Name of Operation: Central Elementary YMCA After School Facility ID: 68000595 Consultant: MARCIA HUMPHREY Operation Type: Center Case Number: Visit Date: 12/17/2025 Number Present: 16 Completed Date: 12/17/2025 Age: From 5 To 11 Total Minutes: 90 Time In: 03:00 PM Time Out: 04:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit has been to complete the second temporary time period visit for this childcare center. When I arrived, I was greeted by the Group Leaders, Mrs. Whitted and Mr.Bradsher. I explained the purpose of the visit. Currently this center operates with a Temporary License, issued on September 8, 2025. The restrictions include daytime care only; School age children only. There is 21 children enrolled and 16 children were present after attendance. I observed the children in the auxiliary classroom involved in different free play centers with the group leaders engaged as they walked around and monitored the children. Indoor Learning Environment: I reviewed all program records requirements. The last fire drill was conducted November 2025. Records: The staff and training worksheet was completed prior to today’s visit; upon my review there were two violations observed. In addition, I reviewed 10% of the children’s records based on the number of children present in care and there were no concerns observed. The following violations were cited: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. This was not available for review for one employee currently employed and working with the children. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. This was not available for review for one employee currently employed and working with the children. .0701(a) In order to comply with NC Child Care Laws and Rules any violation cited must be corrected IMMEDIATELY. A Compliance letter must be sent by December 31, 2025. The letter must address each violation cited. I did talk to Mrs. Whitted regarding the QRIS, Pathways to the Stars. I talked with Ms. Crystal Carter, Director of Licensing in detail regarding the different pathways during the prior visit by phone and email so that she can begin thinking about what pathway she feels would be best for the facility. Based on the discussion the facility is choosing Pathway #2-Classroom and Instructional Quality. The curriculum that was chosen was Funny Daffer School Age. Ms. Crystal stated that she has taken the training for this curriculum and the YMCA is working on purchasing it and getting in out to the facility very soon. This curriculum has child assessments and coaching/training for administrators and lead teachers is required based on the number of Stars the facility will receive. The facility is working on making sure everything is in place in WORKS for The Education Standards Sheets (that currently apply to the program for administrator, program coordinator and group leaders), the Family and Community Engagement Standards, the Facility Continuous Quality Improvement (CQI) Plan and the Individual Continuous Quality Improvement/Professional Development Plan (questions 1-4 were observed completed during today's visit), this was already explained with Ms. Crystal. Ms. Humphrey asked Ms. Crystal to be thinking about and decide what Family and Community Engagement Standards would apply for the facility. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. All staff are required to complete the training within 90 days of hire. The training may be accessed via the DCDEE website at www.ncchildcare.nc.gov. Recognizing and Responding to Suspicions of Child Maltreatment is also available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. I discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff and prior to enrolling children to ensure accuracy. Ensure that all new staff members’ education information is current in the W.O.R.K.S. database. Each staff member must register in the WORKS system as soon as they are hired at this location. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment before the end of the temporary time period.
