This visit was conducted by Turkessha Casterlow, Child Care Consultant, with Marinda Morrow, Operator. Your program currently operates with a Four-Star license effective January 31, 2020. Restrictions include Daytime care only; Minium of 5 preschool children at any time; No more than 2 children less than 1 year old. The program’s compliance history was 99% prior to today’s visit. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health •Administering Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children playing in their outdoor learning environment and daily routines. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items were monitored today. A playground inspection was recorded July 2, 2025. A fire drill was recorded on July 2, 2025. A lock down for the facility was recorded July 2, 2025. There are no pets reported in the home. You stated that you do provide transportation for children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored one vehicle today. I observed a 2021, Kia, Sorento, tag # LLJ-6829. This vehicle was clean, in good repair, had working seatbelts, had adequate tire tread, and was current on inspection and insurance. Your next inspection date on this vehicle is due September 30, 2025, and the insurance is valid through December 23, 2025. The following violations were cited today: Violation Number Comment Rule 923 Operator did not keep records of monthly fire drills, quarterly lockdown or shelter-in-place drills, incident reports, incident logs, accurate attendance, emergency numbers, emergency preparedness and response plan, field trip/transportation permission, pet vaccinations records and/or playground inspections monthly outdoor safety checks for a minimum of one year. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. The program currently operates with a Four-Star Child-Care License that was issued on January 31, 2020. The last Annual Compliance Visit was conducted on May 9th, 2024. Upon arrival at home, I was greeted by the provider and her daughter (an additional provider) were present with five children. The children were observed enjoying free play, with an assortment of toys and materials. During the visit I monitored indoor and outdoor spaces. In the home the dining room/playroom is the primary care area and includes a variety of age-appropriate toys and materials. The outdoor play area also contained a variety of toys. The provider’s license and parental information were posted on a bulletin board in the living room. The provider’s file, children’s files, outdoor inspection, and fire drill reports were reviewed. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. I reviewed 1 newly enrolled child’s file. I also reviewed the additional caregiver’s file. The last fire drill was conducted on 10/14/24. The last shelter and lock down drills were on 9/6/24, the next one is due by 12/31/24. The outdoor inspection was done on 10/15/24. Two (2) violations were cited during today’s visit; one was corrected during today’s visit; the provider had a chance to ask questions to ensure staying in compliance. The program currently operates with a Four-Star Child-Care License that was issued on January 31, 2020. You, Ms. M. Morrow, assisted me with the visit. The last Annual Compliance Visit was conducted on May 24th, 2023. The home's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent (96%) as of 5/9/24. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed four (4) children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. The children at the family child care home were engaging in free play, circle time and morning snack. The caregiver was interacting and meeting the developmental needs of the children. The following items were observed in compliance during today's visit: license posted, and restrictions followed, summary of law posted, attendance taken daily, emergency medical care plan, emergency numbers posted, first aid poster posted, daily schedule, activity plan, storage of hazardous materials and substances, CPR/First Aid and SIDS training completed as required, and Criminal Record Checks completed as required. The last fire drill was conducted on 4/12/24. The last shelter-in-place and lock down drills were conducted on 3/20/24. The last Outdoor inspection was completed on 4/12/24. All staff and children’s files were monitored during today’s visit. A checklist was used to note the requirements I monitored today. Two violations were observed. The provider did ask questions pertaining to staying in compliance and technical assistance and consultation information was given.
