This visit was conducted by Turkessha Casterlow, Childcare Consultant with Melanie Henderson, Director. Your program currently operates with a Five Star license. Restrictions include Daytime care only; Reduced ratio by one in each group from the seven-point level; Meets enhanced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 100% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during free play, playing in their outside learning environment, and teacher directed activities. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on December 16, 2025. A sanitation inspection was completed on May 4, 2026, with a Superior classification. The last fire inspection was conducted on February 20, 2026. Program records and required postings were monitored. A fire drill was conducted on April 14, 2026. A shelter in place and lock down drill was documented on March 12, 2026. An outdoor inspection was documented on April 8, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 9999 A violation was found for which there is no item number. The provider has not submitted all of the employed staff into ABCMS. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before May 20, 2026. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to turkessha.casterlow@dhhs.nc.gov. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit related to violations: •Updating the Automated Background Check Management System (ABCMS) in child care is critical to comply with federal law, ensure child safety through accurate, up-to-date background checks, and streamline staff onboarding. It enables efficient tracking of employee qualification status (valid for five years), manages rosters, and automates results from the NC SBI. The information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. This program operates with a five-star rated license. Upon arrival, I was greeted by the new administrator, Melanie Henderson. I informed Ms. Henderson of the purpose of my visit. The last annual compliance visit was conducted on 2/21/2024. The operator completed the last fire inspection on 2/28/2024 and the last sanitation inspection on 6/4/2024 with superior rating. I conducted a partial assessment of the childcare requirements today. Ms. Henderson accompanied me as I conducted a walkthrough of the indoor and outdoor learning environment. I reviewed five new staff files and program records. CPR and First Aid training was up to date for the staff members. I observed the children playing in the outdoor learning environment, drawing, playing with the toys and materials accessible to them, being comforted, eating lunch, and resting. There were two NCPREK classrooms, one private pay classroom, and the rest were Head Start and Early Head Start. I observed the following violations during today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. "There was no written authorization form for EpiPen in space B2, a three-year old room." 10A NCAC 09 .0803(1)(a & b) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. "In the infant room, the center's safe sleep policy was not posted." .0606(b) 1873 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with existing staff who care for children ages 0-5 within 30 days of adopting the policy. "There was no signed statement acknowledging she reviewed the Shaken Baby Syndrome and Abusive Head Trauma for one new staff member, Jasmine P." .0608 (d) The above violations must be corrected immediately. This childcare center is operating with a five-star license, serving NCPPREK, Head Start, and Early Head Start children. Upon arrival, I was greeted by the director, Joyce Peterson and the Education Specialist, Jennifer Snow. I informed Ms. Peterson the purpose of my visit. The last annual compliance visit was conducted on 2/14/2023. A fire inspection was last completed on 2/17/2023 and the sanitation inspection was last completed on 6/29/2023 with superior rating. Ms. Peterson accompanied me as I monitored the indoor and outdoor learning environment. I observed the children playing in the playground, going outside, finger painting, and playing with a variety of toys and materials. I observed the following violation during today’s visit and provided technical assistance on the items. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. The required information such as the name of the medication and the application instruction of the cream was not entered on the authorization form the parent completed. 10A NCAC 09 .0803(4)(6-9) The above violation must be corrected immediately.
