This visit was conducted by Turkessha Casterlow, Childcare Consultant with Alma Dixon, Director and Tina Kimber, Lead Teacher. Your program currently operates with a Five Star license. Restrictions include Daytime care only; Children in care on ground level only; Meets enhanced ratios; Meets enhanced space; Children under 2 ½ years old in rooms with direct exits only. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on March 9, 2026, and Future Scholars Childcare and Development Center, LLC was listed as current and active. The program’s compliance history was 94% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during free play, playing in their outside play environment, and nap. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Mother Goose. The last annual compliance visit was conducted on April 8, 2025. A sanitation inspection was completed September 10, 2025, with a Superior classification. The last fire inspection was conducted on April 1, 2025. Program records and required postings were monitored. A fire drill was conducted on March 2, 2026. A shelter in place drill and lock down drill was documented on January 5, 2026. An outdoor inspection was documented on March 2, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. No medications were observed nor reported. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored one vehicle today. I observed a 2016 Ford Transit 350, Tag# EHN-2455. This vehicle was clean, in good repair, had working seatbelts, had adequate tire tread, and was current on inspection and insurance. Your next inspection date on this vehicle is due June 30, 2026, and the insurance is valid through March 1, 2027. The following violations were cited during today’s visit: Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. Floor tiles leading to the office are cracked and broken. 15A NCAC 18A .2824(a)&(b) 1314 Emergency information did not name child's health care professional. One child did not have a health professional on file. .0802(c)(2) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Technical assistance discussed during today’s visit related to violations: • Having a child’s health professional (pediatrician/doctor) on file in child care is essential for immediate, informed action during medical emergencies, ensuring staff know who to contact, and allowing for consistent care of chronic conditions like asthma. It facilitates required documentation of immunizations, screenings, and tailored health plans. • Floors in child care must be clean and in good repair to prevent the spread of illnesses, reduce allergy triggers, and ensure physical safety. Since children, especially infants and toddlers, spend most of their time crawling and playing on the ground, clean floors reduce exposure to germs and toxins. Proper maintenance prevents injuries like slips or trips. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • During this visit we discussed Pathway 2. We discussed the requirements for the pathway. After discussing the pathways, it seems that your facility is leaning closer towards Pathway 2. Your curriculum is approved by DCDEE. I suggested DRDP as a possible choice for your formative assessments. I emailed prior to today’s visit a copy of the Application for Assessment for a Rated License. Your application is due to me by Friday April 3, 2026. The following violations were cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There are four wall outlets, three outlets in a surge protector, and two outlets on the table that were not covered. I observed staff/child interactions today. I observed children playing in their outside environment and free play. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on April 11, 2024. A sanitation inspection was completed January 16, 2025, with a Superior classification. The last fire inspection was conducted April 1, 2025. Program records and required postings were monitored. A fire drill was conducted on April 2, 2025. A shelter in place drill and lock down drill was documented on April 2, 2025. An outdoor inspection was documented on April 2, 2025. Staff files and children’s records were monitored per DCDEE procedures. The following violations were cited during today’s visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Upon arrival, I was invited in by a caregiver, Tequila Day. She informed me the owner, Jasmine Ross, was on her way. She arrived as I was monitoring the outdoor learning environment. Ms. Ross left around 11:30 for a personal appointment. The operator completed the last fire inspection on 4/2/2024 and was approved. The last sanitation inspection was completed on 1/17/24 with a superior rating. Ms. Day escorted me as I completed a walkthrough of the indoor and outdoor learning environment. I observed the children having a group time, playing with toys and materials easily accessible to them, ?? I observed the following violations today. I reviewed the items with Ms. Ross and provided technical assistance. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. "There were two holes near a tree by the fence. A branch of the tree was hanging low where a child could run into. These pose a potential safety hazards." 10A NCAC 09 .0601(a) 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. "The medical action plan for a child with allergy was dated August 2022. It has not been updated since then." .0801(b) The above violations must be corrected immediately. The current child care rules specifically state no blankets or other items can be used/placed with a sleeping infant aged 12 months or younger. 2.Weighted blankets or vests can potentially restrict a child’s movement; therefore, they are prohibited for use unless a therapist requests and approves the use. A therapist can only approve the use for children 13 months or older. 3.If the use of a weighted blanket or vest is something a facility wants to consider using, you’d need to contact me prior to use to ensure compliance with applicable requirements.
