The following areas were monitored during the visit: supervision, staff/child ratio, CPR, first aid, special training, criminal background checks, Emergency Medical Care Plan, administering of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, and permit restrictions. The purpose of today’s visit was reviewed with you, Michelle Hines, Lead Teacher. You provided me with applicable staff and program records for review. NC Pre-K and Developmental Day requirements were monitored during today’s visit. Your program currently operates with a Five Star License effective December 6, 2022. The program’s compliance history was 99% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through of indoor and outdoor space was completed today. Staff and children were observed during nap, handwashing routines, snack, and departure. The most recent sanitation inspection for your facility was conducted on April 5, 2023. A Superior sanitation classification was issued with 2 demerits noted on the grade card. The most recent fire inspection for your facility was conducted on October 13, 2023. The most recent lead water testing results were completed on May 4, 2021. Lead testing must be completed every three years. You stated one new staff has been hired since your most recent annual compliance visit conducted on November 16, 2023. Violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 720 Elevated platforms did not have a guardrail or protective barrier appropriate for the height of the equipment and the ages of the children using the equipment. On the playground for preschool children, outdoor space #1, there was a five foot drop off from an elevated platform where a slide was removed on the large composite structure. .0605(o) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. The incident report dated 2/13/2024 did not have steps taken to prevent reoccurrence documented in the report. .0802 (e) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by April 24, 2024. Technical Assistance was provided on the following: - Elevated platforms shall have a guardrail or a protective barrier, depending upon the height of the platform and the age of children in accordance with this Paragraph that will have access to the piece of equipment. Guardrails shall prevent inadvertent or unintentional falls off the platform. The critical height for a platform with a guardrail is the top of the guardrail. Protective barriers shall prevent children from climbing over or through the barrier. The critical height for a platform with a protective barrier is the platform surface. All sides of platforms shall be protected except for the area which allows entry or exit. Measurements for the guardrails and protective barriers are as follows: Equipment used exclusively by children two years of age up to school age: (A) Guardrails - an elevated surface that is more than 20 inches and no more than 30 inches above the underlying surface shall have a guardrail. The minimum height of the top surface of the guardrail shall be 29 inches and the lower edge shall be no more than 23 inches above the platform. (B) Protective Barriers - an elevated surface that is more than 30 inches above the underlying surface shall have a protective barrier. The minimum height of the top surface of the protective barrier shall be 29 inches. -We reviewed requirements for incident reports during the visit. Staff corrected the violation by adding the steps that were taken to prevent the reoccurrence on the incident report during the visit. -Rated License Reassessment: To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. Your facility is in cohort two. The preparation year for cohort two will begin July 1, 2024. The reassessment year will be July 1, 2025, through June 30, 2026. More information will be forthcoming once your cohort year approaches. Consultation (the following items were reviewed with you today): -Sanitation inspection: 10A NCAC 09 .0304(b) states each center shall be inspected annually by an Environmental Health Specialist and receive an approved or superior rating in accordance with applicable sanitation requirements adopted by the Commission for Public Health as described in 15A NCAC 18A .2800. I suggested that you reach out to your local Environmental Health office and request a sanitation inspection to be conducted at your facility by the end of the month. I requested that you remain in contact with me on the status of obtaining a sanitation inspection. -School principals or an appropriate representative may visit http://www.cleanwaterforuskids.org/carolina to register for required asbestos and lead paint testing by May 1, 2024. Refer to the Raise NC Newsletter sent on December 7, 2023. -M. Lisa Wingate’s CPR and First Aid certification expires on November 30, 2023. Mrs. Wingate stated she completed CPR and First Aid training on November 7, 2023, however, she has not received her certification card yet. -Screen time must be documented on a cumulative log or the activity plan that shall be available for review by the Division. If the entire class participates in a screen time activity, this screen time activity may be recorded on your activity plan, including the length of time the screen time activity takes place. If the screen time activity is recorded on the activity plan, you do not need to include this activity on your screen time log for each individual child. -For any head injury that occurs at the facility, I recommend contacting the child’s parent/guardian immediately for the parent to determine if the child needs to be seen by a physician. -Remember to add all caregiving staff, including substitutes and extra-curricular staff, to the Public School Off-Sites Staff Records Verification form. -10A NCAC 09 .3005 CHILD HEALTH ASSESSMENTS (a) A health assessment shall be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program and the assessment may be no more than 12 months old at the time of program entry. The health assessment shall include the following: (1) physical examination; (2) updated immunizations; (3) vision screening; (4) hearing screening; and (5) dental screening. (b) Site-level administrators shall review all health assessment results at the time of the child’s entry into the program to determine whether the assessment includes specific instructions for identified health needs that may require physical or occupational or other therapies to support the child’s development and learning goals. If a screening is marked as “uncooperative” or “unable to complete” due to the child being uncooperative then this requirement is not in compliance. Caroline Howard, Principal, was present during the visit and signed the Visit Summary. This program currently operates with a Five star rated license effective December 6, 2022. The program’s compliance history was 99 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. A walk-through of licensed indoor and outdoor space was completed, and all areas were monitored. Staff and children were observed during free play activities indoors, handwashing routines and lunch. NC Pre-K and developmental day requirements were monitored during today’s visit. The most recent sanitation inspection for your facility was conducted on April 5, 2023. A superior sanitation classification was issued with 2 demerits noted on the grade card. The most recent fire inspection for your facility was conducted on October 13, 2023. The violations observed today were discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. One child was not signed out on October 17, 2023, and two children were not signed out on November 3, 2023. 10A NCAC 09 .0302(d)(4) 853 Incident logs were not completed and maintained as required.
