Name of Operation: YESS Learning Center - Lakecrest Facility ID: 41002041 Consultant: ERICKA BAILEY Operation Type: Center Case Number: Visit Date: 5/14/2026 Number Present: 25 Completed Date: 5/14/2026 Age: From 0 To 4 Total Minutes: 185 Time In: 10:40 AM Time Out: 01:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of this unannounced visit was to monitor compliance with applicable Child Care requirements. An Annual Compliance Monitoring Checklist for Centers was completed during this visit. This facility currently operates with a Four-Star License issued 2/1/2024. The last Annual Compliance Full visit was conducted on 06/04/2025. I was greeted by P. Miller, assistant director. K. Whitaker, the center director was out of town. There were 25 children were present during the visit. The last Sanitation inspection was conducted on 1/28/2026 and the facility received a “superior “classification with two demerits. The last Fire inspection occurred on and the 4/15/2026 facility was approved for day and nighttime care only. This facility was Current-Active on the Secretary of State Website. The Corporation was formed on 3/3/2015. I observed staff members attending to the individual needs of the children. The pre-K classroom were playing outdoors,transitioning to restroom/washing hands followed by eating lunch. The infants and toddlers were eating lunch and participating in open centers in the classroom. The two's - fours year olds were eating lunch and transitioning to nap time. I reviewed three children files and three staff files. There was one violation of Child Care requirements observed on this visit. This violation was corrected during the visit. Therefore, corrective action is not required on your part at this time. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the pre-k classroom there was a power outlet uncovered, this violation was corrected during the visit. 10A NCAC 09 .0604(c) You must continue to maintain compliance with applicable childcare requirements. Please remember it is your responsibility to know and follow all child care rules and requirements. Reminders: Some Criminal Background Checks are due this year. One is currently coming up in June 2026. Criminal Background Checks (CBC) • Start Early: Encourage staff to begin the process at least six months in advance to account for any delays. • Use ABCMS Portals: Regularly check the ABCMS Portal to track when staff qualification letters are due. • Set Reminders: Implement a system of automated reminders for upcoming CBC renewals. Maintain Compliance: Ensure all staff meet the NC Child care Law (General Statute 110-90.2), which mandates CBCs for all child care workers. Please check all trainings to ensure they are update and current so they do not become expired, some CPR/First training expiration dates are coming up this year. QRIS: Since your last visit in December 2025, we have discussed your plans for the QRIS transition. You planned on transitioning to Pathway 2 Classroom and Instructional Quality; I discussed with Ms. Miller that you will need to have all your employer’s information on Works before we can submit the Pathway Application.
