for the facility was recorded June 26, 2025. The most recent fire inspection was on August 13, 2025. I observed a sanitation inspection that was last conducted on May 12, 2025, earning a Superior classification. The facility does provide transportation. The children were on a field trip, and the information was on the vehicles. In Spaces 3, 4, and 7 power strips did not have outlet covers. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In Space 16 the closet door was open and the key was in the door. Cleaning agents were stored at the top of the cabinet. .2820(b) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Children are particularly vulnerable due to their developing bodies, smaller size, and tendency to explore by touching and tasting. Your program currently operates with a Five Star license effective May 17, 2017. Restrictions include daytime care only; Meets enhanced ratios; Meets enhanced space. The NC Secretary of State website was reviewed on August 18, 2025, and The Little School of Hillsborough at Waterstone, LLC was listed as current and active. The program’s compliance history was 87% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during outside play, free play, and routine care. Eight new staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. A playground inspection was recorded July 25, 2025. A fire drill was recorded on July 31, 2025. A lock down drill for the facility was recorded June 26, 2025. The most recent fire inspection was on August 13, 2025. I observed a sanitation inspection that was last conducted on May 12, 2025, earning a Superior classification. The facility does provide transportation. The children were on a field trip, and the information was on the vehicles. The following violations were cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Blue Lizard sunscreen had expired 4/25 in Space 12. The annual compliance visit was conducted on February 27, 2025. Upon arrival at the facility, I stated the purpose of my follow-up visit. Jennie Ogden, Director, and Shannon Borland assisted me during today’s visit. A partial monitoring was conducted today. I monitored a sample of staff files, and a sample children's records. Program files and program requirements, were monitored during the previous annual compliance visit. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. **Documentation that all administrative staff has reviewed the EMC Plan annually was not made available.** 10A NCAC 09 .0802(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. **The administrator's ITS-SIDS was not renewed prior to the expiration date.** .1102(f) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. **Documentation that all administrative staff has reviewed the EPR plan annually.** .0607(e) The above violations must be corrected immediately. The last Annual Compliance Visit was conducted on March 7, 2024. The facility operates with a Five (5) Star License, which was issued on May 17, 2017. Upon arrival at the facility, I was greeted by a staff member and Ms. Shannon Borland, the assistant administrator. I introduced myself and stated the purpose of my visit. Ms. Borland invited me in and together we conducted a walkthrough of the facility. Attendance was taken and observations were made. The children were observed participating in indoor and outdoor free play, routines, group time, teacher’s directed activities and having lunch and rest. A full assessment of childcare requirements was conducted on this date. The permit was posted, restrictions maintained. I monitored supervision, staff/child ratio, adequate approved space. I monitored program records, transportation and the outdoor learning space. The last Fire Inspection was conducted on August 26, 2024. The last Sanitation Inspection was completed on December 2, 2024 and the facility achieved a ‘Superior’ classification. The following violations were observed, cited and technical assistance was provided. In a classroom used for children who are 3-4 years old, I observed a power strip with three uncover plugs. *** 10A NCAC 09 .0604(c) 1867 The depth of the loose surfacing was not based on critical height of the equipment. **On the playground used for children who are 3-5 years old needs mulch added beneath the dome climber.** .0605(k)(1-4) The above violations must be corrected immediately. On January 27, 2025, a member of ERS Institute attended a meeting with the staff to discuss preparation for the new Pathways and ITERS/ECERS 3 and SACERS overview/training. The last annual compliance visit was conducted on 3/07/24. The last fire inspection was completed on 8/26/24 and the sanitation inspection was last completed on 6/17/24 with a superior rating. Ms. Rhodes accompanied me as I completed a walkthrough of the indoor and outdoor learning environment. I reviewed 8 new staff files. I conducted a partial assessment of the childcare requirements today. I observed the children playing in the outdoor learning environment, having group time, working on art projects and preparing for lunch. The infants were observed in various care routines as well as outdoor and indoor activities. Natasha Rhodes, Assistant Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Jennie Ogden, Director, Ms. Rhodes and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On February 29, 2024,Three staff members were in the classroom while another staff member was outside alone with eleven (11)one to two -year-old children. GS 110-91(7);.0713(a-d) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On February 21 & 22, 2024, a staff member lifted two (2) one-year-old children by their hands. In addition, on April 17, 2024, the same staff member lifted a two-year-old child underneath their shoulders causing redness to the child’s upper shoulders. This program is operating with a five-star license. Jennie Ogden, the administrator, greeted and invited me in upon arrival. Caroline Sykes, child care consultant, accompanied me today. A therapist was present during the visit today. I completed a walkthrough of the indoor and outdoor learning environment. Tasha Rhodes, the assistant administrator, accompanied me. I observed the children playing with the toys and materials, being diapered, having a group time, eating a morning snack, playing in the outdoor learning environment, and eating lunch. 10 percent of the staff and children's files were reviewed. The operator completed the last fire inspection on 7/19/2023. The last sanitation inspection was completed on 11/27/2023. During today’s visit, two buses used for transporting school age children were monitored. I found the transportation requirements to be in compliance. I observed the following violations today. This childcare center is operating with a five-star license. Upon arrival, I was greeted by the director, Jennie Ogden, and the assistant director, Tasha Rhodes. I informed them the purpose of my visit. The last annual compliance visit was conducted on 3/15/2023. The last fire inspection was completed on 7/19/2023 and the sanitation inspection was last completed on 5/8/2023 with superior rating. Ms. Rhodes accompanied me as I completed a walkthrough of the indoor and outdoor learning environment. I reviewed 14 new staff files. I conducted a partial assessment of the childcare requirements today. I observed the children playing in the outdoor learning environment, having a group time, working on an art project, riding bikes in a closed off part of the parking lot. The infants were observed being bottle fed, being held, and having an outdoor time. I observed the following violations during today’s visit and provided technical assistance on the items. Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. "In the parking lot area where the children were riding bikes at, there was a can of paint and loose plastic bags accessible to the children. In the storage shed in the same area, there were numerous cans of paint and containers of chemicals. The storage was not locked." .0604(a) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily.
